      • The Versatility of Cervical Vertebral Segmentation in Detection of Positional Changes in Patient with Long Standing Congenital Torticollis

        Background Congenital muscular torticollis (CMT) is a benign condition. With early diagnosis and appropriate management, it can be cured completely, leaving no residual deformity. However, long-standing, untreated CMT can lead to permanent craniofacial deformities and asymmetry.Methods Nineteen patients presented to the author with congenital muscular torticollis. Three dimensional computed tomography (3-D CT) scans was obtained upon patient’s admission. Adjustment of skull’s position to Frankfort horizontal plan was done. Cervical vertebral segmentation was done which allowed a 3D module to be separately created for each vertebra to detect any anatomical or positional changes.Results The segmented vertebrae showed an apparent anatomical changes, which were most noticeable at the level of the atlas and axis vertebrae. These changes decreased gradually till reaching the seventh cervical vertebra, which appeared to be normal in all patients. The changes in the atlas vertebra were mostly due to its intimate relation with the skull base, while the changes of the axis were the most significantConclusion Cervical vertebral segmentation is a reliable tool for isolation and studying cervical vertebral pathological changes of each vertebra separately. The accuracy of the procedures in addition to the availability of many software that can be used for segmentation will allow many surgeons to use segmentation of the vertebrae for diagnosis and even for preoperative simulation planning.

      • Craniofacial Fibrous Dysplasia: Retrospective Study on the Relationship Between the Tumor Volume Changes and the Circulating Serum Calcitonin and Serum Alkaline Phosphatase

        <P>Conclusions: Serum ALP may be useful as a laboratory test for follow-up of patients with isolated craniofacial fibrous dysplasia. However, it cannot represent the tumor's volume changes and 3-dimensional computed tomography scans with tumor volume measurement are mandatory for detecting significant volume changes during follow-up. Investigation of the serum calcitonin in the preoperative period is also recommended on a large scale because it was related to the presence of the tumor.</P>

      • Analysis of the correlation between deformational plagiocephaly and neurodevelopmental delay

        <P><B>Summary</B></P> <P><B>Background</B></P> <P>Deformational plagiocephaly (DP) refers to cranial asymmetry resulting from uneven external forces. A strong association exists between DP and developmental delay. We investigated the effect of DP severity on developmental delay.</P> <P><B>Methods</B></P> <P>Between 2010 and 2016, data from 155 patients with DP were reviewed retrospectively. Two indices were used to evaluate the deformation quantitatively: cranial index (CI) and cranial vault asymmetry index (CVAI). The Bayley Scales of Infant Development-II was used to evaluate the neurodevelopment of patients.</P> <P><B>Results</B></P> <P>According to the CI of the study population, 2 patients showed scaphocephaly, 12 showed mesocephaly, and 141 showed brachycephaly. For CVAI, 10 patients showed values of <3.5, 10 patients showed mild deformity (3.5–6.25), 27 patients showed moderate deformity (6.25–8.75), and 108 patients showed severe deformity. The means of the mental development index (MDI) and psychomotor development index (PDI) were 91.69 ± 16.8 and 92.28 ± 17.59, respectively; after the exclusion of patients with confounding factors, the values were 96.26 and 92.9, respectively. The Spearman correlation coefficients between MDI and CI and CVAI were −0.019662 and 0.118916, respectively, whereas for PDI, the values were −0.195428 and −0.012386, respectively.</P> <P><B>Conclusions</B></P> <P>There was a statistically significant neurodevelopmental delay in patients with DP. However, accelerated neurodevelopment was also encountered in many patients. MDI was found to be more affected by multiple confounding factors than PDI, whereas PDI was only affected by congenital anomalies. There was no definitive relationship between the severity of DP and the degree of developmental delay in our study group.</P>

        <P>Conclusions: Children with untreated congenital muscular torticollis show progressive anatomical changes of the cervical vertebrae which started at the age of 8 months. The severity of the deformity increased with the advance of age as well as with the severity of sternocleidomastoid tightness, which might result in permanent deformities.</P>

        Uses of the Revised Exchange Rate to Maintain Fair Value of Assets under Covid-19 Crisis

        The research aims to identify the effect of using the modified exchange rate in preserving the fair value of the assets, which are the most important economic variables in the Iraqi economy, which are the growth rate of the amount of money in circulation, the value of Iraq’s crude oil exports, the rate of inflation, the rate of growth of the gross domestic product, which expresses the rate of economic growth and the value of spending Government and the value of foreign direct investment in the presence of the COVID-19 crisis. The research found that the effect of the adjusted exchange rate value on both the rate of growth in the amount of money in circulation in Iraq and the growth rate of GDP in Iraq during the period (2004-2018) was not significant, while the significant effect of the value of the adjusted exchange rate on both the value of Iraqi exports Of crude oil in Iraq, the rate of inflation, the value of government spending, the value of foreign investment during the period (2004-2018). The effect of the revised exchange rate was evident in maintaining the fair value of assets in general and in light of the COVID-19 crisis in particular. The research recommended the necessity for the Central Bank of Iraq to use more effective monetary policies that would help increase the impact on the rate of growth of the amount of money in circulation as well as the rate of growth of gross domestic product, the need to work to diversify sources of income in the Iraqi economy and encourage development in all economic sectors and reduce dependence on oil as a main source For income in Iraq, work to develop economic and financial policies that encourage companies and individuals to invest in all non-oil economic sectors, the need to develop monetary policies that can confront emergency situations that may occur, activate the role of crisis management and increase their levels of efficiency.

      • The Reliability of Preoperative Simulation Surgery Planning for Distraction Osteogensis in Craniosynostosis Patients

        Background Craniosynostosis management using distraction osteogensis represent a challenge for surgeons due to the great variability of the skull deformity even within the same etiology. The ability to apply the simulation surgery for improving the preoperative planning for distraction osteogensis could improve the results.Planning and Simulation 14 patients presented with craniosynostosis had been subjected to simulation surgery prior to real surgery. 3D CT scans was obtained upon patient admission. Adjustment of all skull position to Frankfort horizontal plane was done. 3 different distraction osteogensis plans were done for each patient according to the skull morphology. For each plane, movement for each bone segment was done according to the pre-planned distraction vectors. Also the distances of distractions were pre-determined according to the cephalic index as well as brain volume. Intraoperatively, we choose the most appropriate plan for the patient by the surgeon. At the end of distraction, 3D CT scan was obtained, and was compared to the simulation plan. Also the distance and the direction of distraction was compared to that of the plan. Accordingly, the distance was almost matching that of the simulation surgery, however the vector of distraction was not matched.Conclusion Preoperative stimulation planning for craniosynostosis patient is very valuable tool in the surgical management of craniosynostosis patients.

        Prophylactic Effect of Resveratrol Against Ethinylestradiol-Induced Liver Cholestasis

        Estrogens, and particularly glucuronides such as ethinylestradiol (EE), have been shown to cause cholestasis in animal studies, by reducing bile acid uptake by hepatocytes. The aim of the present article was to investigate anticholestatic activity of resveratrol (RES) against liver cholestasis induced by EE in adult female rats. The daily oral administration of the RES at a concentration of 25 mg/kg body weight for 15 days to rats treated with EE (100 μg/kg body weight for 5 days) resulted in a significant protection against EE-induced decrease in both serum cholesterol and bile acid levels as well as against an increase of serum bilirubin concentration. The treatment also resulted in a significant increase in hepatic superoxide dismutase, glutathione peroxidase, glutathione reductase, and catalase activities as well as hepatic protein-bound and nonprotein sulfhydryl groups. RES inhibited serum alkaline phosphatase, alanine aminotransferase, pi-glutathione-S-transferase, gamma-glutamyl transpeptidase, and alpha-glutathione-S-transferase activities, as well as reduced serum tumor necrosis factor-alpha, nitric oxide, and hepatic malondialdehyde as compared to EE-treated rats. The results clearly suggest that RES has a powerful prophylactic action in cholestasis induced by EE. Taken together, RES has potential as a preventive and therapeutic agent for cholestasis and deserves clinical trial in the near future as an adjuvant therapy in women treated with estrogen.

      • CROPWAT 모델을 이용한 기후 변화에 따른 이라크의 밭작물 관개용수량 및 관개계획 모델링

        The objective of this study is to determine the impacts of potential climate change on daily and total crop water requirement (CWR) of citrus trees in Iraq (Three provinces; Baghdad, Sallah Aldeen, Wasit) using crop modeling (CROPWAT 8.0) in conjunction with the climate scenarios derived GCMs. Baseline climate is based on the 30 years period from 1989 to 2019 of the mean daily normal, and two time periods in the future years (2020 to 2050). The climate parameters of the temperature, precipitation, relative humidity, sunshine duration and wind speed were downscaled using the GCMs methods. To determine the future crop water requirements (CWR), the CROPWAT model (FAO, 1992) was used to simulate the daily and the season total CWR and irrigation requirements for the present and the future decades. Crop evapotranspiration (ET) and Reference crop evapotranspiration (ET) for crop were determined using CROPWAT 8.0. The enable efficient use of water and better irrigation practices irrigation scheduling as the supply of water through rainfall is limited in these areas. The study will show that effective water requirement and irrigation scheduling for the crop (citrus tree) in Iraq. the first results show for historical 30 years to Baghdad station the total Etc (1442.3mm/dec) and the total effective rainfall (143.5mm/dec) and the total water irrigation requirement is (1298.8mm/dec) for 12 months to citrus trees. Hence irrigation requirement is needed to satisfy crop water requirement. That lead us the effective water is enough to compensate crop water requirement and the how much we need water for irrigation during the season or during the growth and optimum yield and extension and increase the orchard (citrus trees) without any economic loss. The main problem irrigation in Iraq includes declining of water quality, shortage of water resources after new dams along two rivers in Turkey and Syria are developed, climate changes and low water productivity in the unit area. This study will give us the great knowledge in water requirement and irrigation scheduling to get a high production and horizontal expansion. The CROPWAT model allows the development of irrigation schedule for different management conditions and the calculation of scheme water supply for different areas under different crops.

      • Intelectin-1 and Endocrinological Parameters in Women with Polycystic Ovary Syndrome : Effect of Insulin Resistance

        Objectives: We aimed to evaluate the effect of insulin resistance (IR) on serum Intelectin-1 and endocrinological hormones levels in obese and non-obese women with and without polycystic ovary syndrome (PCOS) in Basrah, Iraq. Methods: From 124 women volunteers, 60 patients with primary and 64 patients with secondary, while 56 normal ovulatory women were taken as controls. Their fasting insulin hormone, intelectin-1, anti-Mullerian hormone, luteinizing hormone (LH), follicle stimulating hormone (FSH), prolactin (PRL), estradiol (E2) and testosterones (T) were determined by ELISA methods. BMI, glucose and quantitative insulin sensitivity check index as well as IR was determined by the homeostasis model assessment. Results: A significant changes (P<0.05) were seen in the level of homeostasis model assessment-IR, E2 and T. Levels of anti-Mullerian hormone, LH, LH/FSH ratio and prolactin were significantly (P<0.01) increased and level of intelectin-1 and E2/T ratio were significantly (P<0.01) decreased, while quantitative insulin sensitivity check index level was not significantly different (P>0.05) between the patients (1oPCOS and 2oPCOS) and control groups. On the other hand, our data reported that FSH level was significantly (P<0.05) lower in obese and higher in non-obese patients with PCOS as compared to control group. Conclusion: Levels of intelectin-1 and endocrinological hormones have significantly associated with body mass index, IR and physical activity in patients and normal groups and the strategies that can modulate levels of these parameters would improve metabolic disarrangements in women with PCOS.

        Synthesis, Anti-inflammatory, Analgesic, and Antibacterial Activities of Some Triazole, Triazolothiadiazole, and Triazolothiadiazine Derivatives

        This study is concerned with the synthesis of new 1,2,4-triazoles, 1,3,4-thiadiazoles, and 1,3,4-thiadiazines derivatives. Derivatives 3a-i were obtained by condensation of 4-amino-3-(4-pyridine)-5-mercapto-1,2,4-triazole 1 with the appropriate aldehyde. Compounds 4a-i were synthesized in a one pot reaction involving compounds 3a-i, formaldehyde, and morpholine. Condensation of compound 1 with the appropriate acids or 4-substituted phenacyl bromide gave compounds 6a-d and 8a-f respectively. The chemical structures of the newly synthesized derivatives were elucidated using different spectral and elemental methods of analysis. All compounds were evaluated for their anti-inflammatory activity and the most potent derivatives were tested for their analgesic activity using indomethacin as a reference drug. In addition, ulcerogenicity and LD_50 for the most active compounds were evaluated. Moreover, the antibacterial activities of the newly synthesized derivatives were investigated.
