Malaria is associated with high rates of morbidity and mortality worldwide, particularly in Africa, Southeast Asia and South America. Nonetheless, several cases of malaria have been reported in Western countries involving travelers from endemic areas, though very few involve pregnant women. In this article, we report a case of a young woman born in Sierra Leone who had been living in Italy for two years. She was admitted to our hospital with malaise; worsening of her condition led to Plasmodium falciparum infection diagnosis early during her hospital stay, as well as an urgent cesarean delivery. We briefly discuss the features of malaria in pregnancy, the difficulties associated with early diagnosis, and the possible fetal and maternal implications, and also consider how the disease may affect anesthetic management.
The Burden of Agoraphobia in Worsening Quality of Life in a Community Survey in Italy

Objective Current nosology redefined agoraphobia as an autonomous diagnosis distinct from panic disorder. We investigated the life-time prevalence of agoraphobia, its association with other mental disorders, and its impact on the health-related quality of life (HR-QoL). Methods Community survey in 2,338 randomly selected adult subjects. Participants were interviewed with the Advanced Neuropsy-chiatric Tools and Assessment Schedule (ANTAS), administered by clinicians. The diagnoses were based on the ICD-10 criteria. The Short-Form Health Survey (SF-12) was used to quantify HR-QoL. Results In the sample, 35 subjects met the criteria for agoraphobia (1.5%), with greater prevalence among women (2.0%) than men (0.9%): odds ratio (OR) 2.23; 95% CI: 1.0-5–2. Agoraphobia was more often seen among those with (n=26; 1.1%) than without (n=9; 0.4%) panic disorder: OR=8.3; 2.9–24.4. Co-morbidity with other mental disorders was substantial. The mean score of SF-12 in people with agoraphobia was 35.2±7.8, with similar levels of HR-QoL in people with (35.3±7.9) or without (34.8±7.3) panic disorder: ANOVA: F(1;33)=0.0; p=1.00. Conclusion One out of seventy people may suffer from agoraphobia in their lifetime. The attributable burden in terms of HR-QoL is substantial and comparable to the one observed for chronic mental disorders such as major depression, post-traumatic stress disorder, or obsessive-compulsive disorder. Reactivity of hydroxyl radicals with neonicotinoid insecticides: mechanism and changes in toxicity

The reactivity of hydroxyl radicals ($HO^{\cdot}$) towards three neonicotonoid insecticides, namely imidacloprid, thiacloprid and acetamiprid was investigated. These radicals were generated by photolysis of $H_2O_2$ solutions. Flash photolysis experiments were used to determine the rate constants of $5.5{\times}10^{10}M^{-1}s{-1}$, $6{\times}10^{10}M^{-1}s^{-1}$, and $7.5{\times}10^{10}M^{-1}s^{-1}$, for the reactions of $HO^{\cdot}$ with acetamiprid, imidacloprid, and thiacloprid, respectively. Continuous irradiation experiments in the absence and presence of $H_2O_2$ allowed the identification and toxicity evaluation of the primary photo- and oxidation products of the insecticides. In all cases, the less toxic 6-chloronicotinic acid was found to be the major product at higher degrees of oxidation. The results reported here indicate that the half life of the insecticides due to their reaction with $HO^{\cdot}$ radicals in natural aquatic reservoirs may vary between 5 h and 19 days, and therefore the hydroxyl radical-mediated oxidation may be a significant abiotic elimination route. However, elimination of the insecticide under such conditions might not improve the quality of the contaminated water, as the primary products of degradation still show considerable toxicity to Vibrio fischeri assays.

Thermophilic fungi produce thermostable enzymes which have a number of applications, mainly in biotechnological processes. In this work, we describe the characterization of a protease produced in solidstate (SSF) and submerged (SmF) fermentations by a newly isolated thermophilic fungus identified as a putative new species in the genus Myceliophthora. Enzyme-production rate was evaluated for both fermentation processes, and in SSF, using a medium composed of a mixture of wheat bran and casein, the proteolytic output was 4.5-fold larger than that obtained in SmF. Additionally, the peak of proteolytic activity was obtained after 3 days for SSF whereas for SmF it was after 4 days. The crude enzyme obtained by both SSF and SmF displayed similar optimum temperature at 50°C, but the optimum pH shifted from 7 (SmF) to 9 (SSF). The alkaline protease produced through solid-state fermentation (SSF), was immobilized on beads of calcium alginate, allowing comparative analyses of free and immobilized proteases to be carried out. It was observed that both optimum temperature and thermal stability of the immobilized enzyme were higher than for the free enzyme. Moreover, the immobilized enzyme showed considerable stability for up to 7 reuses.
SHELS: COMPLETE REDSHIFT SURVEYS OF TWO WIDELY SEPARATED FIELDS

<P>The Smithsonian Hectospec Lensing Survey (SHELS) is a complete redshift survey covering two well-separated fields (F1 and F2) of the Deep Lens Survey (DLS). Both fields are more than 94% complete to a Galactic extinction corrected R-0 = 20.2. Here, we describe the redshift survey of the F1 field centered at R.A.(2000) = 00(h)53(m)25(8).3 and decl.(2000) = 12 degrees 33'55'; like F2, the F1 field covers similar to 4 deg(2). The redshift survey of the F1 field includes 9426 new galaxy redshifts measured with Hectospec on the MMT (published here). As a guide to future uses of the combined survey, we compare the mass metallicity relation and the distributions of D(n)4000 as a function of stellar mass and redshift for the two fields. The mass-metallicity relations differ by an insignificant 1.6 sigma. For galaxies in the stellar mass range 10(10)-10(11) M-circle dot, the increase in the star-forming fraction with redshift is remarkably similar in the two fields. The seemingly surprising 31%-38% difference in the overall galaxy counts in F1 and F2 is probably consistent with the expected cosmic variance given the subtleties of the relative systematics in the two surveys. We also review the DLS cluster detections in the two fields: poorer photometric data for F1 precluded secure detection of the single massive cluster at z = 0.35 that we find in SHELS. Taken together, the two fields include 16,055 redshifts for galaxies with R-0 <= 20.2 and 20,754 redshifts for galaxies with R <= 20.6. These dense surveys in two well-separated fields provide a basis for future investigations of galaxy properties and large-scale structure.</P>

Background/Aims: Patients with genotype 3 hepatitis C virus (G3-HCV) cirrhosis are very difficult to treat compared to patients with other HCV genotypes. The optimal treatment duration and drug regimen associated with ribavirin (RBV) remain unclear. To evaluate the efficacy and safety of daclatasvir (DCV)/sofosbuvir (SOF) plus a flat dose of 800 mg RBV (flat dose) compared to DCV/SOF without RBV or DCV/SOF plus an RBV dose based on body weight (weight-based) in G3-HCV patients with compensated or decompensated cirrhosis. Methods: We analyzed data for 233 G3 cirrhotic patients. Of these, 70 (30%), 87(37%) and 76 (33%) received SOF/DCV, SOF/DCV/RBV flat dose, and SOF/DCV/RBV weight-based dose, respectively. Treatment duration was 24 weeks. Sustained virological response (SVR) was evaluated at week 12 posttreatment (SVR12). Results: Overall, SVR12 was achieved in 220 out of 233 patients (94.4%). The SVR12 rate was lower in the DCV/SOF group than in the DCV/SOF/RBV flat-dose group and the DCV/SOF/RBV weight-based group (87.1% vs 97.7% and 97.4%, respectively, p=0.007). A higher incidence of anemia occurred in the DCV/SOF/RBV weight-based group compared to those in the other two groups (p<0.007). Conclusions: We found that the DCV/SOF/RBV flat-dose regimen is an effective treatment in terms of efficacy and safety in patients with G3-HCV compensated or decompensated cirrhosis. Therefore, antiviral regimens without RBV should be restricted only to naïve patients with G3-HCV compensated cirrhosis who have a clear contraindication for RBV.
