        신체활동이 지체장애인의 자아정체감에 미치는 영향

        The purpose of this study was self-identity of individuals with physical disabilities participating in physical activity. Subjects included 198(male: 143 female: 58) with individuals with physical disabilities. The questionnaire that was developed by Park Ah-Chung(1996) was used self-identity scale of Korean. It was revised by Lee Sung-Guk(1999) which consisted of 6 factors, 48 items. Construct validity of the instrument was checked by Factor analysis. Reliability of questions was confirmed by Cronbach`s α value(.783). The data analysis were Independent t-test, one-way ANOVA, and multiple regression analysis. The results were as follows; First, job and incomes had significant effects on self-identity of individuals with physical disabilities. Second, physical instructors, mass media, and friends had significant effects on self-identity of individuals with physical disabilities. Third, physical activity period, group exercise, and athletes with disabilities had significant effects on self-identity of individuals with physical disabilities. Fourth, high understanding of disability traits significant difference than low understanding of disabilities traits on self-identity of individuals with physical disabilities. These results implied that individuals with physical disabilities participation in physical activity had significant effects on their self-identity according to socio-demographic variables, significant other, type of physical activity, participation type of physical activity, and understanding of disability traits.

        p38-Targeted inhibition of interleukin-12 expression by ethanol extract from <i>Cordyceps bassiana</i> in lipopolysaccharide-activated macrophages

        <P><I>Cordyceps</I> species have been known as ethnopharmacologically valuable mushroom in Korea, China, and Japan. This plant has been reported to exhibit a variety of pharmacological activities such as antioxidative, anticancer, anti-inflammatory, antidiabetic, and antiobesity effects. Although numerous pharmacological potentials of <I>Cordyceps</I> spp. have been demonstrated, immunomodulatory effect of <I>Cordyceps bassiana</I> has not been published yet. To evaluate its immunomodulatory activity, macrophages activated by lipopolysaccharide (LPS) were employed and the production of interleukin-12 (IL-12) was explored in terms of understanding its molecular inhibitory mechanism. Seventy percent of ethanol extract from <I>Cordyceps bassiana</I> (Cb-EE) was able to suppress the expression of IL-12, a cytokine regulating interferon-γ款 (IFN-γ款)-producing T helper type 1 (Th1) polarization response, at the transcriptional levels. The inhibitory effect of Cb-EE seemed to be due to activator protein-1 (AP-1) translocation inhibition, according to immunoblotting analysis with nuclear fraction and luciferase assay. In agreement with this, Cb-EE strongly suppressed the phosphorylation of p38, a prime signal to stimulate AP-1 translocation and IL-12 production, strongly suppressed by SB203580, a p38 inhibitor. Furthermore, this extract also suppressed IFN-γ款 production in both phytohemaglutinin A and LPS-activated splenocytes. Our results suggest that Cb-EE can be applied as a Th1 response regulatory herbal medicine.</P>

      • Biomimetic Liquid-Sieving through Covalent Molecular Meshes

        <P>The porin pores of biological cell membranes enable molecules to be sieved out selectively while water molecules traverse the channel in a single file. Imitating this streaming mechanism is a promising way to create artificial liquid-sieving membranes, but ultrathin molecular pores need to be produced in a large membrane format to be functional under high transmembrane pressures. Here we show that a membrane composed of a covalent molecular mesh can filter mixtures of small molecules in a liquid by the porin-like mechanism. Tetrahedral network formers are polymerized layer-by-layer on a nanoporous substrate to yield a thin layer of a covalent molecular network containing an array of molecular meshes grown by a pore-limited mechanism. Each of the meshes exhibits high water permeability, estimated to be greater than 2500 Lm(-2) h(-1). Glucose or larger molecules are selectively sieved out while the solvent and solutes smaller than glucose traverse the mesh.</P>

        A 60-GHz LTCC SiP with Low-Power CMOS OOK Modulator and Demodulator

        In this paper, a 60 GHz LTCC SiP with low-power CMOS OOK modulator and demodulator is presented. The 60 GHz modulator is designed in a 90-nm CMOS process. The modulator uses a current reuse technique and only consumes 14.4-mW of DC power in the on-state. The measured data rate is up to 2 Gb/s. The 60 GHz OOK demodulator is designed in a 130nm CMOS process. The demodulator consists of a gain boosting detector and a baseband amplifier, and it recovers up to 5 Gb/s while consuming low DC power of 14.7 mW. The fabricated 60 GHz modulator and demodulator are fully integrated in an LTCC SiP with 1 by 2 patch antenna. With the LTCC SiP, 648 Mb/s wireless video transmission was successfully demonstrated at wireless distance of 20-cm.

      • Long-term prognostic impact of osteopontin and Dickkopf-related protein 1 in patients with hepatocellular carcinoma after hepatectomy

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>New biomarkers are essential for improving the prediction of the survival and prognosis of patients with hepatocellular carcinoma (HCC). Alpha-fetoprotein (AFP) is the most widely used biomarker, but the low sensitivity and specificity limit its clinical applications. The diagnostic and prognostic capabilities of osteopontin (OPN), dickkopf-related protein 1 (DKK1), and a combination of these biomarkers are being studied.</P> <P><B>Methods</B></P> <P>From January 2006 to December 2008, patients undergoing hepatectomy for HCC were screened and their serum stored in tumor banks was analyzed. The serum was used to investigate OPN and DKK1 levels by enzyme-linked immunosorbent assay(ELISA). In the paraffin block, the status of OPN and DKK1 positivity was assessed using immunohistochemistry(IHC).</P> <P><B>Results</B></P> <P>A total of 60 patients were enrolled. The optimal cut-off level for survival was identified as 3.0 ng/mL and 5.2 ng/mL of OPN and DKK1, respectively. In multivariate analysis, a high OPN level was the only significant prognostic factor for overall survival [hazard ratio3.79, p = .017]. Considering a combination of AFP (cut-off level, 200 ng/mL) and OPN/DKK1, the patients with high AFP and OPN/DKK1 levels showed significantly lower overall survivals than those with low AFP, high AFP, and low OPN/DKK1 levels (p = .0091 for the AFP and OPN group, p = .0344 for the AFP and DKK1 group). Comparison between IHC and ELISA results for OPN and DKK1 levels did not reveal any significant correlation.</P> <P><B>Conclusions</B></P> <P>Serum OPN and DKK1 levels of HCC patients could be considered as novel biomarkers showing prognostic significance after hepatectomy based on long-term survival data.</P>

        Process-Variation-Adaptive Charge Pump Circuit using NEM (Nano-Electro-Mechanical) Relays for Low Power Consumption and High Power Efficiency

        For some low-frequency applications such as power-related circuits, NEM relays have been known to show better performance than MOSFETs. For example, in a step-down charge pump circuit, the NEM relays showed much smaller layout area and better energy efficiency than MOSFETs. However, severe process variations of NEM relays hinder them from being widely used in various low-frequency applications. To mitigate the process-variation problems of NEM relays, in this paper, a new NEM-relay charge pump circuit with the self-adjustment is proposed. By self-adjusting a pulse amplitude voltage according to process variations, the power consumption can be saved by 4.6%, compared to the conventional scheme without the self-adjustment. This power saving can also be helpful in improving the power efficiency of the proposed scheme. From the circuit simulation of NEM-relay charge pump circuit, the efficiency of the proposed scheme is improved better by 4.1% than the conventional.

        Factors Associated with Clinical Outcomes of Palliative Stenting for Malignant Colonic Obstruction

        Background/Aims: Self-expandable metal stents (SEMSs) can be applied to relieve colorectal obstruction secondary to incurable primary colorectal cancer or extracolonic malignancy. We aimed to identify factors associated with clinical success and the reintervention-free survival (RFS) after palliative stenting. Methods: Cases of palliative SEMS placement between 2005 and 2019 were retrieved from the institutional database and reviewed retrospectively. Logistic regression and log-rank testing followed by Cox proportional hazard analyses were performed to investigate the predictors of the clinical success of palliative stenting and factors associated with RFS, respectively. Results: A total of 593 patients underwent palliative stenting for malignant colonic obstruction (MCO). The technical and clinical success rates were 92.9% and 83.5%, respectively. Peritoneal carcinomatosis was a predictor of clinical failure (odds ratio, 0.33; 95% confidence interval [CI], 0.17 to 0.65) in the multivariate analysis. Peritoneal carcinomatosis (hazard ratio [HR], 2.48; 95% CI, 1.69 to 3.64) and stent expansion >90% on day 1 (HR, 1.62; 95% CI, 1.05 to 2.50) were associated with a shorter RFS. Neither clinical success nor RFS was associated with extracolonic malignancy. Re-obstruction, stent migration, and perforation were responsible for most reinterventions after clinically successful palliative stenting. Conclusions: In patients requiring palliative stenting for MCO, peritoneal carcinomatosis was associated with both clinical failure and short RFS. Stent expansion >90% on postprocedural day 1 was another predictor of a short RFS after clinically successful stenting. A large prospective study is warranted to establish factors associated with RFS after successful palliative stenting for MCO. (Gut Liver 2021;15:579-587)

        Surface Modification of Cast Inconel 740 Superalloy by Heat-Assisted Friction Stir Processing

        Cast In740 Ni-based alloy with large grains generally show remarkable high temperature strength. However, this alloy is still have insufficient surface microstructure-dependent properties. In this study, for improvement of surface properties, surface modification of a cast In740 Ni-based superalloy was successfully performed by friction stir processing using a conventional two-horsepower milling machine and by additional heating to facilitate plastic flow of the hard alloy. Without using a high-power heavy stirring machine, a notable reduction in grain size of 2.9 μm was achieved and a corresponding 30% increase in Vickers hardness was observed. The microstructure in the stir zone was analyzed in terms of the grain size and precipitate distribution. The result of the potential dynamic polarization test and in-situ acoustic emission monitoring show that electrochemical corrosion resistance was improved by this surface modification process.

        Dementia Incidence Rate Before and After Implementing the National Responsibility Policy for Dementia Care in Patients With Vascular Risk Factors in Korea

        Background and Purpose: The National Responsibility Policy for Dementia Care was implemented in September 2017 in Korea. This study aimed to compare dementia incidence in Seoul and Gangwon-do before and after the implementation of this policy. Methods: We extracted insurance claim data from the Korean Health Insurance Review and Assessment Service for people diagnosed with diabetes, hypertension, or dyslipidemia for the first time in Seoul and Gangwon-do, Korea. We defined two enrollment groups based on the policy implementation date: 1) January 1, 2015 to December 31, 2016 (Index 1, pre-implementation), and 2) January 1, 2017 to December 31, 2018 (Index 2, post-implementation). Each group was followed up for 1 year from the time of enrollment. Then, we calculated hazard ratios to compare the incidence of dementia between the two groups, and between Seoul and Gangwon-do. Results: In Seoul, the incidence of dementia was significantly lower in Index 2 than in Index 1 (hazard ratio [HR], 0.926; 95% confidence interval [CI], 0.875–0.979). However, the incidence rate did not differ between the 2 groups (HR, 1.113; 95% CI, 0.966–1.281) in Gangwon-do. In Index 1, the incidence of dementia did not differ between Seoul and Gangwon-do (HR, 1.043; 95% CI, 0.941–1.156), but in Index 2, was significantly higher in Gangwon-do than in Seoul (HR, 1.240; 95% CI, 1.109–1.386). Conclusions: After implementing the National Responsibility Policy for Dementia Care, the dementia incidence rate decreased significantly in Seoul, consistent with other studies, but not in Gangwon-do.
