        Technical and Clinical Outcomes After Colorectal Stenting in Malignant Large Bowel Obstruction: A Single Centre Experience

        Purpose: Malignant large bowel obstruction is a surgical emergency that requires urgent decompression. Stents are increasingly being used, though reported outcomes are variable. We describe our multidisciplinary experience in using stents to manage malignant large bowel obstruction. Methods: All patients undergoing colorectal stent insertion for acute large bowel obstruction in a teaching hospital were included. Outcomes, complications, and length of stay (LOS) were recorded. Results: Over a 7-year period, 73 procedures were performed on 67 patients (37 male, mean age 76 years). Interventional radiology was involved in all cases. Endoscopic guidance was required in 24 cases (33%). In 18 patients (27%), treatment intent was to bridge to elective surgery; 16 had successful stent placement; all had subsequent curative resection (laparoscopic resection 8/18, primary anastomosis 14/18). Overall LOS, including both index admission and elective admission, was 16.4 days. Treatment intent was palliative in 49 patients (73%). In this group, stents were successfully placed in 41/49 (84%). Complication rate within 30 days was 20%, including perforation (2 patients), per rectal bleeding (2), stent migration (1), and stent passage (5). Nineteen patients (39%) required subsequent stoma formation (6 during same admission, 13 during subsequent admission). Overall LOS was 16.9 days. Conclusion: In our experience colorectal stents can be used effectively to manage malignant large bowel obstruction, with only selective endoscopic input. As a bridge to surgery, most patients can avoid emergency surgery and have a primary anastomosis. In the palliative setting, the complication rate is acceptable and two-thirds avoid a permanent stoma.

        A Review of the Role of Carcinoembryonic Antigen in Clinical Practice

        Carcinoembryonic antigen (CEA) is not normally produced in significant quantities after birth but is elevated in colorectal cancer. The aim of this review was to define the current role of CEA and how best to investigate patients with elevated CEA levels. A systematic review of CEA was performed, in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Studies were identified from PubMed, Cochrane library, and controlled trials registers. We identified 2,712 papers of which 34 were relevant. Analysis of these papers found higher preoperative CEA levels were associated with advanced or metastatic disease and thus poorer prognosis. Postoperatively, failure of CEA to return to normal was found to be indicative of residual or recurrent disease. However, measurement of CEA levels alone was not sufficient to improve survival rates. Two algorithms are proposed to guide investigation of patients with elevated CEA: one for patients with elevated CEA after CRC resection, and another for patients with de novo elevated CEA. CEA measurement has an important role in the investigation, management and follow-up of patients with colorectal cancer.

      • Vibroacoustic analysis of stiffened functionally graded panels in thermal environments

        Functionally graded materials (FGMs) have gained substantial attention from researchers due to their exceptional strength and thermal resistance. Their utilization in the aviation and automobile industries has significantly improved the efficiency of various structural components. Moreover, stiffened panels find wide applications in aerospace and automobile structures and these panels are frequently exposed to extreme environments. It is from this perspective that our research is focused on analysing the vibroacoustic response of stiffened functionally graded panels subjected to external dynamic excitations in a thermal environment. In the present research work, a finite element model is developed to conduct the dynamic analysis of functionally graded stiffened panels using the first-order shear deformation theory. Subsequently, a boundary element based model is also developed and coupled with the finite element model to investigate the sound radiation behaviour of those panels in a thermal environment. The material properties of FG stiffened panels are considered as temperature dependent, while the thermal environment is assumed to be acting as linearly varying through the panel’s thickness. The present investigation aim to compare the vibroacoustic responses of different panels due to stiffener orientations, material compositions, power law indices and plate thicknesses at various temperatures. The research findings highlight the significant impact of addition of stiffeners, its orientation and material compositions on the sound radiation characteristics of these panels under thermal environments. The present numerical model can easily be employed for analysing the sound radiation behaviour of other types of flat or curved stiffened panels having arbitrary geometry and boundary conditions.

        Effect of Social Deprivation on the Stage and Mode of Presentation of Colorectal Cancer

        Purpose: Based in a hospital serving one of the most deprived areas in the United Kingdom (UK), we aimed to investigate, using the Indices of Deprivation 2010, the hypothesis that deprivation affects the stage and mode of presentation of colorectal cancer. Methods: All newly diagnosed patients with colorectal cancer presenting to a District General Hospital in the UK between January 2010 and December 2014 were included. Data were collected from the Somerset National Cancer Database. The effect of social deprivation, measured using the Index of Multiple Deprivation Score, on the stage and mode of presentation was evaluated utilizing Microsoft Excel and IBM SPSS ver. 22.0. Results: A total of 701 patients (54.5% male; mean age, 76 years) were included; 534 (76.2%) underwent a surgical procedure, and 497 (70.9%) underwent a colorectal resection. Of the patients undergoing a colorectal resection, 86 (17.3%) had an emergency surgical resection. Social deprivation was associated with Duke staging (P = 0.09). The 90-day mortality in patients undergoing emergency surgery was 12.8% compared to 6.8% in patients undergoing elective surgery (P = 0.06). No association was found between deprivation and emergency presentation (P = 0.97). A logistic regression analysis showed no increase in the probability of metastasis amongst deprived patients. Conclusion: This study suggests an association between deprivation and the stage of presentation of colorectal cancer. Patients undergoing emergency surgery tend to have a higher 90-day mortality rate, although this was not related to deprivation. This study highlights the need to develop an individual measure to assess social deprivation.
