Acute Generalized Exanthematous Pustulosis

Acute generalized exanthematous pustulosis(AGEP) has symptoms of abrupt onset of a widespread pustular eruption on an erythematous. base. Most cases appear to be related to drug reactions, mainly antibiotics, but viral infections and hypersensitivity to mercury may cause AGEP. The essential features of AGEP include. (1) numerous(several dazen) small((5mm), mostly non follicular pustules arising on a widespread erythema:purpura and target-like lesions may be associated; (2) histology showing intraepidermal or subcorneal pustules associated with one or more of the following.clermal edema, vasculitis, perivascular eosinophils, or focal necrosis of keratinocytes; (3) fever (over 38C); (4) neutrophilia, and (5) acute evolution with spontaneous resulotion of pustules within 15 days. We report a case of AGEP which presented with widespread tiny pustules on the whole body except the face, palms and soles. There were petechia, purpura, and vesiculobullous lesions on the axilla, popliteal fossa and upper abdomen. A biopsy specimen from a pustule showed subcorneal pustules with perivascular polymorphous cellular infiltration, marked dermal edema and necrotic keratinocytes. There was complete resolution of the lesions within 10 days. (Kor J Dermatol 1997;35(l): 160-164)

문신 제거에 사용한 Alexandrite 레이저의 효과

Background: Until the recent development of the Q-switched lasers, it was impossible to remove tattoos without causing scarring and sometimes disfiguring. Variations in wavelengths and pulse widths may result in different clinical effects or risks, Objectiye : The purpose of this study was to investigate the therapeutic effects of the alexandrite laser in removing t;ttoos clinically, histologically and ultrastructually. Methods . Fourty-two patients with gray, black, and blue-black colored tattoos were treated with fluences ranging from 6.5 to 8.0 J/crt1 at a minimal interval of 6 weeks. We took pictures of each tattoo prior to treatment and each treatment session in order to evaluate clinical irnprovement. Biopsy specimens were obtained from 8 patients, on prearranged times.' before, immediately after, one week after an3 6 weeks after irradiation. Results . The results if the treatment of 42 patients with tattoos were analyzed. After two treatment sessions, 89% of patients showed 51% to 95% improvement. Transient hyperpigmentation and hypopigmentation occurred in 7 patient.s. However, skin atrophy, hypertrophic scar or infection did not occur. Ir a biopsy specimen taken 6 weeks after irradiation on light microscopy, most of tattoo granules disappeared without collagen fiber change. However, macrophage engulfing t,attoo granules and a minute amount of tattoo granules still remained in the dermis on electron microscopy, Conclusion : The .alexandrite laser is considered to be a safe and effective treatment rnodality for removal of gray, black, and blue-black tattoo pigments. We report an annular type of subacute cutaneous lupus erythematosus(SCLE) which is rare in Korea, A 23-year-old female had erythematous annular rashes with a mild itching sensation on the face, trunk and both extremities. She also had painless oral mucosal ulceration, diffuse hair loss and mild arthralgia. Serologic abnormalities of the patient were positive anti-Ro Ab, ANA and Anti-ds-DNA Ab. The histopathologic findings showed vacuolar degenerations in the basal cell layer, mononuclear cell infiltrate in the upper dermis, and dermal edema. Direct IF revealed granular deposition of IgG and IgA at the dermo-epidennal junction.

The use of rubber gloves has increased in recent years. At the same time, there has been a progressive increase of incidence of occupational and nonoccupational sensitization to rubber components. Chromate sensitivity has been reported as a very common combination in the rubber allergic building workers. We report a case of contact dermatitis to rubber glove and cement in a 69-year-old man who presented erythematous patches with severe itching on both hands, especially on the lateral sides of fingers. On patch tst, he showed positive reactions to thiuram mix, mercaptob-enzothazole, potassium dichromate and rubber glove as is. The patient was treated with topical and sysmetic corticosteroid and antihistamine with a good clinical responses.
