        전산하 단층촬영 투시하 성상 신경절의 경피적 고주파 열 응고술

        Stellate ganglion block is a well established method for the management of certain pain syndromes (e.g., chronic regional pain syndrome, facial pain) in the cervicothoracic region and upper extremity. The stellate ganglion resides between the C7 transverse process and the head of the first rib. Anesthetic injections for the stellate ganglion block are typically made at the level of the transverse process of either the C6 or C7 vertebrae to avoid the pleura, vessels, and nerve roots. Method of positioning the needle tip directly at the ganglion has been described, but are problematic because of the risk of injury to or injection into adjacent structures. It is necessary to know the exact anatomic position of the stellate ganglion when permanent blockade is required by means of radiofrequency thermocoagulation. Whereas fluroscopy shows only bony feature, computerized tomography also images nerves, vessels, and lung, allowing accruate needle placement. We report a case of the percutaneous radiofrequency thermocoagulation of the stellate ganglion after computerized tomography-guided localization.

        모델 예측변수들을 이용한 집중호우 예측 가능성에 관한 연구

        In order to determine the prediction possibility of heavy rainfall, a variety of analyses was conducted by using three-dimensional data obtained from Korea Local Analysis and Prediction System (KLAPS) re-analysis data. Strong moisture convergence occurring around the time of the heavy rainfall is consistent with the results of previous studies on such continuous production. Heavy rainfall occurred in the cloud system with a thick convective clouds. The moisture convergence, temperature and potential temperature advection showed increase into the heavy rainfall occurrence area. The distribution of integrated liquid water content tended to decrease as rainfall increased and was characterized by accelerated convective instability along with increased buoyant energy. In addition, changes were noted in the various characteristics of instability indices such as K-index (KI), Showalter Stability Index (SSI), and lifted index (LI). The meteorological variables used in the analysis showed clear increases or decreases according to the changes in rainfall amount. These rapid changes as well as the meteorological variables changes are attributed to the surrounding and meteorological conditions. Thus, we verified that heavy rainfall can be predicted according to such increase, decrease, or changes. This study focused on quantitative values and change characteristics of diagnostic variables calculated by using numerical models rather than by focusing on synoptic analysis at the time of the heavy rainfall occurrence, thereby utilizing them as prognostic variables in the study of the predictability of heavy rainfall. These results can contribute to the identification of production and development mechanisms of heavy rainfall and can be used in applied research for prediction of such precipitation. In the analysis of various case studies of heavy rainfall in the future, our study result can be utilized to show the development of the prediction of severe weather.

        한반도·동아시아 지역의 실시간 가뭄 감시 및 전망 시스템 개발

        The objectives of this study are to develop a real-time drought monitoring and prediction system on the East Asia domain and to evaluate the performance of the system by using past historical drought records. The system is mainly composed of two parts: drought monitoring for providing current drought indices with meteorological and hydrological conditions; drought outlooks for suggesting future drought indices and future hydrometeorological conditions. Both parts represent the drought conditions on the East Asia domain (latitude 21.15~50.15o, longitude 104.40~149.65o), Korea domain (latitude 30.40~43.15o, longitude 118.65~135.65o) and South Korea domain (latitude 30.40~43.15o, longitude 118.65~135.65o), respectively. The observed meteorological data from ASOS (Automated Surface Observing System) and AWS (Automatic Weather System) of KMA (Korean Meteorological Administration) and model-driven hydrological data from LSM (Land Surface model) are used for the real-time drought monitoring, while the monthly and seasonal weather forecast information from UM (Unified Model) of KMA are utilized for drought outlooks. For the evaluation of the system, past historical drought records occurred in Korea are surveyed and are compared with the application results of the system. The results demonstrated that the selected drought indices such as KMA drought index, SPI (3), SPI (6), PDSI, SRI and SSI are reasonable, especially, the performance of SRI and SSI provides higher accuracy that the others.

        전신 마취후 발생한 전율현상에 대한 Tramadol의 치료 효과

        Background : Tramadol is a centrally acting analgesic with weak opioid agonist properties and has effect on the spinal inhibition of pain. This study was designed to evaluate the efficacy of tramadol in the treatment of postanesthetic shivering. Methods : Sixty patients (ASA class I/II) who showed postanesthetic shivering were randomly assigned into three groups (n=20): Normal saline group; normal saline 10 ml, tramadol (TRD) 0.5 mg/kg group; tramadol 0.5 mg/kg, TRD 1.0 mg/kg group; tramadol 1.0 mg/kg. And all patients received standard postoperative management in the recovery room. Evaluation of the grade of shivering was done at 30 seconds, 2, 5 and 10 minutes from the beginning of the treatment by the same investigator who had injected the drug. The age, sex, weight, duration of anesthesia and axillary temperature were recorded. Results : By 30 seconds, 2 minutes, 5 minutes, and 10 minutes, 0, 4, 9, 9 patients of the 0.5 mg/kg tramadol group (n=20) and 7, 18, 19, 19 patients of the 1.0 mg/kg tramadol group (n=20) stopped the shivering respectively. But in 3 patients of 0.5 mg/kg tramadol group who stopped shivering by 5 minutes, shivering was recurred within 10 minutes and in 3 patients who had not stopped shivering by 5 minutes, shivering stopped by 10 minutes. In the placebo group, only 1 patient stopped shivering by 5 minutes. Conclusions : 1.0 mg/kg of tramadol was effective for the treatment of postanesthetic shivering but 0.5 mg/kg of tramadol was ineffective. (Korean J Anesthesiol 1999; 36: 1003∼1007)
