구강관리 프로토콜 적용이 항암화학요법을 받는 고형암 환자의 구강상태 및 자가간호수행에 미치는 효과

본 연구의 목적은 항암화학요법을 받는 고형암 환자를 대상으로 구강관리 개별교육과 냉동요법 및 벤지다민 구강함수를 포함한 구강관리 프로토콜 적용이 대상자의 구강상태와 수행에 미치는 효과를 파악하기 위함이다. 총 연구 대상자는 31명이었으며, 실험군에 13명, 대조군에 18명이 배정되었다. 실험군에게는 구강관리교육, 냉동요법 및 벤지다민 구강함수를 적용하였으며 대조군은 기존의 구강간호를 제공하였고, 대상자의 구강상태와 자가간호 수행정도를 측정하였다. 수집된 자료는 Mann-Whitney test, Friedman test, 및 Chi-square test로 분석하였다. 연구결과, 매 측정 시기 별 두 그룹 간 구강상태의 차이는 없는 것으로 나타났다. 그러나 두 그룹 모두 측정시기에 따른 구강상태의 변화는 유의하였는데 항암치료 후 14일경에 구강상태점수가 가장 낮게 측정되었다. 자가간호 수행정도는 실험군이 대조군보다 유의하게 높았으며 연구 완료 후 4개월까지 유지되는 것으로 나타났다. 이러한 결과는 종양간호사에 의한 환자개별교육의 효과로 여겨지며 고형암 환자의 구강상태 개선을 위하여 냉동요법과 벤지다민 구강함수에 대한 효과를 파악하기 위한 추가적인 연구가 요구된다. The purpose of this study was to identify the effectiveness of oral care protocol including individual oral care education, cryotherapy and benzydamine gargling on oral cavity status and self-care performance in solid cancer patients receiving chemotherapy. Thirty-one patients were enrolled by convenience sampling in this study and allocated to control (n=18) and experimental group (n=13). In the intervention group, individual oral care education, oral cryotherapy and benzydamine gargling were applied, while the control group received pre-existed oral care. Oral cavity status and self-oral care behavior were measured in five periods. The data were analyzed using the Mann-Whitney test, Friedman test, and Chi-square test. There was no significant difference in oral cavity status between the groups. But the two groups showed the poorest oral cavity status on 14th day. Self-care behavior was significantly higher in the intervention group than the control group. This effect had maintained for 4months after completion of study. The results showed that individual oral care education by oncology nurse is effective to encourage patients to do self-care behavior and a further study is needed to explore the effect of cryotherapy and benzydamine gargling on oral cavity status of patient with solid tumor.

1. Seo Young-eun & 『Her Woman』 : A recent novel by Seo Younb eun, Her Wonrand is a sort of autobiography of the writer. The novel is worth noticeable in that it provides clues in solving problems related to the identity of woman, on which the writer has constantly focused. Thus the purpose of this study is to trace real and unreal aspects of sex and love Seo Young Eun explore in the novel. 2. Double Point of View & Desire of Three-Party Context : The novel starts with the double point of view involving foculiser 'she' and speaker 'I' and later develops with the desire of three-party context. The subject of desire, 'Hyeon Seok Hwa' is sure that her husband killed himself and becomes obsessed with a long, revenant context of three parties, "ex-wife-husband-Hyeon Seok Hwa". This is really reproduced with another context, "Hyeon Seok Hwa-Bang So Yeon-son". In the novel, relations between 'she' and 'I' rushes into sadomasochism along with the code of homosexual love. 3. Patriarchy & Transfer of Pleasure : Hyeon Seok Hwa first meets her lover, So Yeon as she helps her financially. She becomes sexually attached to So Yeon and further emotionally interfere and involve in her at any time. This is the way that man controls woman and similar to the dead husband's way. In other words, she is the dead husband and she(So Yeon) is Hyeon Seok Hwa herself, that is, transfer. The ambiguous sexuality of Hyeon Seok Hwa makes her lose gender and pleasure and further become a being who is not herself. 4. Vigilance & Sign of Subjectivity Restoration : Hyeon Seok Hwa is still obsessed with the desire while, So Yeon wishes to meet someone new. Relations between the two women do not develop any more. Hyeon Seok Hwa kills herself as she finds herself in reality like desert through So Yeon. Her death is not defeat, but ironically relief and emancipation. It also makes real choice by 'I', the very main character who survives. 'I' will be a woman that manages her destiny by crossing over balance between social customs and reality. This is the focus of the narrative.
