        흉부물리요법이 인공호흡기환자의 객담량, 폐유순도, 일회호흡량 및 산소포화도에 미치는 효과

        Purpose: This study investigated the effects of chest physiotherapies on intensive care unit patients mechanical ventilated. Methods: Good lung down position, chest percussion, postural drainage was applied to patients who admitted to ICU. Each patients divided into four groups and each group received different treatments. Sputum amount, lung compliance, tidal volume and oxygen saturation were measured before treatment and immediately, and time flowing. Data was analyzed with frequency, percentage, ANOVA and paired t-test using via SPSSWIN 12.0 program. Results: There were significant differences in variables each characteristics of subjects. Chest percussion increased tidal volume, static lung compliance for the mean time. Desaturation related to suction. Conclusion: Chest percussion influences on lung compliance. Based on this study results and limitation, this study suggests repeated studies in various groups

      • 중환자실 섬망 선별 도구의 유효성 검증

        This study was to validate a delirium screening tool in the medical intensive care unit (MICU). The collected data from 70 subjects were analyzed secondly. They were screened using the Confusion Assessment Method for the Intensive Care Unit (CAM ICU) and Richmond Agitation Sedation Scale (RASS). The kappa coefficient of the inter raters for the reliability test were .871∼1.000. For the validity test, the values for sensitivity, specificity, correct classification rate, positive predictability, negative predictability were found to be 76.0%, 100.0% 91.4%, 76.0%, 100.0%, respectively. In order to verify the reference validity, the Phi correlation coefficient for DSM IV and CAM ICU. As a result, the Phi coefficient was .82 (p<.001) and statistically significant. To verify the construct validity,we used the group comparison method. There was a significant difference in mean score of RASS between the delirium and non delirium groups (t=4.91, p<.001). The findings support that the CAM ICU is reliable and valid in delirium screening in medical intensive care unit. Therefore, This screening tool is recommended for early detection and prevention of delirium. There is a need for studies comparing the patient outcome and delirium findings before and after using the delirium screening tool.

      • 간호교육에서 통합적 플립러닝의 적용 사례

        This study aims to introduce a case of integrated flipped learning in the major subjects of nursing class in an university. This study describes the development and application of the teaching methods. Integrated flipped-learning is a kind of problem-solving teaching method by integrating flipped-learning, case-based learning, team building, role play and debriefing. With well-informed of) online learning materials before class, students have group discussions to solve problems in class, organize strategies for problem solving, and present intervention plans with therapeutic communication skills. The evaluation is conducted by professor evaluation, student mutual evaluation, and self-reflection. In conclusion, this method helps not only strengthen students' learning abilities to utilize knowledge in context, but also improve confidence and deep learning. It can be used in various subjects such as clinical practice or simulation.

      • Intervention Mapping을 활용한 건강 중재 프로그램 개발

        Development of theory-based health intervention program is emphasized to obtain behavioral outcome change in health care. According to this trend, the health professionals have interests in behavioral change theory and systematic development methods for health promotion programs. The aim of this study was to introduce Intervention Mapping (IM) protocol and describes the development process of physical activity program using IM protocol. Six steps of the protocol consists need assessment, defining goals and objectives, adopting theory and determinants, matrix formation by change objectives, strategies and methods, contents and flow of the program, planning for implementation and evaluation. In the example, transtheoretical model (TTM) was applied and 5 determinants of TTM were adopted to in each session to improve physical activities. Developing a theory-based health intervention program using the IM protocol was useful and effective for practioners and researchers.

        여자 대학생의 스마트 폰 앱 활용 신체활동 경험: 포커스 그룹 인터뷰

        We carried out to interview female university students the needs for a successful physical activity promotion programs using smartphone applications. Fifteen healthy female college students with inactive lifestyles participated in focus group interviews after using a smartphone walking application with self-monitoring and social networking functions for 10 days. The contents of the interview were extracted into 18 codes and 6 sub-categories were derived. They were classified into 2 categories; motive or barrier to move, role of application as a motivator. As a result of this study, for the successful integration of a smartphone application into the physical activity program for the female university students, there is a need for considering their motive, sustainable activity, environmental resources with additional application functions.

        당뇨병이 있는 노인의 행위 변화 동기 요소와 자기관리 행위

        Purpose: Diabetes Self Management Behavior (DSMB) is crucial for the elderly with diabetes to prevent diabetes complications and to improve their quality of life. The Purposes of this study were to investigate the current status of DSMB and to identify motivational factors related to DSMB in community dwelling older adults with diabetes. Methods: The subjects were 150 diabetic elderly who visited 2 community senior centers in S city. DSMB scale consisted of 5 sub-domains; Being active, healthy eating, regular medication, glucose monitoring, and foot care rated by a scale with a range of 0 to 7. Personal motivation(i.e., intention to behavior) and social motivation including family support and health professional support were measured. Results: The mean score of DSMB was 4.27. The mean score of intention to behavior was 2.52. DSMB was related to Intention to behavior (r=.461, p<.001), family support (r=.342, p<.001), and health professional support (r=.284, p<.001). In regression analysis, a total of 33.4% of variance in DSMB was accounted for by intention to behavior, family support, and health professional support. Conclusion: To improve DSMB of the elderly, diabetes educator should consider on the strategies across both personal and social motivation related to DSMB.

        국내 기초간호학 교육과정 운영 현황 및 교수자의 기초간호학 교과목에 대한 인식

        Purpose: This study was conducted to investigate the current status of the biological nursing curriculum and faculties’ perceptions on biological nursing subjects in South Korea. Methods: Biological nursing subjects’ titles and credits were searched through the nursing schools’ website. Perceived adequacy of subjects’ title and credit in each biological nursing subject and the degree of perceived importance and satisfaction of learning contents by subjects were collected from 30 professors of biological nursing subjects. Results: Many schools still use the titles of medical subjects for those of biological nursing subjects. The perceived importance and satisfaction of genetics-related learning contents, “nursing case study” or “project team presentation,” were lower than other learning contents. Conclusion: Standardization of biological nursing subjects’ titles and learning contents is needed.

      • 국내 간호대학생의 자아탄력성 증진에 대한 통합적 고찰

        Nursing student resilience is defined as the ability to successfully cope with the challenges and adversities in the academic and clinical settings using proper self-control. This study is to review and synthesize, systematically, the research dealing with resilience and factors influencing resilience in Korean nursing students. Study design is integrative review and eligible studies were identified by searching KoreaMed, KISS, RISS4U, and NSDL (google scholar) with key words of ‘nursing school student’ and ‘resilience’. Three researchers independently reviewed the studies. Twelve non-experimental studies were selected. Resilience was measured using two tools with scores ranging from 66 to 72 out of 100. Students perceived clinical practices and circumstances as adversity and expressed it as feeling insecure or stressful. Resilience was related to both internal factors (self-efficacy, competence, problem solving, stress coping, critical thinking, emotional intelligence, and communication skills) and external factors (family support and teaching effectiveness). Resilience was positively associated with academic adaptation, clinical practice satisfaction, major satisfaction, school satisfaction and professional identity. No literature was on resilience enhancing interventions or the evaluation of them. Further research is needed to examine the correlations, develop interventions that improve resilience and evaluate them based on theories.

        간호대학생의 규칙적 신체활동에 영향을 미치는 요인

        Objectives: This study aimed to investigate the distribution of stages of change of physical activity and identify the influence of self-efficacy for physical activity, stress, and stress-coping style on regular physical activity in nursing students. Methods: A cross-sectional survey was conducted at 3 universities in the cities of C, I, and U in Korea. Data on stages of change for physical activity, self-efficacy for physical activity, stress-coping style, stress level, and health behaviors were collected from 209 nursing students. Data were analyzed using ANOVA, chi-square, Pearson’s correlation, and logistic regression analyses. Results: The highest proportion (39.2%) of nursing students were at the contemplation stage of change for physical activity. The mean score of self-efficacy was 2.67 out of 5 points. Among the participants’ stress-coping styles, the average number of active coping styles was 4.74 out of 7 points, and passive coping styles was 4.67 out of 7 points. Regular physical activity was related to self-efficacy, stress coping, and stress level. Regression analyses showed that self-efficacy for physical activity (odds=2.30), emotion-focused stress coping (odds=1.65), and stress coping involving seeking social support (odds=1.44) were significant predictors of regular physical activity (R2=0.20). Conclusions: The recognition of individual’s self-efficacy, stress-coping styles, and stages of change for physical activity might be a factor in improving regular physical activity among nursing students. While nursing students at the lower stages need more self-efficacy, those at the higher stages require more social support resources in the program for physical activity for nursing students.
