기능성 고 Prolactin 혈증 환자에서의 시상하부 - 뇌하수체 축의 기능에 대한 연구

In order to assess an influence of raised prolactin levels on hypothalamic-pituitary axis function, LH-RH test and estrogen provocation test were performed in 30 cases of idiopathic hyperprolactinemic patients and the results were summarized as follows: 1. Baseline estradiol, progesterone, LH and FSH values in idiopathic hyperprola. ctinemic patients were not statistically different from those values in normoprolact inemic control group. 2. The difference of gonadotropin response to the administered LH-RH was not statistically significant between idiopathic hyperprolactinemic group and control group. 3. The difference of negative positive estrogen feedback between idiopathic hyp erprolactinemic group and control group was not statisticaIIy significant. Above findings suggest that raised prolactin levels exert no significant and direct influences on hypothalamic-pituitary function in these idiopathic hyperprolactinemic patients.

Four hydrolytic enzymes, glycogen, fat, P.A.S. positive material and mucin were studied histochemically in normal adult human ovaries subject to a early follicular phase in 6 cases and to a late follicular phase in 9 cases. In alkaline phosphatase, slight activities first appeared in some developing theca cells of growing follicle and then activities gradually increased till it reached the peak in large graafian follicle. In acid phosphatase, moderate activities appeared in ovum, granulosa cells and developing theca cells of growing follicle as a small granular form and it lasted till to the mature follicle. In esterase, the activities were generally absent except a slight one in the theca interna cells of mature follicle. In -glucuronidase, moderate activities appeared in the theca and granulosa cells of mature follicle. In glycogen, slight amount first appeared in the granulosa cells of some growing follicles, then moderate amount in the granulosa and theca cells of mature follicle. In fat, moderate amount appeared in the theca cells of mature follicle. In P.A.S. positive material, slight amount appeared between granulosa cells and ovum of growing follicle that gradually increased in amount till it showed as small granular forms in follicular fluid of mature follicle. In mucin, Moderate amount appeared in granulosa and theca cells of growing follice with the same findings in the mature follicle.

The activity, contents and distribution of four hydrolytic enzymes, glycogen, fat, P.A.S. positive material and mucin were studied histochemically in normal adult human ovaries subject to a menstrual corpus luteum in 12 cases and to a degenerating corpus luteum in 15 cases. In hydrolytic enzymes, strong activities of ALP, ACP and esterase were noted in both granulosa and theca lutein cell layers of menstral corpus luteum. The activity of -glucuronidase was a slightly decreased one comparing with that of other enzymes. All emzyme activities were then progressively lost with beginning of degenerating corpus luteum but a few macrophages showed a persistent activities till corpus luteum became corpus albicans. In fat, moderate amount of globules appeared in both lutein cell layers of menstrual corpus luteum with a slight increased contents in theca lutein cells. Fat globules were then almost disappeared in degenerating corpus luteum. In glycogen, moderate amount were noted in grannulosa lutein cells and capillary endothelial cells of menstrual corpus luteum but progressively lost with a beginning of degenerating corpus luteum. In P.A.S. positive material, moderate amount were noted in both lutein cell layers of menstrual corpus luteum but slightly stronger in granulosa lutein cells. In degenerating corpus luteum but slightly stronger in granulosa lutein cells. In degenerating corpus luteum, both lutein cell layers lost their contents and only a few macrophages showed a reaction. In mucin, moderate amount appeared mainly in connective tissue of degenerating corpus luteum.

저자들은 29세의 초임부에서 유도성신경병질 1예를 경험하였기에 간략한 문헌고찰과 아울러려 증례를 보고하였다. Incidence of the entrapment neuropathy in pregnancy is very rare and relatively few cases are encountered in obstetric literature. Recently we experineced a case of this neurologic complication that we present with a brief review of literature.

본 증례는 Spiegelberg`s criteria에 충족한 난소임신의 2례였으며 이에 문헌적 고찰과 함께 보고하는 바이다. Two cases of ovarian pregnancy which have spiegelberg`s criteria for a primary ovarian pregnancy, are presented with a brief review of literatures.
