        Effect of the High Frequency Chest Wall Oscillation (HFCWO) on Pulmonary Function and Walking Ability in Stroke Patients

        Purpose: This investigation aimed to determine the effects of treadmill training (TT) and high frequency chest wall oscillation (HFCWO) on pulmonary function and walking ability in stroke patients as well as propose an exercise program to improve cardiovascular function. Methods: Twenty hemiplegic stroke patients were randomized to either the control group (CG) (n=10) or the experimental group (EG), which received TT and HFCWO (n=10). Pulmonary function was quantitated using patient forced vital capacity (FVC) and forced expiratory volume at one second (FEV1) while walking speed was assessed by the 10m walking test (10MWT). Further, walking endurance was determined utilizing the 6-minute walk test (6MWT). Subjects of the EG performed the study protocol for 60 minutes, five times a week for six weeks; CG patients did not participate in regular exercise. To determine significance for the differences observed before and after exercise, within-group and between-group comparisons were conducted utilizing paired and independent t-tests, respectively, with the level of significance set at α=0.05. Results: Within-groups, significant differences were observed in both FVC and FEV1 (p<0.01) following completion of the study protocol. Further, between-group comparisons demonstrated significant differences in both FVC (p<0.05) and FEV1 (p<0.01). Post-exercise, significant changes in the 10MWT and 6MWT score were observed between the EG and CG (p<0.01). Further, statistically significant differences were observed in 6MWT scores between-groups (p<0.05). Conclusion: The TT and HFCWO effectively improved pulmonary function and walking ability in subjects with stroke. The proposed program can be applied to stroke patients as a useful therapy.

        추가적인 등 가동 운동이 돌림근띠 복원술 환자의 어깨 관절가동범위와 통증 및 장애 지수에 미치는 영향

        Purpose: The study aimed to determine the effect of extra thoracic mobilization exercises on shoulder pain and function inpatients who had undergone rotator cuff repair. Methods: Following the recording of baseline measurements, 20 subjects who had undergone rotator cuff repair wererandomized into two groups: the experimental group (n = 10), which did thoracic mobilization exercises, and the control group (n = 10), which received traditional physical therapy. Each group participated in the intervention five times per week for two weeks. The pain (using the visual analogue scales [VAS]), the flexion and abduction range of motion (ROM), and the shoulder pain and disability index (SPADI) scores of both groups were recorded pre- and post-intervention. Paired t-tests were used to determine whether post-intervention scores were significantly different from pre-intervention scores, and independent t-tests were used to analyze differences in the dependent variables between the two groups. Results: After the two-week intervention, both groups experienced significantly decreased VAS and SPADI scores (p < 0.05) and significantly increased flexion and abduction ROM (p < 0.05). The experimental group that undertook the thoracic mobilization exercises showed greater improvements in pain, flexion and abduction ROM, and the SPADI than the control group (p < 0.05). Conclusion: These results suggest that thoracic mobilization exercises reduce shoulder pain and disability and enhance function in patients who have undergone rotator cuff repair.

        하네스를 착용한 상하지 협응 패턴운동이 만성 뇌졸중 환자의 보행능력과 균형능력에 미치는 영향

        Purpose: The current study seeks to examine the effect of coordinative pattern exercises of the upper and lower extremities using harnesses and walking rails on the walking and balance abilities of chronic stroke patients, and to develop effective programs and training methods to improve the functions of such patients. Methods: Subjects included 16 patients with hemiplegia caused by stroke. The subjects were randomly divided into an experimental group (n=8), on which coordinative pattern exercises of the upper and lower extremities were conducted, and a comparison group (n=8) that received typical exercise therapy. The experimental group underwent 30 minutes of typical exercise therapy and 30 minutes of coordinative pattern exercises of the upper and lower extremities, while the comparison group underwent typical exercise therapy for 30 minutes twice a day, five days per week for a six-week period. To evaluate walking ability, 10-m walking tests (10MWT) and 6-m walking tests (6MWT) were conducted. To assess balance ability, timed up and go tests (TUG) were performed. Results: After the intervention, significant (p<0.05) differences were seen in the 10MWT, 6MWT, and TUG in both the experimental and comparison groups. As for the 10MWT, the experimental group showed more significant improvement than the comparison group (p<0.05). In terms of the 6MWT, no significant differences were found between the groups, while the experimental group showed more significant differences than the comparison group in the TUG (p<0.05). Conclusion: The results from the current research indicate that training programs that apply coordinative pattern exercises of the upper and lower extremities with harnesses are extremely effective for improving the walking and balance abilities of chronic stroke patients.

        앞쪽머리자세가 있는 만성 목통증 환자에게 PNF를 이용한 목안정화운동 적용이 자세와 기능 장애에 미치는 효과

        The study aimed to determine the effect of the proprioceptive neuromuscular facilitation (PNF) cervical stabilization exercise on pain, cranio vertebral angle (CVA), and neck disability index (NDI) in chronic neck pain patients with forward head posture. Following baseline measurements, 26 subjects (n=26) with chronic neck pain were randomized into two groups: the experimental group (n=13), which received PNF cervical stabilization exercise, and the control group (n=13), which received traditional physical therapy. Each group participated in the intervention for three times per week, for six weeks. The visual analogue scales (VAS) for pain, CVA, and NDI Korean version (KNDI) of both groups were recorded at both pre- and post-intervention. Paired t-tests were used to determine significant changes in the post-intervention period compared with pre-intervention, and independent t-tests were used to analyze differences in the dependent variables between the two groups. After the six-week intervention, both groups experienced significantly decreased VAS and KNDI (p<.05) and significantly increased CVA (p<.05). The experimental group that received the PNF cervical stabilization exercise showed greater improvements in pain, CVA, and KNDI than those of the control group (p<.05). These results suggest that the PNF cervical stabilization exercise reduces neck pain and functional disability and enhances CVA in chronic neck pain patients with forward head posture.

        Jeonbuk Tri-Pull Taping과 고유수용성신경근촉진법 운동이 뇌졸중 환자의 어깨관절 가동범위, 통증, 아탈구, 팔 기능 및 일상생활수행능력에 미치는 영향 -사례연구

        Purpose: This study aims to determine the effect of Jeonbuk tri-pull taping and proprioceptive neuromuscular facilitation (PNF) exercise on the shoulder’s active range of motion, pain, subluxation, upper extremity function, and activities of daily living in patients with stroke. Methods: In this study, Jeonbuk tri-pull taping and PNF exercise were applied to three patients with stroke and subluxation. The tape was removed and new tape applied for two days every Monday, Wednesday, and Friday over six consecutive weeks. PNF exercise was applied five times a week for six weeks. To measure the range of motion, a smart phone clinometer application was used, and the degree of pain was measured using a visual analogue scale (VAS). A jig measuring method was employed to measure the distance of subluxation. The Fugl-Meyer Assessment (FMA) was used to evaluate arm function, and the modified Barthel Index (MBI) was employed to evaluate the activities of daily living. Results: The shoulder’s active range of motion was improved in the patients compared to the range of pre-tests, and the pain and subluxation distance were reduced compared to those of pre-tests. Arm function and activities of daily living were increased compared to those of pre-tests. Conclusion: The study results verified that Jeonbuk tri-pull taping and PNF exercise are useful when applied to patients with subluxation and stroke.

        PNF를 이용한 작은가슴근 신장과 아래등세모근 강화 운동이 둥근 어깨 자세를 가진 돌림근띠 복원술 환자의 자세변화와 어깨 관절가동범위 및 통증에 미치는 영향

        Purpose: This study aims to determine the effect of proprioceptive neuromuscular facilitation (PNF) pectoralis minor stretching and lower trapezius strengthening exercise on posture change, shoulder range of motion (ROM), and pain in rotator cuff repair patients with a rounded shoulder posture (RSP). Methods: Following baseline measurements, the subjects (n = 25) with rotator cuff repair were randomized into two groups: the PNF group (n = 13), which engaged in PNF pectoralis minor stretching and lower trapezius strengthening exercise, and the control group (n = 12), which received traditional physical therapy. Each group participated in the intervention five times per weekfor two weeks. The RSP values for the postural changes, flexion and abduction ROM, and visual analogue scales (VASs) of both groups were recorded at both pre- and post-intervention. In addition, before and after the intervention, a paired t-test was conducted to compare the within-group changes and an independent t-test to compare the between-group differences. Results: After the two-week intervention, both groups experienced significantly decreased RSP and VAS values (p < 0.05) and a significantly increased ROM (p < 0.05). Meanwhile, the PNF group showed greater improvements in RSP, ROM, and VAS than the control group (p < 0.05). Conclusion: These results suggest that PNF pectoralis minor stretching and lower trapezius strengthening exercise reduce posture and shoulder pain and enhance ROM in rotator cuff repair patients with RSP.

        고유수용성신경근촉진법 엉덩허리근 신장기법이 허리통증 환자의 통증과 엉덩관절 가동범위에 미치는 영향 : 단일사례연구

        This study aimd to determine the effect of proprioceptive neuromuscular facilitation (PNF) iliopsoas muscle stretching techniques on the pain and hip joint range of motion in patients with low back pain. In this study, PNF iliopsoas muscle stretching techniques was applied to three patients with low back pain. PNF iliopsoas muscle stretching techniques was performed 13 times during intervention phase. The subjects were measured 5 times in the baseline phase, 13 times in the intervention phase. A visual analogue scale (VAS) was used for the measurement of the level of pain. The FABER test (FABERT), modified Thomas test (MTT), and prone hip extension test (PHET) were used for the measurement of the hip joint range of motion. The pain was reduced compared in the patients to those of pre-tests, and the hip joint range of motion was improved compared to the range of pre-tests. The study results verified that PNF iliopsoas muscle stretching techniques are useful when applied to patients with low back pain.

        고유수용성신경근촉진법을 사용한 몸통 강화 및 안정화 운동이 기저핵 뇌졸중 환자의 균형과 보행 능력에 미치는 영향

        This study aimed to determine the effects of trunk strengthening and stabilization exercise using proprioceptive neuromuscular facilitation (PNF) on the balance and walking ability in patients with basal ganglia stroke. Five patients with basal ganglia stroke volunteered for this study. PNF trunk strengthening and stabilization exercise was performed for average 30 minutes per day, five times a week for four weeks (a total of 20 sessions). Outcome measures included one-leg standing test (OLST), Berg balance scale (BBS), and tandem walking test (TWT) for balance function, and 10 meters walk test (10WMT) for walking function. Significant improvements were found for the OLST, BBS, TWT, and 10WMT scores after the intervention (p<.05). These results suggest that trunk strengthening and stabilization exercise using the PNF may be favorably used for patients with basal ganglia stroke.

        만성허리통증의 고유수용성신경근촉진법 효과 -국내연구의 메타분석과 체계적 고찰

        Purpose: This study compares and examines the effects of proprioceptive neuromuscular facilitation (PNF) on patients with chronic low back pain through systematic literature review and meta-analysis. Methods: Domestic literature was searched with combinations of keywords including “proprioceptive neuromuscular facilitation,” “PNF,” “back pain,” and “low back pain” using the Research Information Sharing Service (RISS), Korean Studies Information Service System (KISS), and Korean Medical Database (KMbase). Six studies (n=148) were finally included in the analysis through a selection and exclusion process. The quality of the studies was evaluated using the PEDro scale. Results: According to the meta-analysis results, the low back pains of the PNF group and the contrast group showed a standardized mean difference (SMD) of 2.21 (95% CI: -3.35, -1.07, p=0.01, I²=83%) after intervention. Thus, the PNF group showed a statistically significant decrease in low back pain compared with the control group. In addition, the SMDs of the Oswestry Disability Index (ODI), lung function, and the Roland and Morris Disability Questionnaire (RMDQ) were -1.34 (95% CI: -1.88, -0.79, p<0.01, I²=35%), 1.14 (95% CI: 0.49, 1.79, p=0.01, I²=0%), and -1.59 (95% CI: -2.56, -0.62, p=0.01, I²=46%), respectively. Thus, the PNF group showed statistically significant differences from the control group. Conclusion: At present, there is some limit to obtaining definite results about effect sizes because there are relatively few randomized controlled experiments that analyze the effects of PNF exercise in patients with chronic low back pain. Therefore, continuous efforts should be made to conduct randomized clinical trials and long-term efficacy studies in the future.

        비특이성허리통증 환자의 신체 기능과 활동에 대한 코어안정화운동과 넙다리뒤근 신장기법 융합의 효과

        본 연구는 코어안정화운동과 넙다리뒤근 신장기법 융합이 비특이성허리통증 환자의 통증, 골반경사, 장애 수준 및 균형능력에 어떠한 영향을 미치는지 알아보고자 하였다. 비특이성허리통증 환자 20명을 대상으로 일반적 물리치료를 실시한 대조군(n=10)과 코어안정화운동과 넙다리뒤근 신장기법을 적용한 실험군(n=10)으로 무작위 배 정하였다. 중재 전과 4주 후에 시각적상사척도, 골반경사, 오스웨스트리 허리통증 장애수준 및 버그균형척도를 평 가되었다. 연구결과, 두 집단 모두 통증, 골반경사, 장애수준 및 균형에 향상된 결과를 보였다(p<0.05). 집단 간 비 교에서는 실험군이 대조군보다 통증, 골반경사, 장애수준 및 균형에 더욱 향상된 결과를 보였다. 비특이성허리통증 환자의 통증과 골반경사의 감소 및 장애수준과 균형의 향상을 위해 코어안정화운동과 넙다리뒤근 신장기법을 유 용하게 적용할 수 있을 것으로 여겨진다. This study attempts to identify the effects of core stabilization exercise (CSE) and hamstring muscles stretching (HMS) convergence on pain, pelvic tilt, disability level, and balance of patients with non-specific low back pain (NSLBP). In total, 20 patients with NSLBP were randomly divided into a control group (CG, n=10), in which the traditional physical therapy was performed, or an experimental group (EG, n=10), in which CSE and HMS was performed. Assessments were made using the visual analogue scale, pelvic tilt, Oswestry disability index, and Berg balance scale before and after the 4-week intervention period. Both groups showed significant improvements in pain, pelvic tilt, disability level, and balance after the intervention (p<.05). Notably, a significant difference was noted in the pain, pelvic tilt, disability level, and balance of the patients in the EG (p<.05). This study proved that application of CSE and HMS effectively reduces pain and improves pelvic tilt, disability level, and balance of patients with NSLBP.
