      • S-116 Unsatisfactory results of fecal microbial transplantation on refractory pseudomembranous colitis.

        Introduction:?Fecal microbiota transplantation (FMT) was a treatment option for recurrent and refractory pseudomembranous colitis (PMC). However, due to a lack of research on the FMT, protocol and complication is not clear. We have experienced 6 FMT cases. As below, we describe 3 patients with unsatisfactory FMT result.?Case 1:?An 87-year-old man was tranfered to nephrology with recurrent UTI. After use of antibiotics, diarrhea was continued and Clostridium difficile(C.difficile) toxin test was positive. PMC wasn’t improved by metronidazole and oral vancomycin. So, we performed FMT with 200 g of fresh feces via gastric Tube. We wanted to perform FMT via jejunal endoscope, but, it was not available in our hospital. Diarrhea was improved immediately after FMT, but aspiration pneumonia was developed on both whole lung and cured with meropenem. Despite of using antibiotics, PMC didn’t recur.?Case 2:?An 89-year-old man complained of mucoid diarrhea after treatment of cholecystitis.Symptom was not improved by casual management, even aggravated. So, we performed FMT with small amount of feces via colonoscopy. After FMT, septic condition was improved but diarrhea was relapsed within days.?Case 3:?An 88-year-old man was in septic shock state with diarrhea and abdominal distension.He showed positive result of C.difficile toxin and toxic megacolon on simple abdomen image. Symptom was not improved by metronidazole, oral vancomycin and vancomycin enema even though by tigecycline. We performed FMT with 200 g of fresh feces via colonoscopy and maintained tigecycline. After FMT, septic condition was cured but diarrhea and colon dilatation were only partially improved.?Conclusions:?We can suggest that FMT via gastric tube has high risk of aspiration pneumonia and enough feces are important for successful FMT. Lastly, continuous use of antibiotics can impair the effect of the FMT. FMT is unfamiliar to patients and their family yet. But, it is very effective treatment of PMC despite its complication. We hope that various situations with FMT support more advanced management on PMC.

        HMS 운용방안에 관한 연구

        The Navy is in the process of developing a sonar-operation strategy to increase the effectiveness of underwater target searching capability. HMS is the basic strategy to detect underwater targets. The advantages of HMS is that, it has a short preparation time to operate and can be always used regardless of sea conditions and weather. However, it is difficult to effectively detect underwater targets due to the interaction between marine environments and sonar-operations. During the research, the effectiveness of the HMS system's underwater target searching capability was analyzed by integrating various search and defense patterns, and environment conditions into the simulation. In the simulation the ship search an evasive target within a set region. The simulation presented results for an effective searching and defense methods of underwater targets. These research results can be used as foundation for advancing and improving the sonar operational tactics.

        심초음파검사에 의한 최근 15년간의 심혈관질환의 변화

        The improvement of nutritional status, the change of life style, and medical progress have changed the pattern of cardiovascular disease. There were many epidemiologic studies on cardiovascular disease in 1970s and 1980s, but rare in recent years in Korea. The purpose of this study was to investigate the recent change of the cardiovascular disease by examining of the echocardiographic diagnosis of these 15 years. Echocardiography data of 23,339 successive cases that were done at Keimyung University Dongsan Hospital, from January 1981 to December 1995 were reviewed and classified into twelve disease group and a normal group. Major findings of this study were as follows: The mean age of patients who received echocardiogram tended to increase by more than 10 years through this period. The frequency of ischemic heart disease, hypertensive heart disease, and arrhythmia had increased throughout the period, and adult congenital heart diseased, valular heart disease, infective endocarditis, pericardial disease were of decreasing tendency in relative frequency of cardiovascular area. Frequency of cardiovascular disease, such as hypertension, ischemic heart disease, was higher in female than in male. The mean ages of each cardiovascular disease group were increasing except ischemic heart disease and cor pulmonale. On consideration of above findings, the detailed epidemiologic study for cardiovascular diseases might be recommented, and now, we should plan for the effective management of ischemic heart disease and hypertensive heart disease that are increasing in these years.

        고밀도화 공정에 의한 Fe-Co 계 밸브시트 합금의 조직변화와 열적 특성

        Infiltration is a popular technique used to produce valve seat rings and guides to create dense parts. In order to develop valve seat material with a good thermal conductivity and thermal expansion coefficient, Cu-infiltrated properties of sintered Fe-Co-M(M=Mo,Cr) alloy systems are studied. It is shown that the copper network that forms inside the steel alloy skeleton during infiltration enhances the thermal conductivity and thermal expansion coefficient of the steel alloy composite. The hard phase of the CoMoCr and the network precipitated FeCrC phase are distributed homogeneously as the infiltrated Cu phase increases. The increase in hardness of the alloy composite due to the increase of the Co, Ni, Cr, and Cu contents in Fe matrix by the infiltrated Cu amount increases. Using infiltration, the thermal conductivity and thermal expansion coefficient were increased to 29.5 W/mK and $15.9um/m^{\circ}C$, respectively, for tempered alloy composite.
