Atopic dermatitis and psoriasis overlap: New disease entity defined by spatial transcriptomic analysis
Background: Atopic dermatitis (AD) and psoriasis (PsO) are chronic T-cell mediated inflammatory skin diseases that have been considered to have distinct clinical and immunologic features. Yet, patients demonstrating clinicopathologic overlapping features between AD and PsO exist, which pose tough challenges for dermatologists in perspectives of diagnosis and managing treatment options.
Objective: The purpose of this study is to investigate the molecular characteristics of a disease subset referred to as AD-PsO overlap at the epidermal and dermal levels.
Methods: Spatial transcriptomic profiling using the GeoMx® platform was employed to investigate gene expression levels in epidermal and dermal immune cells from lesional skin of typical AD, typical PsO, and the AD-PsO overlap patients.
Results: Principal components analysis and heat map of differentially expressed genes demonstrated that patients with overlap phenotype exhibited characteristics of both AD and PsO, where the overlap group participants’ samples were divided into two distinctive clusters. Well-known marker genes for AD or PsO confirmed the molecular spectrum of diseases by spatial transcriptomic analysis. Moreover, some novel biomarker candidate genes were discovered to distinguish the subclusters.
Conclusions: Spatial transcriptomic analysis of lesional skin tissue from patients with AD, PsO, and overlapping clinicopathological features of both diseases revealed the molecular spectrum of diseases from AD to PsO.
