국내 전이성 유방암의 5년 전체생존율은 34%에 불과하고 완치가 어려워, 생존기간 연장과 삶의 질 향상을 목표로 아로마타제 억제제나 항암화학요법이 오랫동안 사용되었지만 치료제의 발전은 정체돼 있었다. 하지만 세포 성장에 관여하는 단백질인 사이클린 의존성 키나아제(CDK) 4/6억제제의 등장으로 치료제 선택의 폭과 향상된 무진행생존기간(PFS)을 제공할 것으로 여겨진다.
본 연구는 이전에 전신요법을 시행하지 않은 페경 후 HR-양성/HER2-음성 진행성 또는 전이성 유방암 환자들에게 생존률 개선을 입증하며 국내 허가 및 급여에 성공한 Palbociclib과 Letrozole의 병용요법(이하 Palbociclib군)과 새로이 허가를 받은 Ribociclib과 Letrozole의 병용요법(이하 Ribociclib군)의 효과를 비교하고자 한다. 현재까지 두 치료법의 직접비교 무작위배정임상시험(RCT) 자료가 없으므로 간접비교(Indirect Treatment Comparison, ITC)를 통해 각각의 치료법의 임상효과를 비교하였다.
대상환자와 비교하고자 하는 치료군에 대한 체계적 문헌 고찰을 수행했고, 결과변수는 무진행생존기간으로 하여 국내외 데이터베이스에서 검색을 실시하였다. 최종 선정된 문헌은 Palbociclib군과 위약을 비교하는 Finn et al(2016)의 연구와 Ribociclib군과 위약을 비교하는 Hortobagyi et al(2016, 2018)연구였다.
두 문헌의 질 평가 및 비뚤림 위험 평가를 시행하였고 환자의 선정 기준, 선정된 환자의 특성, 유효성 평가 변수 와 통계분석 방법 등을 비교해 본 결과 두 연구 간에 유사성을 보였기 때문에 간접 비교를 수행하였다. 간접비교 결과 무진행생존에 대한 상대위험비(hazard ratio)는 0.979 [95% 신뢰구간 0.717, 1.337]으로 Ribociclib군과 Palbociclib군간의 유의한 차이를 보이지 않았다. 또한 산출된 신뢰구간이 체계적 문헌 고찰을 통해 설정한 비열등성 허용한계인 1.34 미만에 분포하여 이전에 전신요법을 시행하지 않은 HR-양성/HER2-음성 진행성 또는 전이성 환자의 무진행생존기간에 대한 Ribociclib군의 치료 효과가 Palbociclib군의 치료 효과에 비해 열등하지 않는 것으로 확인하였다.
따라서 이미 급여되어 사용되고 있는 Palbociclib에 더하여 Ribociclib도 급여권에 진입하게 된다면 환자들에게 더 많은 치료 옵션이 제공될 수 있을 것으로 기대된다.

The overall 5-year survival rate of metastatic breast cancer is only 34% in Korea, and it is hardly curable. Aromatase inhibitors and chemotherapy have been used for a long time to improve survival and quality of life for those patients, but the development of therapeutics has been stagnant. The emergence of a cyclin-dependent kinase (CDK) 4/6 inhibitor, a protein involved in cell growth, is expected to provide a range of therapeutic options and improved progression-free survival (PFS).
The purpose of this study is to compare the efficacy of palbociclib (in combination with letrozole), first approved and reimbursed in Korea by demonstrating improved survival in patients with postmenopausal hormone receptor-positive (HR+) / human epidermal growth factor receptor 2-negative (HER2-) advanced or metastatic breast cancer who have not previously undergone systemic therapy, with ribociclib(in combination with letrozole), newly approved CDK 4/6 inhibitor in Korea. The head-to-head comparison of the two treatments in combination with letrozole to date is not available, so the clinical efficacy of each treatment group was compared using the Indirect Treatment Comparison (ITC).
A systematic literature review was conducted based on the treatment groups and the patients. The primary outcome for the comparison was PFS. Finally, selected studies were Finn et al. (2016), comparing the palbociclib plus letrozole with placebo, and Hortobagyi et al. (2016, 2018), comparing the ribociclib plus letrozole with placebo.
The quality of the two studies and the risk of bias analysis were evaluated, and an indirect comparison was performed as the patient selection criteria, characteristics of selected patients, efficacy evaluation variables, and statistical analysis showed similarities between the two studies. Indirect comparisons showed that the relative risk ratio for progression-free survival was 0.979 [95% confidence interval 0.717, 1.337], indicating no significant difference between the ribociclib and palbociclib treatment groups. Besides, the calculated confidence intervals were below the 1.34 non-inferiority margin, which was established by a systematic literature review in advance, confirming that the ribociclib plus letrozole were not inferior to the palbociclib plus letrozole in improving the progression-free survival of HR+ / HER2- advanced or metastatic patients who had not previously undergone systemic therapy.
Therefore, in addition to palbociclib, ribociclib is expected to offer more treatment options to patients if it is reimbursed.
