Objective : The purpose of this study was to investigate the effect of the level of discrepancy between actual body type(ABSI) and subjective body type perception on depression and to analyze how this relationship appears differently by gender.
Method : Based on the 2016-2022 National Health and Nutrition Survey data, the effect of discrepancies between actual body type (ABSI) and subjective body type perception on depression in adult men and women was analyzed. According to the ABSI criteria, the types of body type recognition discrepancies were divided into 3 and 9 groups, variables that could affect the demographic characteristics, health behavior and health status, and subjective health status of the study subjects were controlled, and the differences in the risk of depression according to these groups were compared by gender. Depression was based on the PHQ-9 scale, and multivariate logistic regression analysis was performed after controlling demographic factors and health behaviors.
Results : The group (G3) that recognized the body type as heavier than it actually was (OR=1.703, 95% CI=1.290–2.248) and women (OR=1.402, 95% CI=1.150–1.710) significantly increased the risk of depression. In particular, when obesity was recognized even though it was a normal weight, OR=3.266 for men and OR=1.500 for women showed the highest risk of depression. In men, the risk of depression was also significantly increased in the group (G1) perceived as lighter than it actually was (OR=2.120), but there was no significant difference in women. In addition, low age, low income, low education, unmarried status, and attempt to lose weight were significantly related to depression. Drinking frequency was significantly correlated with depression only in women.
Conclusion : These analysis results suggest that body shape index (ABSI) can be used as a new indicator of obesity diagnosis along with body mass index (BMI), and body shape perception distortion has different effects on depression according to gender, and in particular, the tendency to overestimate than it actually is has been an important factor in increasing the risk of depression. This suggests the need to consider gender differences and body shape perception levels together in the association between weight and depression.
