Background: This study was conceived by a need to investigate the mental health of nurses doing COVID-19-related work at a tertiary general hospital amid the COVID-19 pandemic and provide basic data to offer them treatment support and interventions for their mental health.
Objective: The study aims to examine the subjects in their general characteristics, characteristics of their COVID-19-related work, and distribution level of depression, anxiety, and sleeplessness and analyze factors influencing their depression, anxiety, and sleeplessness.
Methods: Data was collected through a survey with 234 nurses at C, a tertiary general hospital in Seoul, from September 13~19, 2021. Collected data was analyzed with the SPSS 26.0 program. Frequency analysis was carried out to examine the general characteristics of the subjects, and the results were presented in frequency and percentage. Cross tabulation analysis and chi-square testing followed to examine differences in the distribution of depression, anxiety, and sleeplessness according to their characteristics, and the results were presented in chi-square values and p-values. In addition, binary logistic regression analysis was performed to analyze factors influencing their depression, anxiety, and sleeplessness, and the results were presented in odds ratio and 95% confidence interval.
Results: There were more women (94.0%) than men in the subject group. Those who were in their twenties made up the biggest proportion at 43.2%. As for position, common nurses recorded the highest percentage at 67.5%. The biggest proportion of the subjects had nursing work experiences less than 5 years at 45.7% and did shift work at 72.6%. The largest number of nurses worked at a general ward (37.6%), which was followed by an ICU (18.8%), outpatient department (15.8%), negative pressure isolation ward (11.5%), emergency room (8.5%), and operating room (7.7%), in that order.
The analysis results of COVID-19 work show that there were more nurses (55.6%) that had experienced frontline COVID-19 work than those who had not (44.4%). The nurses were categorized according to no COVID-19 work, doing COVID-19 work for less than six months, and doing COVID-19 work for more than six months.
As for the health behavior and mental health of nurses, 96.8% did not smoke, and 83.3% consumed alcohol; 60.3% said that they did not exercise or do leisure activities; 11.1%, 15.8%, and 54.9% suffered from depression, anxiety, and sleeplessness, respectively, which shows that the highest proportion of nurses suffered sleeplessness.
More nurses recorded a high level of resilience (88.9%) than a low level of resilience (11.1%).
In the study, a univariate analysis was performed based on cross tabulation analysis to examine the depression of the subjects according to their general characteristics, job characteristics, COVID-19 work, health behavior and mental health, and resilience. The analysis results show statistically significant differences in their depression according to age, educational background, monthly income, marital status, position, nursing career, shift pattern, resilience, and days of service amid COVID-19.
Another round of univariate analysis was performed based on cross tabulation analysis to examine their anxiety according to their general characteristics, job characteristics, COVID-19 work, health behavior and mental health, and resilience. The analysis results point to statistically significant differences in their anxiety according to marital status, position, nursing career, shift pattern, department, depression treatment experiences before COVID-19, anxiety treatment experiences before COVID-19, and resilience.
In addition, the study carried out a univariate analysis based on cross tabulation analysis to examine their sleeplessness according to their general characteristics, job characteristics, COVID-19 work, health behavior and mental health, and resilience. The analysis results found statistically significant differences in their sleeplessness according to the form of employment and anxiety treatment experiences before COVID-19.
The study analyzed factors influencing depression with a multivariate analysis and found that ICU and resilience had statistically significant effects on their depression.
The study then analyzed factors influencing anxiety with a multivariate analysis and found that ICU and resilience had statistically significant effects on their anxiety.
The study carried out another round of multivariate analysis to analyze factors including their sleeplessness and found no factors that had significant impacts on their sleeplessness.
Conclusion: The analysis results of the study demonstrate differences in mental health, including depression, anxiety, and sleeplessness between the nurses on the frontline of COVID-19 and nurses not on the frontline of COVID-19 based on the duties of nurses on the frontline of medical responses to the spread of COVID-19. The study examined the effects of general characteristics, job characteristics, COVID-19 work, health behavior, and resilience between frontline nurses and non-frontline nurses on their mental health and found that the duties of a nurse had the same significant impacts on mental health between the frontline and non-frontline nurses rather than the relationship with COVID-19 work. These findings imply a need for institutional support to solve the mental health issues of nurses at a tertiary general hospital.
