본 연구의 목적은 감각 처리의 어려움이 있는 자폐 스펙트럼 장애 아동을 대상으로 감각 조절 기법을 활용한 비디오 모델링 중재가 양치질 수행에 미치는 영향을 알아보고자 하였다.
연구 대상은 자폐 스펙트럼 장애 진단을 받은 3명의 아동이었으며, 연구 설계는 단일 대상 연구 설계(single-subject experimental design) 중 ABA설계를 사용하였다. 연구는 기초선 기간(A) 1주 동안 4회 측정, 중재 기간(B) 3주 동안 10회 측정, 재 기초선 기간(A’) 1주 동안 4회 측정으로 진행되었다. 감각 조절 기법 비디오 모델링 중재는 중재 기간(B) 동안 매일 1회, 총 21회 시행되었으며, 양치질 수행의 질과 회피 행동 빈도는 비디오로 촬영하여 분석하였다.
독립변수는 캐릭터 애니메이션 감각 조절 기법 비디오 모델링이었으며, 종속변수는 매회기마다 대상자별 조작적 정의를 바탕으로 측정한 양치질 회피 행동 빈도 및 양치질 수행의 질의 변화였다. 또한 한국판 감각 프로파일 2(K-CSP 2)를 통해 구강 감각 민감성과 구강 감각 처리 능력의 변화를 측정하였으며, 캐나다 작업 수행 측정(COPM)을 사용하여 대상자의 수행도 및 보호자의 만족도 변화를 측정하였다. 회피 행동 빈도는 보호자로부터 전달받은 영상을 통해 5초 간격으로 등간 기록 분석하였으며, 양치질 수행의 질 변화는 작업 수행의 질 척도(PQRS-OD)의 조작적 정의를 사용하여 측정하였다. 양치질 회피 행동 빈도와 양치질 수행의 질 변화는 회기별로 시각적 분석을 하여 꺾은선 그래프 형태로 제시하였고, K-CSP 2와 COPM은 중재 전, 중재 후, 그리고 재 기초선이 종료된 직후에 각각 측정하여, 그 결과는 막대그래프 형태로 제시하였다.
연구 결과, 감각 조절 기법 비디오 모델링 중재 후 자폐 스펙트럼 장애 아동들의 양치질 회피 행동이 평균 30.5% 감소하였으며, PQRS-OD의 양치질 수행의 질도 모든 대상에서 향상되었다. 또한 K-CSP 2의 구강 감각 민감성이 감소하고, 구강 감각 처리 능력도 개선되었다. COPM 결과, 보호자가 평가한 아동의 양치질 수행도와 보호자의 만족도가 중재 후 크게 향상된 것으로 나타났다.
따라서 본 연구는 감각 조절 기법과 비디오 모델링을 결합한 중재가 자폐 스펙트럼 장애 아동의 양치질 수행을 향상시키고, 구강 감각 처리 문제를 완화하는 데 효과적인 중재 방법임을 증명하였다는 점에서 큰 의의가 있다.

The purpose of this study was to investigate the effects of a video modeling intervention utilizing sensory modulation techniques on the toothbrushing performance of children with autism spectrum disorder (ASD) who experience sensory processing difficulties.
The study participants included three children diagnosed with ASD, and the research design employed an ABA design, a type of single-subject experimental design. The study consisted of a baseline phase (A) measured four times over one week, an intervention phase (B) measured ten times over three weeks, and a return-to-baseline phase (A') measured four times over one week. The sensory modulation video modeling intervention was implemented once daily during the intervention phase (B) for a total of 21 sessions. The quality of toothbrushing performance and the frequency of avoidance behaviors were analyzed based on video recordings.
The independent variable was the video modeling intervention using character animation and sensory modulation techniques, while the dependent variables included changes in the frequency of toothbrushing avoidance behaviors and the quality of toothbrushing performance, measured based on operational definitions for each session and participant. Additionally, changes in oral sensory sensitivity and oral sensory processing abilities were assessed using the Korean version of the Sensory Profile 2 (K-CSP 2). Changes in the participants’ performance and caregivers’ satisfaction were evaluated using the Canadian Occupational Performance Measure (COPM).
The frequency of avoidance behaviors was analyzed using interval recording every 5 seconds based on video footage provided by caregivers. Changes in the quality of toothbrushing performance were assessed using the operational definitions of the Performance Quality Rating Scale for Occupational Therapy (PQRS-OD). Data on avoidance behavior frequency and performance quality were visually analyzed and presented as line graphs for each session. K-CSP 2 and COPM were measured before the intervention, after the intervention, and immediately after the return-to-baseline phase, with the results displayed as bar graphs.
The study results indicated that the sensory modulation video modeling intervention led to an average reduction of 30.5% in toothbrushing avoidance behaviors among children with ASD. Additionally, all participants showed improvements in toothbrushing performance quality as measured by PQRS-OD. The K-CSP 2 results demonstrated reduced oral sensory sensitivity and improved oral sensory processing abilities. COPM results showed significant improvements in caregiver-rated performance and satisfaction regarding the children’s toothbrushing activities after the intervention.
This study holds significant implications by demonstrating that an intervention combining sensory modulation techniques with video modeling effectively enhances the toothbrushing performance of children with ASD and alleviates issues related to oral sensory processing.
