Aims/Introduction: The purpose of this retrospective study was to compare the incidence of hypoglycemia between long acting insulin and non-long acting insulin and to compare the incidence of hypoglycemia between insulin detemir and insulin degludec in diabetic patients requiring insulin.
Materials and Method: Based on the KIDS KAERS Database (KIDS-KD) maintained by Korea Adverse Event Reporting System (KAERS), all adverse events by insulin from January 2016 to December 2017 were collected. Insulin and insulin analogs were classified by the ATC code.
According to the WHO-ART code, "Convulsions hypoglycaemic", "Hypoglycaemia", "Hypoglycaemia aggravated", "Hypoglycaemia unawareness", "Hypoglycaemia morning", "Hypoglycaemic reaction", "Hypoglycaemia neonatal" and "Coma hypoglycaemic" were defined as hypoglycemia.
Total cases of adverse events by insulin are 5173. There are 3220 cases by long acting insulin and 1948 cases by non-long acting insulin. Among the adverse events by long acting insulin, 296 cases by insulin detemir and 739 cases by insulin degludec were collected.
Results: The number of adverse events caused by long acting insulin was 3220 and the number of hypoglycemia was 968. The number of adverse events caused by non-long acting insulin was 1948 and the number of hypoglycemia was 997. When comparing these two groups, we found that long acting insulin produced less hypoglycemia (ROR 0.41; 95% CI 0.39-0.43). There are 739 adverse events caused by insulin degludec. Of these, 172 are hypoglycemia. And 296 adverse events caused by insulin detemir. Of these, 83 were identified as hypoglycemia. Compared with Insulin degludec and Insulin detemir, insulin degludec showed less hypoglycemia (ROR 0.78; 95% CI 0.71-0.86). Adverse event reports can be categorized into 'Certain', 'Probable', 'Possible', 'Unlikely', 'Unclassified', 'Unassessable', and 'Not_applicable' depending on causality assessment. Insulin degludec was less likely to cause hypoglycemia when compared with insulin degludec and insulin detemir using data ('Certain', 'Probable', 'Possible') that were estimated to have causality (ROR 0.41; 95% CI 0.37-0.46).
Conclusion: Compared with Insulin detemir and Insulin degludec, Insulin degludec showed less hypoglycemia than insulin detemir in Korean diabetic patients requiring insulin.
