정부는 국내 제약기업의 혁신 역량을 강화하고 세계 선진 시장에서 경쟁력을 갖춘 기업으로 성장하도록 연구개발(R&D), 인력양성, 수출지원 예산 확대, PIC/S 및 ICH에 정회원 가입 등 그 간 다양한 노력을 기울여왔다. 그러나 국내 내수 시장의 약가인하, 과도한 가격 경쟁으로 인해 국내 제약기업이 성장의 기회를 찾기에 어려운 실정이다. 외형적으로 국내 의약품 시장은 과거 대비 빠른 속도로 성장하고 있지만 그 실속에는 기업규모 간 양극화가 두드러진다. 소규모 제약사 수는 큰 폭으로 증가하고 있지만 중소 제약사에서 중견 및 대형 제약사로 성장하는 사례가 거의 없는 것이 현실이다. 현 상황에서 국내 제약사들에게 주어진 유일한 돌파구는 해외 시장이다.
이에 따라 본 연구는 해외 시장 진출을 목표하고 있는 국내 제약회사들에게 정부 기관이 현실적이고 구체적인 맞춤형 지원 방안을 제공하는데 도움이 될 수 있도록 제약사의 기업유형 (중소기업, 비중소기업) 및 매출규모별 해외 인허가 과정에서 체감하는 장애요인과 지역별 허가 성공률을 비교분석하였다. 더 나아가 분석 결과를 바탕으로 제약기업의 제반 여건 및 경쟁력 현황에 적합한 해외 인허가 진원 방안을 도출하는 것을 목표하였다.
국내 제약사 중 선진국 또는 신흥국에 허가 경험이 있는 제약사를 대상으로 조사하고자 하여 동남아시아국가연합(ASEAN), 동아프리카공동체(EAC), 라틴아메리카연합(PANDRH) 등 국제기구 및 아시아태평양경제협력체 (APEC) 회원국에 해외 인허가 경험이 있는 국내 제약사 28개를 대상으로 연구를 진행하였다. 조사대상 제약사 해외 인허가 담당자에게 이메일로 설문 조사서를 송부하고 설문조사서 배포 후 해외 인허가 과정에서의 애로사항 및 해결 방안에 대한 원활한 논의를 위하여 총 2차례의 자문회의를 실시하였다. 설문 조사서는 조사 대상 제약사의 수출 품목 및 규모와 허가 신청 및 취득 국가에 대해 조사한 일반사항, 해외 인허가 담당자들이 인허가 단계별 체감하는 애로사항, 인허가 정책 만족도 및 선호도에 관한 의약품 해외 인허가 건의사항으로 구성하였다. 해외 인허가 애로사항에서 각 항목에 대한 체감 난이도를 수치화하기 위해 1에서 5까지의 리커트 척도(Likert Scale)를 사용하였다.
조사대상 제약사의 최근 5년간 허가 성공 비율을 조사했을 때 신흥국 대비 선진국에서 조사기업의 허가 성공률이 낮았다. 매출규모와 기업유형에 따른 허가 성공 비율을 살펴보았을 때에는 매출규모가 작은 기업일수록 그리고 중소기업일수록 선진국에서 허가 신청 비율이 낮은 것으로 나타났다. 해당 설문 조사서에서 허가 취득 실패 이유와 장애요인의 심각성에 대해 조사했을 때 허가 취득까지 매우 긴 시간이 소요되고(매우 심각 60.0%) 수출 대상국의 규제당국의 소통애로(43.0%)가 가장 큰 실패 요인임을 확인할 수 있었다.
인허가 단계별 어려움 정도에 대한 조사 결과에서 매출 소규모 기업일수록 IND와 GMP 과정을 어려워하고 있으며 중소기업은 비중소기업 대비 IND와 NDA 과정에서 체감 난이도가 유의미하게 높음을 알 수 있었다 (p<0.05). 현재 정부의 인허가 지원 정책에 대한 국내 제약사들의 선호도를 살펴보았을 때 매출 규모가 1000억원 미만이 기업 70.0%가 국가 간 상호인증을 가장 선호하고 있으며 매출 규모 1000억 원 이상인 기업의 경우 해외 인허가 정보 데이터베이스와 국가 간 상호 인증 추진 방안 두 지원책 시행이 가장 우선순위라고 인식하고 있었다.
본 연구결과를 토대로 국내 제약기업이 해외 시장 진출을 확대하고 성공적인 해외 인허가 성과를 얻기 위해서는 국내 임상시험관리기준 개선 및 GMP 관리와 실사 수준을 상향조정 등 국내 기준의 국제조화가 필요하며 해외 인허가 정보 확산을 위한 효율적인 시스템을 구축하는 것이 시급하다는 결론을 도출하였다.

In order to maximize Korean pharmaceutical companies‘ competence in the global market, the government has been exerting a great amount of effort through various support measures. Over the recent years, government grants on research and development, human resources development, and export support for pharmaceutical companies have been increased and Korea has become a member nation of PIC/S and ICH. However, the progress appears slower than expected. Korean pharmaceutical companies are struggling in search for growth opportunities due to drug price cuts, excessive price competition in the domestic market, and visible market stagnation. Furthermore, despite the fact that the number of small-scale pharmaceutical companies has increased at a rapid rate over the past years, the reality is that hardly any small-sized companies have grown into medium-sized companies or 'Big Pharmas'.
The purpose of this study is to provide guidance for appropriate agency planning and governmental support measures by identifying the perceived barriers of Korean pharmaceutical companies face in global regulatory affairs. To provide the basis for guidance, analyses on regulatory success rate of Korean pharmaceutical companies in various regions and the disparity in the aforementioned difficulties have been compared between different profit sized pharmaceutical companies as well as between the SMEs (Small and Medium-sized Enterprises) and non-SMEs.
A survey was conducted on global regulatory affairs personnel at Korean pharmaceutical companies. Subjects were selected by the following criteria: having regulatory experience in at least one member country of the Asia-Pacific Economic Cooperation (APEC), Association of Southeast Asian Nations (ASEAN), East African Community (EAC), Pan American Network for Drug Regulatory Harmonization (PANDRH) and planning to participate in 2018 Convention on Pharmaceutical Ingredients (CPhI). Questionnaires were sent by email to 60 global regulatory affairs personnel and a total of 28 responses were collected. Then, a meeting was held to consult with the respondents in detail about the regulatory barriers and their potential solutions. The questionnaires consisted of general items investigating the characteristics of the respondents and their companies surveyed, regulatory success and failure experience, and difficulties faced by companies in acquiring drug approval in foreign countries. Likert Scale from 1 to 5 was used to quantify the level of difficulties.
Study result revealed that in general, Korean pharmaceutical companies had harder time getting drug approval in the advanced countries such as the United-States and Eastern Europe compared to emerging countries. Smaller the profit size was and for SMEs in comparison to non-SMEs, there was lower success rate of drug approval. In part 2 where the extent of difficulty in global regulatory stages was gauged, significant difference existed between the size and the type of companies. In other words, smaller profit-sized companies and SMEs both perceived IND process relatively more difficult.
On the preference of agency support measures, smaller profit-sized companies and SMEs preferred 'Mutual recognition of regulatory standards' the most. Larger profit-sized companies and non-SMEs preferred both 'Mutual recognition of regulatory standards' and 'Establishment of information database for global regulatory standards' to the same extent.
The study result led to the conclusion that domestic pharmaceutical companies expect substantial regulatory harmonization of domestic standards with the global standard, specifically improvements in domestic standards of Good Clinical Practice(GCP) and Good Manufacturing Practice(GCP) inspection. In addition, more efficient system of disseminating information on overseas regulatory standards and guidelines would be effective in addressing the barriers in global regulatory process for the Korean pharmaceutical companies.
