분만중 발생한 치골결합 분리 - 증례 5예 = Separation of symphysis pubis during vaginal delivery - Report of 5 cases -

국문 초록 (Abstract)

치골결합은 두 치골의 양단이 섬유골판에 의해 결합되어 있는 반관절이다. 분만중 치골결합 분리는 드물며 빈도는 521명당 1명에서 30,000명당 1명으로 다양하다. 손상은 Progesterone과 Relaxin에 의해 약화되어 있는 골반 인대에 분만중 태아의 두부하강으로 인한 힘에 의해 야기된다. 예후는 아주 좋다. 이에 저자들은 분만중에 발생한 치골결합 분리 4례와 분만중 경막외마취 시술후 발생한 치골결합 분리 1례를 발견하여 경과 관찰하였기에 간단한 문헌 고찰과 함께 보고하는 바이다.

Symphysis pubis is a nonsynovial joint at the confluence of the pubic bones that is joined by fibrocartilagenous tissue. Peripartum pubic separation is an unknown event with incidence estimates ranging from 1:521 to 1:30,000. The injury is caused by the fetal head exerting pressure on pelvic ligaments that have been relaxed by progesterone and relaxin. Prognosis is excellent. We experienced 4 cases of peripartum symphyseal separation and 1 case of peripartum symphyseal separation with epidural anesthesia among 5487 deliveries from 1994 to 1998. We report these cases with a brief review of literatures.
