In order to evaluate the optimal dosage and the retinotoxicity of cefamandole nafate, rabbits were intravitreally injected with cefamandole nafate ranging from 0.125mg to 10mg. At the end of 1, 2, 4 and 8 weeks after cefamandole nafate injection, each group of rabbits was killed, and the eyes were enucleated. The retinal preparations were examined by light and electron microscope.
The results were summarized as follows:
1. With dosages of 0.5mg or less, there was no histological damage to the retina.
2. Histopathologically, with higher dosages 0.8 to 10mg, there were progressive destruction of retina including vaculoation of rod and cone cell layer, outer plexiform layer, ganglion cell layer, and outer nuclear layer, and clumping of melanin granules of pigment epithelium. Eventually there were complete loss of inner and outer segment of visual cells, and partially disorganization of outer nuclear layer. Phagocytic cells derived from the pigment epithelium appeared in the retina of animals treated with higher doses of 5mg of cefamandole nafate.
3. Ultrastructurally, there were mitochondrial swelling and formation of myelin-like structures within the degenerative mitochondria in the inner segment. Many myelin-like structures were also observed in the outer segment of the retina of animals injected with higher dosages of 1.25mg of cefamandole nafate.
4. There were marked increase of melanin pigments in phagocytic cells derived from pigment epithelia.
