본 연구는 일부 지역사회 노인들의 인구사회학적 특성, 건강관련행위특성, 건강상태, 사회적 지지망과 사회활동 등에 따른 우울수준을 파악하고, 우울의 중재변수로서 사회적 지지와 자기존중감, 상태불안, 대인의존형행동특성 및 통제신념과 같은 사회심리적 요인이 우울과 어떠한 관련성이 있는지 그 상대적 중요성을 평가하고자 시도하였다.
조사대상은 도시지역 노인 396명(남자 182명, 여자 214명), 농촌지역 노인 394명(남자 174명, 여자 220명), 합계 790명(남자 356명, 여자 434명)으로 하였으며, 조사는 2007년 4월 1일부터 6월30일까지의 기간 동안에 표준화된 무기명식 면접조사용 설문지를 사용하여 면접조사를 실시하였다.
그 결과 전체 조사대상자의 CES-D에 의한 우울수준 평균점수는 16.65±10.97점이었으며, 도시지역 노인이 18.81±10.31점, 농촌지역 노인이 14.49±11.20점으로 도시지역노인에서 더 높았다. 인구사회학적 특성별 우울수준 평균점수는 남자보다 여자에서, 65-74세군보다 75세 이상 군에서, 학력이 낮은 군일수록, 직업이 없는 군에서, 배우자가 없는 군에서, 혼자 사는 군에서, 월수입이 낮은 군일수록, 생활비를 자녀가 조달하거나 정부보조를 받는다는 군에서, 일상생활에 만족하지 못한다는 군에서 유의하게 높았다. 건강관련행위 특성별 평균 우울점수는 주관적인 수면의 질이 좋지 않다는 군에서, 흡연 군에서, 운동을 규칙적으로 하지 않는 군에서, 규칙적인 식사를 하지않는 군에서 유의하게 높았다. 건강상태별 평균 우울점수는 주관적인 건강상태가 좋지 않다는 군에서, 신체적인 부자유스러움이 있다는 군에서, 눈의 부자유스러움(잘 안 보임)이 있다는 군에서, 귀의 부자유스러움(잘 안 들림)이 있다는 군에서, 치아의 부자유스러움(음식물 씹기 어려움)이 있다는 군에서, 요실금이 있다는 군에서, 건망증이 있다는 군에서, 일상생활수행능력과 도구적 일상생활수행능력에서 도움이 필요하다는 군에서 유의하게 높았다. 사회적 지지망과 사회활동특성별평균 우울점수는 자녀수가 적을수록, 친구 수가 적을수록, 친구와의 접촉빈도가 낮을수록, 친척수가 적을수록, 외출 빈도가 낮을수록, 취미활동의 빈도가 낮을수록, 사회봉사활동의 빈도가 낮을수록 유의하게 높았다. 위계적 다중회귀분석 결과에 의한 조사대상 노인들의 우울수준에 영향을 미치는 요인으로는 사회심리적 요인을 독립적으로 회귀모델에 투입하였을 경우 이들의 설명력은 73.2%로 매우 높았으나 그 밖에 인구사회학적특성, 건강상태 및 건강관련행위특성, 사회적 지지망과 사회활동특성 등의 변수들을 추가적으로 투입하였을 경우에는 설명력이 크게 증가하지 않았다. 따라서 사회심리적 요인이 우울수준에 큰 영향을 미치고 있음을 알 수 있었다.
결론적으로, 노인들의 우울수준을 줄이기 위해서는 현재의 건강상태를 증진시킬 수 있는 다양한 방안의 모색과 사회심리적 요인을 적절하게 통제할 수 있는 프로그램의 개발 및 실시가 필요 할 것으로 생각된다.

다국어 초록 (Multilingual Abstract)

Objectives: This study was performed to determine the levels of depression symptoms among the community elderlies and to reveal its related factors, specifically aimed at revealing psycho-social factors such as social support net-work, sense of self-esteem, state anxiety, interpersonal behavior trait and locus of control.
Methods: The interviews were performed during the period from April 1st to June 30th, 2007 to the 790 elderlies in urban and rural areas.
Results: The mean scores of depression (CES-D) among all subjects were 16.65±10.97(urban: 18.81±10.31, rural: 14.49±11.20) and they were higher in urban than rural. The level of depression symptoms was influenced by the variables of educational level, with or without spouse, bear for living expenses, sense of satisfaction in daily life. As for health related behaviors, they were influenced by the variables of with or without regular exercise. As for health status, they were influenced by the subjective health status, with or without disability of body, visual acuity, hearing ability, ability of food mastication, urinary incontinence, with or without help in activity of daily living (ADL) and instrumental activity of daily living (IADL). As for social support net-work and social activities, they were influenced by number of offspring, frequency of offspring contents, frequency of going out and activity of hobbies. As for psycho-social factors, they were influenced by sense of self-esteem, state anxiety and social support, especially, the level of depression symptoms was higher influenced by the variables of psycho-social factors than other variables.
Conclusions: Lower of the level of depression symptoms among the community elderlies would require development and application of programs to keep under management psycho-social factors as well as looking for ways to promoting the present health status.
