고도의 진구성 고관절 탈구에서 인공 고관절 전치환술 시 이용된 전자하부 이중 Chevron 절골술

국문 초록 (Abstract)

목적: 고관절의 고도의 진구성 탈구 환자에서 대퇴골 전자 하부의 이중 chevron 절골술을 동반하여 시행한 인공 고관절 전치환술에 대하여 보고하고자 한다.
대상 및 방법: 1997년 4월부터 2001년 10월 사이에 선천성 고관절 탈구 및 화농성 관절염의 후유증으로 인한 고도의 진구성 완전 탈구(Crowe 분류 Ⅳ) 진단 하에 대퇴골 전자하부 이중 chevron 절골술을 이용한 인공 고관절 전치환술을 시행받은 6예 6명의 환자를 대상으로 하였으며 환자의 평균 연령은 평균 44.5세로 최소 19세, 최고 64세였다. 수술 전 및 수술 후의 임상적 평가를 위하여 Harris Hip Score와 하지 부동을 측정하였으며 수술 직후 및 추시 방사선 사진을 통하여 골유 합 여부 및 비구와 대퇴골의 해리 여부를 관찰하였다.
결과: Harris Hip Score는 수술 전 평균 61점에서 수술 후 최종 추시 결과 평균 94점으로 증가하였다. 하지 부동은 수술 전 평균 6 ㎝ (4-7.8 ㎝)에서 수술 후 평균 1.8 ㎝ (0-2.8 ㎝)으로 평균 4.1 ㎝ 연장되었다. 5예에서 수술 후 4개월에서 골유합을 얻었으며 최종 추시까지 비구와 대퇴골의 해리는 관찰되지 않았다. 합병증으로는 1예에서 수술 후 대퇴 신경의 감각신경분지의 부분마비를 보였으나 수술 후 3개월에 완전히 회복되는 소견을 보였으며, 1예에서 수술 후 9개월에 감염으로 인한 재치환술을 시행하였다.
결론: 고관절의 고도의 진구성 완전 탈구 환자에서 전자하부 이중 chevron 절골술을 동반한 인공 고관절 전치환술을 시행하여 중기 추시 결과 임상적 및 방사선학적으로 좋은 결과를 얻었으며 향후 지속적인 추시 관찰을 요할 것으로 생각된다.

다국어 초록 (Multilingual Abstract)

Purpose: This study evaluated total hip replacement arthroplasty (THRA) combined with a double-chevron subtrochanteric osteotomy for the treatment of neglected highly dislocated hips in adults.
Materials and Methods: Six cases treated with THRA combined with a double-chevron subtrochanteric osteotomy under the diagnosis of a neglected highly dislocated hip (Crowe classification Ⅳ) from April 1997 to October 2001 were followed up for between 21 months and 75 months. Harris hip scores, leg length discrepancy pre and postoperatively, osteotomy site union and implant loosening were chekced.
Results: The average Harris hip score increased from 61 points preoperatively to 94 points postoperatively. The average leg length discrepancy was decreased from 6 ㎝ preoperatively to 1.8 ㎝ postoperatively. Bone union of the osteotomy site was obtained at 4 months after surgery. The radiographs showed no acetabular and femoral loosening sign at the latest follow up.
Conclusion: According to the clinical and radiological assessments, THRA combined with double chevron subtrochanteric osteotomy gave satisfactory results in adults with a neglected highly dislocaled hip in adults on a midterm follow up.
