한국과 대만의 건강보험제도 비교연구 = 체계비교와 제도형성

국문 초록 (Abstract)

본 연구는 한국과 대만의 건강보험체계를 비교분석하여 결정적 차이점을 찾고 이로 인한 사회정책의 결과를 설명하기 위한 것이다. 이러한 연구진행을 위해 비교방법론 중 최대유사체계설계(Most Similar Systems Design)방법을 적용하였다. 왜냐하면 두 나라는 비슷한 역사적 맥락을 가지고 있고 건강보험체계도 같은 NHI모형에 속하기 때문이다.
비교분석의 결과 보건의료재정지출지표면에서는 매우 비슷한 수준에 있고, 건강보험체계지표에서는 대부분의 지표들이 유사한 수준에 있음에 반해 지불방식항목에서 결정적 차이점이 있다. 지불방식이 한국은 행위별수가제이고 대만은 총액예산제이며, 현상지표로서 국민건강보험에 대한 국민만족도에 현저한 차이가 있는 것으로 나타났다. 본 연구는 비교론적 시각에서 이러한 결정적 차이요소가 현상의 차이에 대한 원인요소가 되는지를 분석하였다. 아울러 대만이 한국과 달리 총액예산제도라는 지불제도개혁을 시행할 수 있었던 요인들을 사회정책형성이론의 맥락에서 살펴보았다.
건강보험의 지불제도개혁은 이익집단정치의 게임이기 때문에 정치적 타이밍, 이해당사자의 협력과 그것에 대한 인센티브, 그리고 국가가 주도하는 제도적 설계가 중요함을 제시하였다.

다국어 초록 (Multilingual Abstract)

This study is trying to explain the social outcomes due to the critical difference between health insurance of Korea and Taiwan through comparative analysis. For this purpose, MSD which is one of the comparative method was applied. because historical context of Korea is similar to that of Taiwan and two countries have same health insurance system, NHI.
As a result, Korea and Taiwan are little different in health expenditure, health insurance system. but there is critical difference in payment system, fee for service in Korea and global budgeting in Taiwan. and as a index of social outcome, the people's satisfaction on NHI is higher than that of Korea. This study analysed that the difference of payment system cause the difference of satisfaction and explored the factors which make Taiwan successful in introducing global budgeting system.
The reform of payment system in health insurance is characterized as politics of interests group game so that political timing, collaboration of stake holders and incentives, institutional design lead by state are important.
