연구배경 및 의의: 성격장애는 흔하게 만날 수 있는, 심각한 지장을 초래하기도 하는 질환으로, 임상적으로나 사회적 측면에서 그 문제점이 범세계적으로 부각되고 있는 추세임. 성격장애에 관한 이해와 연구는 이 병에 대한 제대로 된 평가에서부터 시작되어야 하나, 현재의 성격장애 평가는 그 내용이 정확하지 않고 대체적으로 진단을 신뢰하기 어려운 상황이므로 개선이 절실히 요구됨. 성격장애 환자는 흔히 DSM-IV 진단기준 축 1의 질환이 공존하는 데, 성격장애가 공존할 경우 축 1 상태의 치료성과에 큰 영향을 미치게 되기 때문에, 정신질환에 대한 적절한 치료를 위해서는 공존하는 성격장애에 대한 이해가 절실히 요망됨.
목적: 본 연구과제의 목적은 섭식장애 환자들을 대상으로 기록과 정보에 근거한 성격장애 평가절차 (Schedule for Personality Assessment from Notes and Documents; SPAN-DOC) 를 통해 다음의 논점들을 밝히는 것임: (a) 섭식장애의 세 형태인 신경성 식욕부진증, 신경성 폭식증 및 최근 주목받는 카테고리인 달리 분류되지 않는 섭식장애 (Eating Disorders Not Otherwise Specified, EDNOS) 에서의 성격 상태의 차이 (b) SPAN-DOC으로 평가한 성격상태가 섭식장애 및 치료성과에 미치는 영향 (후향적 연구)
방법: 본 연구는 영국 런던 소재 섭식장애 3차 진료기관에서 수행됨. 총 90명의 섭식장애 환자들이 참여하였으며, 각 진단별로는 신경성 식욕부진증 47명, 신경성 폭식증 32명, 대식증 9명, 정서적 섭식장애 2명임. 모든 환자들은 SPAN-DOC 을 이용해 성격정도를 평가했으며, 더불어 자가 설문을 이용해 현 진단체계에 근거한 성격장애 평가, 총체적인 일반정신병리 평가와 치료 전후 섭식장애의 병리를 평가하였음. 아울러 SPAN-DOC 의 신뢰도 및 타당도 연구도 함께 진행하였음.
결과: SPAN-DOC 결과 4 개의 성격유형이 도출되었음. 이는 지나친 완벽성을 요인으로 하는 '완벽주의형' (42명), 충동성, 분노, 취약성 등을 요인으로 하는 '취약형' (25명), 불안, 예민성, 자신에 대한 무가치함, 자살성 등을 요인으로 하는 불안/의존형 (21 명), 정서적 의존요구, 기분기복, 과시성 등을 요인으로 하는 불꽃형 (n=2) 으로 분류되었음. 섭식장애 환자의 69.5 % 는 성격에 문제가 있는 것으로 평가되었으며, 구체적으로는 34.9% 는 '성격곤란', 24.3% '성격장애' 로 진단 가능하였으며, 2,4% 는 '심각한 성격장애' 였음. 성격장애의 심각성과 섭식장애의 정도 및 단기치료효과와는 뚜렷한 상관성을 찾을 수 없었음.
파급효과: 본 연구결과는 의무기록을 이용한 성격장애 평가 절차인 SPAN-DOC 이 효과적인 평가 수단임을 시사함. 연구에서 얻어진 데이터는 섭식장애환자에서의 성격분포는 4유형으로 분류하는 것이 타당함을 입증함. 본 연구를 통한 세계적인 성격장애 선도그룹과의 연계는 성격장애에 대한 총괄적인 이해와 더불어 우리나라에서의 이 분야 연구의 질적 향상 및 국제적 저변 확대를 도모할 수 있게 함. 아울러 본 연구 과제 수행을 통해 성격장애의 효과적인 선진치료기법들을 습득함으로써 성격장애 치료비용 절감뿐 아니라, 섭식장애를 포함한 축 1 질환의 치료성과를 높일 수 있게 됨.

Background and Significance : Personality disorders are common, create considerable morbidity, are associated with high costs to health care services and to society, and negatively interfere with the progress of Axis I mental disorders. The tools currently used to assess personality disorders are inaccurate, largely unreliable, frequently incorrect and in need of improvement. Personality disorders are frequently comorbid with Axis I conditions, and the comorbidity of these disorders influences the therapeutic outcomes. Thus, this comorbidity needs to be better understood in order to improve the treatment planning process.
Objectives : The aim of this study was to evaluate the Schedule for Personality Assessment from Notes and Documents (SPAN-DOC) in patients with eating disorders. The SPAN-DOC was evaluated on the basis of the following: (a) differences in personality status among patients with anorexia nervosa, bulimia nervosa, and eating disorders not otherwise specified (EDNOS) was assessed; and (b) the impact of personality on eating disorders and treatment outcome was investigated (retrospective study).
Methods : The study was conducted at the tertiary national referral center for eating disorders in the United Kingdom. A total of 90 patients with eating disorders consisting of 47 patients with anorexia nervosa, 32 with bulimia nervosa, 9 with binge eating disorder and 2 with food avoidant emotional eating disorder were participated the study. All patients were evaluated using the SPAN-DOC. Participants were also subjected to a comprehensive evaluation, including a self-reported evaluation of personality disorder, general psychopathology, and an evaluation of eating disorders at the time of assessment and the end of treatment.
Results : Patients categorised as Perfectionistic group, Vulnerable group, Anxious/Dependent group, and Flamboyant group. 69.5 % of patients with eating disorders showed problems in their personality; 34.9% were diagnosed as a personality difficulty, 24.3% were diagnosed as a personality disorder, and 2,4% were diagnosed as a severe personality disorder. Severity of personality disorder was not significantly associated with severity of eating disorder as well as short-term treatment outcome.
Dissemination : The findings of the study showed the SPAN-DOC is an effective tool for assessing personality disorders. The data provided accumulating evidence for the validity of four personality subtypes in patients with eating disorders. The educational value of this study being linked to world leading personality disorder research group made the researcher to develop a comprehensive understanding of personality assessment, increase the quality of research in Korea, and promote its international expansion. The clinical value of this study was the comprehensive acquisition of personality disorder assessment and its treatment, which would improve the poor outcome of patients with comorbid Axis I disorders, including eating disorders, as well as personality disorder itself. This would be solid basis in coordination with other countries of the international research network for revisional work of ICD-10 of WHO.
