노인장기요양보험의 서비스 질 관리 개선에 관한 연구:영국의 질 관리 시스템과 그 시사점을 중심으로 = A Study on the development of the Control of Service Quality in Long-Term Care Insurance in Korea: Focusing on the quality control system of the UK and suggestions

The purpose of this research is to look at the overall policy on the control of service quality, and then to find policy suggestions in Korea. Firstly, I discuss the policy on the control of service quality in the New Labour Government at the macro level. Based on this, the fuction and role of Commission for Social Care Inspection(CSCI) are discussed. Then, National Minimum Standards and the management of social care workforce are examined, respectively. By looking at the policy and experience of the UK, I try to find policy suggestions considering the peculiar conditions in Korea at the moment. According to this research results, the three major policy suggestions can be found.
Firstly, an ‘independent’ inspection commission for long-term care needs to be developed in the mid-long term. However, considering the conditions of Korea, we need to develop a platform for the systematic control of service quality by emphasizing the function of monitoring of National Health Insurance Corporation(NHIC) in the short term.
Secondly, National Minimum Standards needs to be prepared which is appropriate to the overall conditions in Korea. By accumulating and using the experiences of NHIC, the standards which the needs of service user are centred, need to be developed. It is desirable that this can be used as a basis to inspect care service providers.
Thirdly, the control system for care service workforce should be devised in a national level. By providing systematic education and training for the workforce, it might be necessary that care workforce can devote themselves for service users beyond the interests of service providers.

국문 초록 (Abstract)

본 연구의 목적은 영국의 서비스 질 관리 정책에 대해서 살펴보고, 한국에의 정책적 시사점을 찾는데 있다. 영국 신노동당 정부의 서비스 질 정책에 대해서 먼저 거시정책적 맥락에서 살펴봤다. 이를 바탕으로 서비스 질 감독기구(CSCI)의 기능과 역할에 대해서 논의한 후, 서비스 제공의 국가 최소 기준과 서비스 제공인력의 관리 시스템을 각각 점검했다. 영국의 정책과 경험을 통해서, 한국의 특수상황을 고려해서 시사점을 찾고자 시도했다. 본 연구 결과, 크게 세 가지의 정책적 시사점을 찾을 수 있었다. 첫째, 중장기적으로 장기요양 서비스의‘독립적인’감독 기구의 설립을 진지하게 논의할 필요가 있다. 그러나, 한국의 현실을 고려할 때 단기적으로는 국민건강보험공단이 모니터링 기능을 강화함으로써 체계적인 질 관리 감독의 토대를 마련할 필요가 있다. 둘째, 한국적인 상황에 맞는 서비스 질의 국가 최소 기준을 마련할 필요가 있다. 공단의 경험을 바탕으로 해서 서비스 이용자의 욕구가 중심이 되는 기준을 마련, 이를 통해서 서비스 제공 기관을 평가할 수 있는 근거 기준으로 활용하는 것이 바람직하다.셋째, 서비스 질의 중요한 축인 서비스 제공 인력에 대한 국가적인 차원의 관리 시스템이 구축되어야 한다. 이들에게 체계적인 교육과 훈련을 시켜서 궁극적으로는 서비스 제공 인력들이 해당 기관의 이해관계를 넘어서 서비스 이용자의 입장에서 일할 수 있도록 하는 것이 절실할 것이다.
