독일의 건강보험체계와 통일 이후의 제도 변화 = A Study on the german Health Insurance System and Institutional Changes after Reunification

다국어 초록 (Multilingual Abstract)

The German health insurance system, which introduced the social insurance system, was reorganized from a system divided into East and West Germany to a system based on West Germany after it was reunited in 1990. After the enactment of the Disease Insurance Act in 1883, it reached its 135th anniversary, and 26 years after the unification, the German health insurance system experienced considerable changes. This German experience is still the most important example to prepare for future unification for us, which remains a divided nation. Nevertheless, there is still a lack of basic understanding of the German health insurance system. First, look at an overview of the health insurance system. Also, we will briefly review the changes in the new health insurance system before and after the reunification and the reform process of the health insurance legislation. This leads to implications for how to create a unified Korea health insurance system. These studies will help to set reform direction of health insurance in the future and to carefully examine the increase in the self burden for strengthening the protection. When designing a unified Korean health insurance system, it is necessary to consider the impact of the German labor market reform in the 2000s on the social insurance system. Of course, this should be planned in order to formulate a new legal system and to formulate the transitional provisions in order to rebuild the social insurance system in the new state.

국문 초록 (Abstract)

사회보험제도를 태동시킨 독일의 건강보험제도는 1990년 재통일을 맞이하면서 동· 서독으로 양분된 체계에서 서독 중심의 체계로 재편되었다. 1883년 질병보험법 제 정 이후 135주년을 맞이하였고, 통일 후 26년이 경과하면서 독일의 건강보험체계는 상당한 변화를 경험하였다. 이러한 독일의 경험은 아직 분단국가로 남아 있는 우리 에게 향후 통일을 대비한 가장 중요한 사례가 되고 있다. 그럼에도 불구하고 독일 건강보험체계에 대한 기본적인 이해가 여전히 부족한 현실 에서 먼저, 건강보험체계를 개관하고, 통일 전·후 동독지역 건강보험체계의 변화와 실질적인 건강보험법제의 개혁과정을 파악함으로써 통일 한국의 건강보험체계를 어떻게 조성할 것인가에 대한 시사점을 도출하였다. 이는 향후 건강보험의 개혁 방향성을 설정하고 보장성 강화를 위한 본인부담율 증가 문제를 신중히 검토하는데 도움이 될 것이다. 또한 2000년대 독일의 노동시장개혁이 사회보험체계에 미친 영 향을 고려하는 한편, 동독지역 사회보험체계의 재구축을 위한 경과규정의 구체화와 새로운 법체계의 수립이라는 차원에서 통일 한국의 건강보험체계를 설계할 필요가 있다.
