본 연구는 국내 의료폐기물 관리체계가 감염성 제어 중심으로 운영되는 과정에서, 화학·공정·환경 안전이 독립적으로 관리됨에 따라 발생하는 구 조적 한계를 규명하고, 이를 해소하기 위한 데이터 연계 기반의 ‘통합안전 관리체계’를 제안하는 데 목적이 있다. 이를 위해 의료폐기물 관리와 연관 된 「폐기물관리법」, 「화학물질관리법」, 「산업안전보건법」, 「약사 법」, 「대기환경보전법」, 「감염병예방법」, 「교육환경법」등 관련 법 령을 분석하고, 의료폐기물의 열화학적 특성 및 처리 시나리오별 경제성· 환경성을 다룬 선행연구 데이터를 심층 검토하여 현행 제도의 문제점을 실증적으로 진단하였다.
선행 연구에서 의료폐기물은 휘발분 81.3%, 탄소 함량 75.9%에 달하는 고 위험 에너지원임이 밝혀졌음에도 불구하고, 현행 제도는 이에 대한 성상 정보를 수집하지 않는 불완전한 관리를 지속하고 있음이 확인되었고, 고발 열량(5,250kcal/kg) 의료폐기물의 성상 고려 없는 단순 소각은 질소산화물 (NOx) 농도를 약 30% 급증시키는 등 공정 불안정성을 유발하는 것으로 보고되었다. 본 연구는 이러한 문제점이 제도적 한계에서 기인함을 규명하 고, 이를 ① 성상 정보 부재, ② 유해 물질 경로 추적 불가, ③ 공정 안전 정보 미연계, ④ 피드백 제어 미흡으로 구성된 ‘4대 정보 비연계 요인’으로 정의하였다.
이러한 한계를 극복하기 위해 본 연구는 ‘3단계 통합안전 프레임워크’를 제안하였다. 1단계(디지털 정보 통합)에서는 병원정보시스템(HIS)과 올바 로시스템을 연동하여 화학적 성상 정보에 대한 디지털 인벤토리를 구축한 다. 2단계(반입 제어 및 분류)에서는 성상 기반의 반입 허용기준(WAC)을 법제화하고 전처리를 의무화하여 고위험 물질의 소각 공정 내 유입을 통 제한다. 3단계(공정 최적화 및 분산 처리)에서는 멸균 잔재물의 자원화 (SRF)를 위한 법·제도적 기반 하에, 거점형 공동 처리(Hub & Spoke) 모 델과 최적가용기법(BAT)을 도입하여 처리의 경제성과 사회적 수용성을 동시에 확보한다.
본 연구의 타당성을 검증하기 위해 선행 연구 결과를 기반으로 볼 때, 온 사이트 전처리 및 자원화 도입 시 기존 위탁 소각 대비 온실가스 배출량은 55.6%, 처리 비용은 약 54~60% 절감되는 등 본 정책 도입시 경제적·환경 적 기대효과가 있을 것으로 분석되었다. 특히 환경부가 최근 인화성 의료 폐기물에 대해 유해성정보자료(HIS) 작성을 의무화한 조치는, 관리체계가 물량·감염성 중심에서 정보연계 기반의 통합안전 관리로 전환되기 시작한 긍정적 신호로 평가된다. 결론적으로 본 연구는 의료폐기물의 감염성·화학·공정·환경 안전 등 요소 간 통합관리를 위해 유기적 연계 관리체계를 제시하였으며, 이는 선진국 수준의 안전 기준과 국민의 건강권을 동시에 충족하는 한국형 통합안전 관리체계 구축의 실효성 있는 기초 자료로 활용될 수 있을 것이다.

This study is to identify the structural limitations arising from the independent management of chemical, process, and environmental safety within the current domestic medical waste management system, which is operated with a focus on infection control, and to propose a data connectivity-based 'Integrated Safety Management System' to resolve these issues. To this end, this study analyzed relevant laws including the 「Waste Control Act」, 「Chemical Substances Control Act」, 「Occupational Safety and Health Act」, 「Pharmaceutical Affairs Act」, 「Clean Air Conservation Act」, 「Infectious Disease Control and Prevention Act」, and 「Educational Environment Protection Act」. Furthermore, it empirically diagnosed the problems of the current system by in-depth reviewing empirical data from prior studies regarding the thermochemical characteristics of medical waste and economic and environmental feasibility by treatment scenario.
Although prior studies have revealed that medical waste is a high-risk energy source with volatile matter reaching 81.3% and carbon content of 75.9%, it was confirmed that the current system persists in incomplete management that does not collect property information. It was also reported that simple incineration of high-calorific medical waste (5,250 kcal/kg) without consideration of its properties causes process instability, such as a surge in nitrogen oxide (NOx) concentrations by approximately 30%. This study identified that these problems stem from institutional limitations and defined them as the 'Four Major Information Non-linkage Factors,' consisting of: ① Absence of property information, ② Untraceability of hazardous material pathways, ③ Non-linkage of process safety information, and ④ Insufficiency of feedback control.
To overcome these limitations, this study proposed a '3-Step Integrated Safety Framework.' In Step 1 (Digital Information Integration), a digital inventory for chemical property information is established by linking the Hospital Information System (HIS) with the Allbaro System. In Step 2 (Inflow Control and Classification), the inflow of high-risk materials into the incineration process is controlled by legislating property-based Waste Acceptance Criteria (WAC) and mandating pre-treatment. In Step 3 (Process Optimization and Decentralization), economic feasibility and social acceptance are simultaneously secured by introducing a 'Hub & Spoke' model and Best Available Techniques (BAT) under the legal and institutional basis for resource recovery (SRF) of sterilized residues.
In validating the feasibility of this study based on the results of prior studies, it was analyzed that the introduction of on-site pre-treatment and resource recovery would have economic and environmental expected effects, such as reducing greenhouse gas emissions by 55.6% and treatment costs by approximately 54~60% compared to existing consignment incineration. In particular, the Ministry of Environment's recent measure mandating the preparation of Hazardous Information Sheets (HIS) for flammable medical waste is evaluated as a positive signal that the management system is beginning to transition from a volume and infection-centric approach to information connectivity- based integrated safety management.
In conclusion, this study presented an organically linked management system for the integrated management of elements such as infectious, chemical, process, and environmental safety of medical waste. This will serve as effective foundational data for establishing a 'Korean-style Integrated Safety Management System' that simultaneously satisfies safety standards at the level of developed countries and the public's right to health.
