Since the development of a highly sensitive radioimmunoassay with a specific affinity for the beta-βsubunit of hCG, it has been possible to measure the level of hCG as low as 1.25 mIU/ml without endogenous hLH interference within a few days following implantation. It can be used to establish the diagnosis very early pregnancy, suspected ectopic or intrauterine abnormal pregnancy with negative pregnancy test and provided a valuable clinical aid in the management of threatened abortion. In order to evaluate diagnostic value of serum hCG -βsubunit in normal and abnormal early pregnancy, sera obtained from 32 cases which consist of 17 cases of early normal pregnancy, 7 cases of ectopic gestation and 8 cases of abnormal intrauterine pregnancy, were analyzed. The results were as follows. 1) The specific and nonspecific serum hCG levels could be detected as early as the 26th day from LMP and rose sharply in parallel with gestational age until the 40th to the 50th day from LMP. 2) The nonspecific serum hCG levels in normal pregnancy closely paralleled with dose in the urine as measured with Pregnosticon Plano slide test. 3) The hCG -subunit levels in ectopic pregnancy ranged from 874 mIU/ml to 5,200 mIU/ml, and they were lower than the levels of normally progressing pregnancy at the same gestational age. 4) The nonspecific and specific serum hCG levels in abortive ectopic gestation were higher than in the ruptured type. 5) In abnormal intrauterine pregnancy, the specific and nonspecific serum hCG levels were significantly lower than those in normal pregnancy, but in nonabortive type of threatened abortion, hCG levels were similar to normal pregnancy. 6) Specific radioimmunoassay of serum hCG was the most reliable test in diagnosed both early normal pregnancy and ectopic gestation and may be of some value in prediction of the outcome for the threatened abortion.
