삼중음성유방암(triple-negative breast cancer, TNBC)은 치료 옵션이 제한된 고위험 아형으로, 항암제 내성이 주요 임상적 난제로 남아 있다. 세포골격 결합 단백질인 Moesin(MSN)은 TNBC 환자에서 발현이 상승되어 있음이 보고되었으나, MSN이 유방암의 발생 및 항암제 내성 과정에서 수행하는 구체적인 분자적 역할은 명확히 규명되지 않았다. 이에 본 연구에서는 MSN이 IL-6 의존적 신호전달 경로 내에서 암줄기세포(cancer stem cell, CSC) 특성과 항암 내성을 조절하는 핵심 매개 인자로 작용하는지를 규명하고자 하였다.
실험 방법:
본 연구는 MSN 의존적 자가분비 경로(autocrine loop)로서 extracellular interleukin-6 (IL-6)과 NF-κB 간의 상호작용을 중심으로 수행되었다. 또한 GTPase 매개 STAT3 인산화 신호경로를 포함한 MSN의 분자적 작용 기전을 규명하기 위해, 다양한 in vitro 세포주 모델(MDA-MB-231, Hs578T, BT-20, MDA-MB-468)와 in vivo 생쥐 이식 모델을 이용하였다. MSN의 발현 수준에 따른 종양 개시능(tumor initiation), 구형형성(sphere formation), ALDH 활성도, 및 CSC 마커(CD24⁻/CD44⁺) 변화를 분석하였으며, RNA sequencing 및 공개 데이터베이스(METABRIC, TCGA)를 통한 통합 생물정보학 분석을 병행하였다.
실험 결과:
MSN의 높은 발현은 TNBC 환자에서 전체 생존율(overall survival) 및 무병 생존율(disease-free survival) 감소와 유의한 상관성을 보였다. In vivo 모델에서 MSN은 종양의 개시 및 성장을 촉진하였다. 기전적으로, MSN은 IL-6/NF-κB 자가조절 피드백 루프를 통해 IL-6 전사 활성을 증가시켰으며, IL-6의 LPAR1 결합은 MSN의 인산화를 유도하였다. 이어 CDC42–PAK4 복합체의 활성화를 통해 pSTAT3–MSN 복합체의 핵 내 이동을 촉진하였고, 그 결과 IGFN1, EML1, SRGN 등 암줄기세포 관련 유전자의 발현을 증가시켜 Adriamycin 내성을 유발하였다. 흥미롭게도, FDA 승인 STAT3 억제제인 Atovaquone의 단독 또는 Adriamycin 병용처리는 MSN-STAT3 신호 경로를 억제하여 세포 생존율을 감소시키고, 항암제 내성을 회복시켰다.
실험 결론:

Resistance to chemotherapy remains a major clinical challenge in triple-negative breast cancer (TNBC), an aggressive subtype characterized by the absence of estrogen receptor, progesterone receptor, and HER2 expression. Although moesin (MSN), a member of the ERM (ezrin-radixin-moesin) family that links the actin cytoskeleton to the plasma membrane, is markedly upregulated in TNBC, its precise role in tumor progression and therapeutic resistance has not been fully elucidated.
Methods
We investigated the molecular mechanism by which MSN regulates the IL-6/LPAR1/NF-κB/STAT3 signaling axis to promote cancer stemness and chemoresistance in TNBC. To elucidate the functional significance of MSN, A combination of in vitro and in vivo experiments was performed, including loss- and gain-of-function approaches using siRNA/shRNA-mediated knockdown and plasmid-based overexpression, Cancer stem cell- like properties were assessed through sphere-formation and limiting dilution assays, while molecular interactions were examined by co-immunoprecipitation, and immunoblot analyses. The effects of MSN modulation on tumor growth and therapeutic response were further evaluated in xenograft mouse models. Collectively, these approaches were designed to comprehensively characterize the role of MSN in regulating tumor initiation, self-renewal capacity, and drug resistance.
Results
High MSN expression was significantly associated with shorter overall and disease-free survival in patients with TNBC. Mechanistically, MSN enhanced IL-6 transcription through an autoregulatory feedback loop involving IL-6 and NF-κB signaling. Upon IL-6 binding to the G-protein–coupled receptor LPAR1, sequential phosphorylation of MSN and the downstream CDC42–PAK4 complex was induced, facilitating the nuclear translocation of the phosphorylated STAT3–MSN complex. This complex functioned as a transcriptional activator of cancer stemness-related genes, including IGFN1, EML1, and SRGN, thereby promoting self-renewal and conferring resistance to Adriamycin. Importantly, pharmacologic inhibition of STAT3 using the FDA-approved inhibitor Atovaquone, in combination with Adriamycin, effectively restored chemosensitivity and markedly suppressed tumor growth in vivo.
Conclusions
Our findings demonstrate a pivotal role of the MSN–STAT3 signaling axis in orchestrating IL-6/LPAR1-mediated cancer stemness and chemoresistance in TNBC. This study uncovers a mechanistic link between MSN and the IL-6/NF-κB/STAT3 feedback circuit, identifying MSN as a previously unrecognized upstream regulator of STAT3 activation. By establishing the functional relevance of the pSTAT3–MSN nuclear complex in sustaining self-renewal and drug resistance, our work highlights the therapeutic potential of targeting this pathway. Moreover, these results provide a strong rationale for repurposing clinically approved STAT3 inhibitors, such as Atovaquone, to restore chemosensitivity and improve therapeutic outcomes in TNBC patients with elevated MSN expression.
