This study is intended to identify how much a 12-week initial exercise program(5 cycles or more a week; 60 min. per cycle) after reconstruction of Anterior crucial ligament may have effects on the myofunction of knee joint, femoral circumference and the functional assessment of knee joint. In this study, 34 subjects with their Anterior crucial ligament reconstructed( exercise group and control: 17 persons respectively) were asked to join in a rehabilitation program devised herein; Exercise group was asked to join in 2-week isometric exercises from the day after operation, and then in isometric and uniform exercises under the formulated guidance of hospital for the benefit of their ultimate rehabilitation. Even after discharge(in 4 weeks after operation), they were guided to keep on with exercises in neighborhood sports center, while control group was asked to join simply in the orientation about rehabilitative exercise program, because many of control were supposed to leave hospital early for personal reasons.
Based on these research procedures hereof, the findings of this study can be summarized as follows:
1. Myofunction of knee joint
1) For myofunction of normal extensor, exercise group exerted stronger muscle power(angular velocity = 180°sec) than control(p <.001). Though the former group also exerted a little stronger muscle force(angular velocity = 60°/sec) and muscular endurance(angular velocity = 300°/sec) than the latter, there was not any significant difference on statistical level.
2) For myofunction of normal hamstring, exercise group exerted stronger muscle force(angular velocity = 60°/sec) and power(angular velocity = 180°/sec) than control(p <.001). In addition, the former group also exerted significantly higher muscular endurance(angular velocity = 300°/sec) than control on statistical level(p <.01).
3) For myofunction of affected extensor, exercise group exerted stronger muscle force(angular velocity = 60°/sec) and power(angular velocity = 180°sec) than control(p <.01). In addition, the former group also exerted significantly higher muscular endurance(angular velocity = 300°/sec) than control on statistical level(p <.05).
4) For myofunction of affected hamstring, exercise group exerted stronger muscle force(angular velocity = 60°/sec; p <.01) and power(angular velocity = 180°/sec; p <.05) than control. Although the former showed a little higher muscular endurance(angular velocity = 300°/sec) than control, there was not any significant difference on statistical level.
5) For the rate of myofunctional defects, control reached significantly higher rate of defect in the muscle force of extensor(angular velocity = 60°/sec) and hamstring(angular velocity = 60°/sec) than exercise group on statistical level(p <.05).
6) For the ratio of protagonist and antagonist(angular velocity = 60°/ sec), exercise group showed significantly higher ratio of them on normal (p<.001) and affected(p <.05) side than control on statistical level.
2. Femoral circumference
In regard to dimensional variations, exercise group showed significantly more extended femoral circumference on normal(p<.05) and affected(p<.001) side in 12 weeks after operation on statistical level than before. But control showed significantly more shortened femoral circumference on both normal and affected side(p <.05) than before operation on statistical level.
3. Functional assessment of knee joint
For the ratings from functional assessment of knee joint, both exercise group(p <.001) and control(p <.01) showed significantly higher ratings in 12 weeks after operation on statistical level than before.
Summing up, it is concluded that the 12-week initial rehabilitative exercise program for cases with Anterior crucial ligament reconstructed is helpful to enhance the myofunction of femoral knee joint in exercise group than in control, and also improve both femoral circumference and functional ratings of knee joint.
