This study examines whether the residential mobility career experienced during childhood and adolescence is associated with an elevated risk of mental disorders in adulthood within the Korean context, an issue that has recently drawn attention through large-scale cohort studies in Western countries but has yet to be addressed in South Korea. To this end, we constructed a large-scale cohort spanning 22 years (2002-2023) from the customized database of the Korean National Health Insurance Service (NHIS), comprising 6,668,197 individuals born between 1992 and 2002, which corresponds to a near-census of the corresponding birth cohort group. For each individual, residential moves across municipal (si-gun-gu) boundaries during late childhood and early adolescence (ages 10-15) were tracked, and the initial diagnosis of a mood disorder (ICD-10 F30-F39, encompassing depressive, bipolar, and persistent mood disorders) was observed from late adolescence into early adulthood (ages 16-25). Of the cohort, 9.7% (646,627 individuals) received an initial mood disorder diagnosis during the follow-up period. A survival analysis using a Cox proportional hazards model, stratified by birth year and municipality of residence at age 10 and adjusting for individual and regional characteristics, revealed a clear dose-response relationship: compared with those who never moved, the hazard of a mood disorder diagnosis was 11.6% higher for those who moved once (HR = 1.116), 24.9% higher for those who moved twice (HR = 1.249), and 39.1% higher for those who moved three or more times (HR = 1.391), indicating that more frequent residential mobility is associated with a higher risk of mental disorders. These findings provide population-scale empirical evidence that, as in Western societies, frequent residential mobility during childhood and adolescence may be associated with an elevated risk of mental disorders in adulthood in South Korea as well. Based on this result, we propose a new policy concept of Growing in Place (GIP) - supporting children and adolescents to grow up within stable residential environments in their communities - and discusses its policy directions for integrating housing welfare and public health.
