모바일 환경에서 국제표준에 기반한 개인건강기록 정보교류 시스템 구축 및 다기관 임상적용 서비스 플랫폼 개발 = Development of PHR Data Exchange Netoworks and a Platform using CCR/CCD Standard in Mobile Smartphone Environment

다국어 초록 (Multilingual Abstract)

Purpose: 1) CCR+ and CarePlatform : Development of personal health record exchanging system and the service platform for using in mobile smart-phone environment based on the international PHR standards CCR/CCD, 2) CancerCare: Development of mobile PHR app based self management system and service model for cancer patients, 3) DialysisNet: Constructing and operating of the pilot for clinical data exchange among multi medical centers for hemodialysis patients with mobile PHR app.
Research Contents : 1) Standardize contents of International standards, ASTM CCR (Continuity of Care Record) and HL7 CCD (Document) in Korean and Develop operating technique of mobile PHR for patient centered integration and exchange of personal health data, 2) Develop ISO/IEC 11179 metadata standard based mapping algorithm for mapping between CCR/CCD data elements and EMR information from participant institutions and construct semantic server for supporting of expandability of CCR/CCD contents, 3) CCR+, CancerCare & DialysisNet: Develop PHR plag and play app module for managing general PHR, high risk disease(cancer) and chronic disease(kidney failure), 4) DialysisNet: Construct a pilot system for clinical data exchanging and data sharing network among multi-medical centers and operate it in the test-bed in order to support continuous care and to provide availability of free movement for hemodialysis patients, 5) Construct a pilot for PHR operating and operate its fundamental technique for sharing clinical data among major university hospitals (SNU, GacheonUH, AjouUH, BusanUH, CatholicUH) and small clinics, 6) CarePlatform: Construct a platform (a Facebook-like Platform) and its APIs to support the development of distributed environment based decision making module for secure personal health data sharing.
Conclusion and expectation : 1) Improve continuous medical care through patient centered management by integrating scattered personal health records among multiple institutions, 2) Extend current PHR for general patients to high risk disease(cancer) and chronic disease(kidney failure), 3) Ensure technical skills for utilizing international standards of healthcare information and develop Korean standard for healthcare information, 4) Construct data exchanging network among multiple institutions and using it as collaborative development environment through building a smart platform.

연구목표 : 1) CCR+ and CarePlatform: 모바일 스마트폰 환경에서 지속진료기록 (CCR/CCD) 국제표준에 기반한 건강정보 교류 시스템 구축 및 서비스 플랫폼 개발, 2) CancerCare: 암환자 모바일 PHR 앱 기반의 자가관리 시스템 및 서비스 모델 개발, 3) DialysisNet: 혈액투석 환자 모바일 PHR 앱 기반의 다기관 정보교류 파일롯 구축 운영.
연구내용 : 1) 환자중심 개인건강정보의 통합 및 교류 국제표준인 ASTM CCR (Continuity of Care Record)과 HL7 CCD (Document) 컨텐츠의 한글 표준화 및 모바일 PHR 운영기술 개발, 2) 메타데이터 국제표준(ISO/IEC 11197)에 기반한 각 참여기관 EMR 정보의 CCR/CCD 데이터엘리먼트 매핑 알고리즘 구현 및 CCR/CCD 컨텐츠 확장성 지원 시맨틱 서버 구축, 3) CCR+, CancerCare & DialysisNet: 표준환자 PHR 및 중대질환(암)과 만성질환(신부전) 관리를 위한 PHR 플러그 앤 플레이 앱 모듈 개발, 4) DialysisNet: 혈액투석 환자의 공간이동성 및 진료지속성 지원을 위한 다기관 인공신실을 대상으로 정보교류 및 공유 네트워크 파일롯 시스템 구축과 시범운영, 5) 대학병원(서울대, 가천대, 아주대, 부산대, 가톨릭대) 및 지방의료원연합회 정보교류PHR 운영 파일롯 구축 및 운영기술 확보, 6) CarePlatform: 개인건강정보의 안전한 교류에 기반한 분산환경 인공지능 의사결정지원 모듈 개발 지원을 위한 플랫폼 (a Facebook-like Platform) 구축 및 API 개발 보급.
연구개발에 따른 기대성과 : 1) 여러 의료기관에 흩어진 진료정보의 개인중심 통합관리를 통한 진료지속성의 향상, 2) 현재의 표준환자용 CCR의 유용성을 실제 중대질환(암)과 만성질환(신부전) 관리로 확장, 3) 스마트 의료정보의 국제표준 기술력 확보와 한글표준 개발 및 의미론적 확장성 지원, 4) 다기관 정보교류 네트워크 구축과 스마트 플랫폼 구축을 통한 협업적 개발환경 확보
