    Background: Urban green spaces are emerging as a critical environmental health determinant, especially given that over 90% of the Korean population lives in urban areas with limited green space. Green spaces provide a variety of ecological functions, including air pollution mitigation and temperature regulation, while also promoting physical activity and psychological well-being. Children are particularly vulnerable because environmental exposure during the critical developmental periods of fetal and infancy can have lifelong health consequences. Congenital anomalies (CA) and allergic rhinitis (AR) are two pediatric health problems that highlight this concern. Prenatal exposure to green spaces may offer protection against birth defects by reducing maternal exposure to environmental stressors such as air pollution and extreme heat, and by promoting physical activity. Furthermore, while green spaces improve air quality, they can also serve as a source of allergenic pollen. Both diseases result in significant healthcare costs. In Korea, the infant mortality rate due to CA is projected to reach 0.4 per 1,000 live births in 2024, and the annual treatment cost of AR exceeds 695 billion won, with children aged 5 to 9 bearing the greatest burden. Despite growing research interest, relevant evidence remains limited and inconsistent. Most studies have relied solely on the amount of green space, relying on the Normalized Difference Vegetation Index (NDVI), but recent research suggests that health effects may vary depending on the type of green space. Therefore, this study aims to address this research gap by utilizing high-resolution land cover data to investigate the impact of various green space types on CA and AR risk during critical developmental periods.
    Method: This dissertation examined the association between green space exposure and two childhood health outcomes using different study designs tailored to each research question.
    Chapter 2 assessed prenatal green space exposure and CA using a case-control design using National Health Insurance claims data (2008-2013) among 142,422 children (35,629 cases and 106,793 controls, 3:1 matching). Green space exposure was measured at the administrative district level using both Landsat satellite NDVI and high-resolution land cover maps, and our study distinguished between artificial and natural green spaces. The statistical method used was generalized estimating equations, accounting for clustering within districts.
    Chapter 3 examined the association between childhood green space exposure and AR using longitudinal data from the Environment and Child Development Cohort (n=303). Individual exposure to green space was assessed at birth, 4 years of age, and 8 years of age within 250 m, 500 m, and 1,000 m of their residence using NDVI and land cover maps. Mixed-effects logistic regression models and logistic regression models were used to analyze associations across age groups and identify vulnerable developmental periods.
    Result: Chapter 2 analyzed 142,422 children from seven major Korean cities and found that higher built green space exposure at birth was associated with reduced risk of nervous system anomalies (adjusted odds ratio per 1-unit increase: 0.97, 95% CI: 0.94-0.99). Sensitivity analyses by quartiles demonstrated significantly lower risk in the third and fourth quartiles compared to the first quartile, though the trend test was not significant. After applying false discovery rate correction for multiple testing across CA subtypes, associations did not maintain statistical significance, necessitating cautious interpretation.
    Chapter 3 found that among 303 children followed to age 8, an IQR increment in total green space at age 4 was associated with significantly reduced odds of AR (adjusted odds ratio: 0.58, 95% CI: 0.35-0.95). This protective effect was primarily driven by built green spaces (adjusted odds ratio: 0.62, 95% CI: 0.41-0.95), while natural green spaces showed no significant association. Green space exposure at birth and age 8 showed no significant associations with AR risk.
    Conclusion: These findings have important implications for urban planning and public health policy. The protective effects of built-in green spaces, including parks, residential landscapes, and other accessible green spaces, highlight the importance of creating and maintaining accessible green infrastructure for urban residents, especially pregnant women and families with young children. Urban planning strategies should prioritize the development of accessible, well-designed green spaces near residential areas, rather than simply increasing overall vegetated cover. Future research should investigate the mechanisms underlying these type-specific effects, investigate optimal green space characteristics for various health outcomes, and explore how factors such as green space quality, maintenance, and use patterns influence health benefits in different urban environments.
