미국 장기요양보허정책의 운영 경험 및 개혁 논의의 시사점 = An Analysis of Long-Term Care Policy in the United States: Lessons for Korea

다국어 초록 (Multilingual Abstract)

Population aging is a worldwide issue and more so for Korea, thus it is very important to formulate appropriate solutions based on an in-depth analysis of the experiences of the developed countries.
The purpose of this paper is to analyze the experiences and the reform debate of long-term care in the United States and to draw valuable lessons for Korea. When introducing the public long-term care program in 2007, Korea should seriously consider key policy issues that include the philosophy and goals, public-private partnership, institutional vs noninstitutional services, role of informal care-givers, integration of acute and long-term care, the administrative form, the share between a self-governing body and an insurer, serving both the elderly and non-elderly disabled, assurance of high-quality care, and expenditure controls.

국문 초록 (Abstract)

본 연구는 장기요양보호의 역사가 오래 된 미국을 대상으로 제도의 결함을 시정하기 위한 개혁 논의과정에 초점을 맞춰 장기간에 걸친 운영 경험을 심층적으로 분석한 다음, 우리에게 시사하는 바가 큰 교훈과 정책과제를 도출하기 위해 수행되었다. 여타 선진국과 구별되는 미국 장기요양보호정책의 가장 큰 특징은 역사가 매우 오래되었음에도 불구하고 고령화와 장애에 의한 사회적 위험을 별도로 다루는 독립적인 단일 제도가 없고, 노인 대상의 사회 정책적 차원에서 도입된 각종 관련 법규와 정책에 근거하여 만들어진 분립적이고 다원화된 프로그램을 통해 발전되어 왔다는 점이다. 사회적 환경과 역사적 배경이 다른 우리로서는 미국의 경험에서 얻은 교훈을 그대로 적용하는 것은 무리가 따르며, 오랜 기간에 걸쳐 축적된 미국 제도의 장단점을 우리의 특성과 상황에 맞춰 취사선택하고 시행착오를 면밀하게 분석한 결과를 타산지석의 교훈으로 삼아야 할 것이다.
