Food dependent exercise induced anaphylaxis has been recognized a variant of exerciseinduced anaphylaxis, since My.ulitz reported one case of anaphylaxis induced by ingestion of shellfish followed by exercise in 1979. We experienced a patient who developed anaphylaxis induced by ingestion of Wheat flour made food followed by exercise postprandially from 30 min to 2 hr, several times for 4 years but he developed no attack with wheat flour not followed by exercise postprandially. His skin reactivities to the inhalant allergens, Dermatophagoides pteronyssinus and D. farinae, and other several dusts, foods, such as lobster, cracod, buckwhest showed positive and wheat flour and bread strong positive. Among 4 staps of provocation test, 1st oral challenge test with wheat flour, 2nd exercise test, 3rd oral challenge test with honspecific food which was not made of wheat flour followed by exercise test, and 4th oral challenge test with wheat flour followed by exercise test, neither first 3 steps of provocstion tests, but only last, fourth provocation test showed positive response. We confirmed this case ss food(wheat flour) dependent exercise induced qnaphylaxis by provocation test.
