    중환자실 환자에게서 발생하는 욕창은 회복을 지연시키고 감염 및 패혈증과 같은 합병증을 유발하여 의료비와 간호 업무 부담을 크게 늘리는 대표적인 입원 합병증이다. 임상에서는 Braden Scale 등 여러 욕창 위험도 평가 도구가 활용되고 있으나, 평가자 간 일치도가 낮고 시간에 따라 변하는 환자 상태를 충분히 반영하지 못한다는 한계가 있다.
    본 연구는 서울 소재 상급종합병원 중환자실에 입원한 성인 환자 36,340명의 5년간(2019~2023년) 전자건강기록 데이터를 활용한 후향적 코호트 연구이다. 인구학적 특성, 활력징후, 혈액검사, 의식수준, 약물, 수술 정보 등 98개 변수를 수집하였으며, 총 437,991개의 shift-level 관찰 데이터를 분석하였다. 시간가변 Cox 모형, Random Survival Forest, RF-SLAM, BoXHED 2.0 등 4개의 생존분석 모델을 구축하고 성능을 비교하였다. 모델의 판별력은 C-index, 시간 의존적 AUC로, 보정력은 Integrated Brier Score로 평가하였다. Variable Importance와 SHAP 분석을 통해 주요 예측 변수와 설명가능성을 확보하였다.
    연구결과, BoXHED 2.0 모델이 C-index 0.8604로 가장 우수한 판별력을 보였으며, 보정력 또한 IBS 0.1598로 가장 뛰어난 성능을 나타냈다. 시간 의존적 AUC 분석 결과, 입실 초기(3~7일)에 모델의 예측력이 가장 높았다(BoXHED 2.0: Day 3 AUC 0.819). 변수 중요도 분석 결과, Albumin의 변동성과 평균값이 가장 중요한 예측변수로 확인되었으며, CRP, PT_INR 등이 주요 위험인자로 식별되었다. SHAP 분석을 통해 각 변수가 욕창 발생 예측에 미치는 영향의 방향성과 크기를 정량화하였으며, Albumin 수치가 낮을수록, CRP와 PT_INR이 높을수록 욕창 발생 위험이 증가하는 것으로 나타났다.
    본 연구는 머신러닝 기반 생존분석이 중환자실 환자의 욕창 발생을 높은 정확도로 예측할 수 있으며, 설명가능 인공지능을 통해 임상 현장에서 모델의 예측 근거를 이해할 수 있음을 입증하였다. 개발된 예측모델은 고위험 환자를 조기에 식별하고 맞춤형 예방 전략을 수립하는 데 활용될 수 있으며, 간호사의 의사결정을 지원하고 제한된 의료 자원을 효율적으로 배분하는 데 기여할 것으로 기대된다. 향후 다기관 검증 연구와 실시간 임상 의사결정 지원시스템을 구축하여 모델의 실제 임상 효과를 평가하는 연구가 필요하다.

    Pressure injuries are common complications in intensive care unit (ICU) patients that delay recovery and increase healthcare costs. Current risk assessment tools such as the Braden Scale have limitations including low inter-rater reliability and inability to capture dynamic changes in patient conditions. This study aimed to develop machine learning-based survival analysis models for predicting pressure injury development in ICU patients using electronic health record (EHR) data and to provide model explainability through explainable artificial intelligence techniques.
    This retrospective cohort study analyzed EHR data from 36,340 adult ICU patients admitted to a tertiary hospital in Seoul over a 5-year period (2019~2023). A total of 98 variables including demographics, vital signs, laboratory tests, consciousness levels, medications and surgical information were collected, comprising 437,991 shift-level observations. Four survival analysis models were developed and compared: time-varying Cox model, Random Survival Forest, RF-SLAM, BoXHED 2.0. Model performance was evaluated using C-index and time-dependent AUC for discrimination, and Integrated Brier Score for calibration. Variable importance and SHAP analysis were conducted to identify key predictive variables and ensure model explainability.
    The BoXHED 2.0 model demonstrated superior performance in both metrics, achieving the highest discrimination with a C-index of 0.8604 and the best calibration with an Integrated Brier Score (IBS) of 0.1598. Time-dependent AUC analysis revealed that predictive performance was highest during the early ICU stay (day 3~7), with BoXHED 2.0 achieving an AUC of 0.819 on day 3. Variable importance analysis identified serum albumin variability and mean values as the most important predictors, followed by CRP, PT_INR. SHAP analysis quantified the direction and magnitude of each variable’s impact on pressure injury prediction, revealing that lower albumin levels and higher CRP and PT_INR values were associated with increased risk.
    This study demonstrated that machine learning-based survival analysis can accurately predict pressure injury development in ICU patients with high discrimination and that explainable AI techniques enable understanding of the model’s prediction rationale in clinical settings. The developed prediction models can be utilized to identify high-risk patients early and establish personalized prevention strategies, supporting nurses’ decision-making and facilitating efficient allocation of limited healthcare resources. Future research should focus on multi-center validation studies and implementation of real-time clinical decision support systems to evaluate the actual clinical impact of these models.
