    Background/Aims: This study aimed to develop a forward head posture orthosis (FHPO) designed to correct forward head posture (FHP) and to evaluate its clinical effectiveness compared to a conventional Philadelphia collar. Methods: Eighteen participants with FHP, defined as a craniovertebral angle (CVA) ≤48° and craniorotation angle (CRA) ≥ 143°, were randomly assigned to either the FHPO group (n=9) or the Philadelphia collar group (n=9). Participants wore their assigned orthosis for 3 hours daily over 4 weeks. Postural parameters including CVA, CRA, shoulder angle (SA), and forward head distance (FHD) were measured before, after 2 weeks, and after 4 weeks of use. Results: After 4 weeks, significant intergroup differences were observed in CVA, SA, and FHD (p<0.05). The FHPO group showed improvements in CVA and SA at 2 weeks and in CRA and FHD at 4 weeks, whereas the Philadelphia collar group demonstrated significant changes in CVA, CRA, and FHD only after 4 weeks, with no improvement in SA. Conclusion: The FHPO provided faster and greater corrective effects on FHP than the Philadelphia collar. It is considered an effective orthosis for postural correction, offering superior biomechanical improvement along with comparable comfort and user compliance.
