국가 건강검진 기관 전문평가 수행 = Implementation of qualityassessment in national health screening center

국문 초록 (Abstract)

본 연구는 국가 건강검진기관 전문평가를 수행하여 검진기관 전문평가 방안 및 검진기관의 질 개선방안을 마련하기 위하여 수행되었음. 구체적인 연구목표는 국가 검진기관 전문평가 지침서의 타당성 검증을 위한 시범적용 수행과 수정․보완된 평가 항목 및 지침에 의거하여 검진기관 전문평가를 수행하는 것임. 검진기관 전문평가 시범적용은 총 10개 기관을 대상으로 수행되었으며, 시범적용 후 평가 항목 및 평가 수행시 개선방안을 도출하였음. 시범적용 이후, 본평가는 총 100개 기관을 대상으로 수행되었으며, 평가대상기관의 선정은 건강보험공단의 검진기관 일반평가 결과와 연간 검진건수에 따른 질환의심판정률, 환수자료, 검진기관의 지역분포를 고려하여 질병관리본부에서 선정하였음. 평가대상 기관수는 검진기관 유형별로 일반(생애전환기) 40개소, 영유아 30개소, 구강 20개소, 출장 10개소이었음. 검진기관 전문평가의 평가문항 구성은 일반(생애전환기) 및 출장검진기관이 각각 90개, 94 개 문항으로 평가항목은 신체계측과정, 혈압측정과정, 시력측정과정, 청력측정과정, 영상촬영 과정, 검체채취 및 관리과정, 상담실시과정(출장 검진기관은 구강검진과정)으로 구성됨. 영유아 검진기관의 평가문항은 총 20개 문항으로 진찰, 신체계측, 상담 및 교육, 발달과정, 구강 검진기관은 총 9개 문항, 진찰 및 상담, 검사, 구강보건교육과정으로 구성되었음. 평가방법은 일정 자격요건을 갖추고 평가위원 교육을 이수한 평가위원이 검진기관을 방문 조사하여 평가를 수행하였음. 평가문항별 중요도에 따라 가중치를 부여하여 평균 점수를 산정한 결과, 일반(생 애전환기)검진기관 92.0점, 영유아 검진기관 97.8점, 구강 검진기관 91.0점, 출장 검진기관 93.1 점이었음. 향후 국가 건강검진기관 전문평가 수행시에는 전문평가의 목적에 따른 평가문항 및 평가 척도의 개선이 요구되며, 전문평가의 평가주기가 명확히 설정되어져야 할 것임. 또한 평가 수행 방법과 자료수집에 대한 법적 검토를 통해 효율적인 평가방법이 고안되어져야 할 것임.

다국어 초록 (Multilingual Abstract)

The objectives of this research are to develop a more effective methodology for the future researches and ultimately to find improvement points in service quality of national health screening centers by conducting service quality evaluation researches. Pilot researches will be conducted in order to verify the validity of evaluation items and research guidelines, and to find improvements points, which will be reflected in the main research hereafter. Pilot researches were conducted in ten national health screening centers which produced several improvement points for main researches. After the pilot researches, the main researches were conducted in a hundred national health screening centers: forty general physical examination centers, thirty physical examination centers for infants and children, twenty oral health examination centers, and the on-site physical examination centers. These subjects were selected based on the NHIC’s evaluation research results, the number of annual examination cases, suspected disease judgment rate considered along with the number health examination cases, redemption data and the aspects of distribution of regional health screening centers. Regarding evaluation items, there are ninety questions for general physical examination centers, and ninety four questions for on-site physical examination centers. Evaluation categories are composed of physical measurement, blood pressure, sight measurement, auditory measurement, laboratory management, chest raiodiology examination and consultation. Additionally, there are twenty questions for physical examination centers for infants and children which compose of several categories: checkups, physical measurement, developmental examination and consultation. Lastly, there are nine questions for oral health examination centers composing of several categories checkups, consultation, dental examination and oral health education. Evaluation committee members should have work experience in the health screening centers and they were ordered to take the training courses before beginning the evaluation. Evaluation committee members conducted on-site inspection. In considering the weight on depending on the importance of the assessment items, the results of calculating the average score is 92.0 mark in physical examination center, 97.8 mark physical examination center for infants and children, 91.0 mark oral health examination center, 93.1 mark in on-site physical examination center. For the main researches in the future, questions and scales in the evaluation sheet should be revised and improved. Additionally, the evaluation interval should be clearly fixed. Lastly, data collecting process and evaluation criteria should be more clarified based on legal reviews.
