영국의 커뮤니티 케어 정책의 역사적 변천과 쟁점: 노인케어의 혼합경제를 중심으로 = Process of Development and Its Issues of Community Care Policy for Older People in the UK: Focused on a Mixed Economy of Care

다국어 초록 (Multilingual Abstract)

This article examines the origin and development of community care policy for older people in the UK and to draw out some of the policy implications of community care in Korea. During ‘consensus politics’ between the Conservative and the Labour, community care was major objectives of social policy and the public sector played main role in health and social care provision. Since the mid-1970s, the welfare system was restructured and the Conservatives pushed extensive neoliberal reforms. Consequently, during ‘conflict politics’ of Thatcherism, despite the fact that community care was further emphasized for the purpose of economic benefits, it happened a massive growth of private sector residential and nursing homes in the 1980s and early 1990s. This irony was partly caused by Thatherite politics to curtail the role of local authorities and to keep down the influence of the Labour into them. The ‘new’ market-led community care allowed under the 1990 Act resulted in strengthening care gap and inequality of care resources. It suggests that the role of the public sector should be expanded to meet the social care needs of frail elders and the fostering of social capital at the local level will be needed to develop community care in Korea.

국문 초록 (Abstract)

본 연구는 영국에서 노인을 위한 커뮤니티 케어정책의 기원과 이차 대전 이후 1990년대까지의 전개과정을 역사적 관점에서 고찰하고, 커뮤니티 케어정책에 영향을 미친 사회경제적 조건과 정치적 이데올로기를 확인하고 한국의 커뮤니티 케어정책에 함의하는 바를 살펴보고자 한다. 영국의 구빈법과 자선활동에 기원을 둔 커뮤니티 케어는 영국의 복지국가 확립과 성장, 위기와 재편의 시기를 거치면서 확장기와 침체기, 면영화의 단계를 거쳐 왔다. 전후 30년 이상의 합의의 정치 시기에는 보수당과 노동당 모두 커뮤니티 케어의 발달이 일관된 정책목표였고, 지방정부의 직접적인 서비스제공과 재정 지원이 중요한 역할을 하였다. 1970년대 중반의 경제위기와 복지재편은 영국에서 복지다원주의 논쟁을 촉발시켰고, 국가와 시장뿐 아니라 자원부문과 비공식부문 간의 복지혼합이 더욱 강조되었다. 1979년 대처의 집권은 시장화와 민영화를 수반한 광범위한 개혁으로 복지영역에서 국가역할의 축소와 시장질서의 확대를 가져왔다. 보수당 정부의 공공지출 감축과 민간부문 선호로 커뮤니티 케어정책은 더욱 강조되었지만, 실제로 노동당을 견제하기 위한 대처의 강력한 중앙집권적 지방화정책의 결과, 1980년대와 90년대 초까지 민간 시설케어가 급성장하면서 정작 커뮤니티 케어는 후퇴하는 역설적 결과가 나타났다. 이를 시정하기 위한 1990년대 일련의 조치들은 시장 질서를 본격적으로 복지영역에 도입하면서 케어의 혼합경제를 강력히 추진하였다. 보수당 정부의 신자유주의적 개혁으로 재가서비스를 위한 자산조사와 비용청구가 증가되어 이용자들의 불안과 좌절을 낳았고, 케어제공의 지역적, 계층적 불균형이 심화되었다. 급속한 고령화 속도와 노년경험의 보편성, 케어의 상호성 등을 고려하면, 커뮤니티 케어를 단순히 노인문제를 해결하는 복지서비스의 일부로 협소하게 다루기보다는 21세기 고령사회의 핵심적 사회정책으로 재조명할 필요가 있다. 이런 점에서 영국에 대한 고찰은 케어서비스 시장에 민간참여가 가속화되고 질적 통제가 시급한 한국사회에 케어서비스의 질과 공공성을 확보하고 커뮤니티 내 사회적 자본을 확대할 필요성을 시사한다.
