국민건강영양조사 제7기 3차년도(2018) 구강검사 전문조사원 교육 및 질관리 = Standardization for Oral Health Survey in KNHANES (2018)

1. Purpose of the study
○ The purpose of this study is to ensure reliability of oral health survey through calibration training in KNHANES.
2. Methods
(1) Contents of the study
○ Ensuring and evaluation of reliability through calibration training

② Training using dental models
③ Simulation of oral health examination with human subjects
④ Field instruction and reproducing examination
⑤ Revision of the manual of oral health survey
3. Results
○ Appropriate reliability of examination was attained through calibration training. Nevertheless, effort for promoting the quality of examination should be continued.
○ Number of examiners should be reduced for the quality of examination. For this purpose, Korean Center for Disease Control and Prevention need to hire dentists as examiners.
○ Attitude of examiners is very important for the quality of examination. To raise attitude to appropriate level, number of trainee should be reduced further.
○ Duration of study should be matched with real survey period. Current administration-oriented study period doesn’t match with the real survey period of quality control.
○ Recruiting method for periodontal examinee needs to be modified by fortifying pre-examination screening.
○ Repair system for air compressor on Mobile Examination Car should be improved not to leave it in the status of malfunction for more than a day.

국문 초록 (Abstract)

1. 연구목표
○ 국민건강영양조사 구강검사의 수행과 관련하여, 구강검사 인력에 대한 교육훈련과 현장지도를 통해 조사의 신뢰도를 확보한다.
2. 연구 내용 및 방법
(1) 연구내용
○ 구강검사 조사자 교육훈련을 통한 조사기준 일치수준 확보 및 결과평가
○ 2018년 국민건강영양조사 조사지역 현장 정도관리
(2) 연구방법
① 검진 기준에 대한 이론교육
② 다양한 치아우식증과 보철 상황을 재현해 놓은 모형을 이용하여 교육
③ 피검자를 대상으로 모의구강검진을 시행 후, 신뢰도 평가
④ 교육훈련 피교육자에 대한 현장 지도 실시 및 결과평가
⑤ 구강검사 지침 보완
3. 연구결과
○ 국민건강영양조사 구강검사 전문조사원인 공중보건의 교육훈련 과정의 하나로 현장정도관리를 위한 구강역학 전문위원 교육훈련을 시행하였으며, 책임연구원인 최연희 교수를 Gold standard로 하여 검진결과 일치도를 측정하였으며, 그 결과는 치주병 유병자 기준 1차 평균 Kappa index가 0.700에서 2차 1.000으로 상승하였음.
○ 또한 기존의 지침서 뿐만 아니라 WHO기준의 검진지침 기준을 비교하여 개선방안을 모색 하였으며, 이를 7기 이내에 수정 보완하여 8기에 도입할 목표로 지침서를 검토하였음.
○ 구강검사 전문조사원 교육훈련은 총 2회에 걸쳐 시행되었으며, 1차는 2018년 하반기 구강검사 전문조사원 교육훈련, 2차는 2019년 상반기 구강검사 전문조사원 교육훈련을 목적으로 함. 그 결과, 치아상태 및 치료필요도 및 치주병 유병자 기준 Kappa index가 평균 0.7 이상을 보였음.
○ 구강역학 전문위원이 교육훈련 이후 공중보건의가 실제 검진현장에 투입되는 초기에 현장정도 관리를 진행하였으며, 검진일과 훈련기간의 차이가 클수록 다소 낮은 일치도를 보이기도 하였으나, 환류과정을 통하여 교육훈련 효과를 상기시키며 검진결과에 도움을 주었음.
○ 치주건강 측정 개선방안에 대한 모색으로 기계식 치주낭 측정검사 방법인 Florida probe의 사용법에 대하여 검토를 시행하였으며, 질병관리본부 소속 공중보건치과의에 한하여 일치도 확인 및 사용법 숙지 정도에 대하여 확인하였음.
○ 질병관리본부 소속 공중보건의 정기교육을 총 5회에 걸쳐 시행하였음. 이를 통해 구강검사 전문 조사원으로서 소속감을 고취시켰으며, 치주건강 측정 개선방안에 대한 양질의 의견을 수립함.
