배경: 대한진단검사정도관리협회(Korean Association of External Quality Assessment Service, KEQAS)는 정량검사 외부정도관리 프로그램의 이상치 검출을 위해 Tukey 방법을 사용하고 있다. 그 러나 Tukey 방법을 사용할 때 중앙값, 1사분위수 및 3사분위수가 모두 동일하여 사분위범위(interquartile range, IQR)가 0이 되는 상황이 발생할 수 있다. 이러한 경우 중앙값을 제외한 모든 값이 이상치로 분류되어 과도한 이상치 제외를 초래할 수 있다. 본 연구 290 www.labmedonline.org 는 KEQAS 외부정도관리 자료를 사용하여 이상치 검출을 위한 대 체 방법을 모색하고자 하였다.
방법: KEQAS에서 사용하는 Tukey 방법의 대체 방법으로 영국의 외부정도관리 프로그램에서 사용하는 Healy 방법을 선정하였다. Tukey 방법으로 이상치 판정 시 IQR이 0이 되는 경우가 빈번히 발 생하는 혈액가스검사 프로그램을 대상으로 두 이상치 검출 방법 을 비교하였다. 2023년 시행된 혈액가스검사 프로그램 자료 중 IQR 이 0인 65개 세분류에서 두 방법 간 이상치 비율을 비교하였다. 결과: 65개 세분류 중 31개 세분류는 Tukey와 Healy 방법으로 각 각 이상치를 검출했을 때 이상치 제외 후 남아 있는 결과가 모두 동일했으며, 두 방법 간 이상치 비율에 차이가 없었다. 나머지 34개 세분류에서는 Tukey 방법으로 검출한 이상치가 28.2 ± 8.0%였고, Healy 방법으로 검출한 이상치는 3.9 ±4.5%였다. 이 34개 세분류 중 17개 세분류는 Healy 방법이 Tukey 방법에 비해 통계적으로 유 의하게 낮은 이상치 비율을 보였다 (McNemar’s test, P < 0.05).
결론: Healy 방법은 KEQAS의 정량검사 외부정도관리 프로그램에 서 Tukey 방법의 한계를 극복하기 위한 실용적인 대안으로 활용될 수 있다.

Background: The Korean Association of External Quality Assessment Service (KEQAS) employs the Tukey method to detect outliers in quantita- tive proficiency testing (PT) programs. However, when the median, first quartile, and third quartile are identical, resulting in an interquartile range (IQR) of zero. This scenario can classify all values, except the median, as outliers, leading to excessive exclusion of data points. We aimed to ex- plore an alternative method for outlier detection by comparing different approaches using KEQAS PT data.
Methods: As an alternative to the Tukey method, we adopted the Healy method, which is used in the United Kingdom’s national PT programs. The Blood Gas Analysis (BGA) program, a representative program where the Tukey method frequently produces an IQR of zero, was selected as the simulation target. Outlier rates between the two methods were compared across 65 subcategories with an IQR of 0 in the 2023 KEQAS BGA data. Results: In the Healy method, all participants showed identical results in 31 out of 65 subcategories, indicating no difference in outlier detection between the two methods for these cases. In the remaining 34 subcategories, the proportion of outliers detected by the Tukey method was 28.2 ± 8.0%, whereas the Healy method detected only 3.9 ± 4.5%. Among these 17 subcategories exhibited a statistically significant lower outlier rate with the Healy method compared to the Tukey method (McNemar’s test, P < 0.05).
Conclusions: The Healy method may offer a practical alternative for addressing the limitations of the Tukey method for detecting outliers in quantitative PT programs.
