당뇨병성 신경병증은 당뇨 환자에서 이환율과 사망률을 증가시키는 심각한 합병증으로 병리학적으로 신경섬유의 축삭 위축이 진행하여 결국 신경섬유의 소실이 일어난다. 이 연구는 형태계측기를 이용하여 대두가 당뇨병성 신경병증에 미치는 영향을 살펴보기 위해서 Sprague-Dawley 흰쥐를 정상 대조군, 당뇨군, 대두식이 당뇨군으로 나누어 각각 4주와 8주에 실험을 실시하였다. 각 군의 혈당 및 HbA1c를 측정하고 유수초 신경섬유의 수를 세고 신경섬유 크기에 따른 분포를 평가하였으며, 광학현미경 및 전자현미경 소견을 비교하였다. 대두식이 당뇨군의 혈당은 실험 4주에는 정상 대조군과 당뇨군 사이로 조절되었고 8주에는 인슐린을 투여하지 않은 당뇨군에 비해 현저하게 감소하여 정상 대조군 수준으로 조절되었다. 유수초 신경섬유의 수는 4주에는 실험군 간에 차이가 없으나 8주에는 인슐린을 투여하지 않은 당뇨군은 정상 대조군에 비해 유의하게 감소하여 당뇨병성 신경병증에서의 축삭 소실을 나타내고 대두식이 당뇨군은 인슐린을 투여하지 않은 당뇨군에 비해 유수초 신경섬유의 수가 유의하게 많고 정상 대조군과는 차이를 보이지 않았다. 인슐린을 투여한 당뇨군과 투여하지 않은 당뇨군 간에 유수초 신경섬유의 수는 통계적으로 유의한 차이가 없었다. 신경섬유의 크기에 따른 분포를 살펴보면 당뇨군과 대두식이 당뇨군은 정상 대조군에 비해 크기가 작은 섬유의 비율이 더 많았다. 전자현미경 소견상 인슐린을 투여하지 않은 당뇨군은 유수초 신경 및 무수초 신경의 변성이 뚜렷하나 대두식이 당뇨군과 인슐린을 투여한 당뇨군은 정상 대조군과 마찬가지로 이러한 변성이 관찰되지 않았다. 이상의 결과를 종합할 때 대두식이가 당뇨병에서 혈당의 조절 및 당뇨병성 신경병증의 예방에 도움이 될 것으로 보인다.

Neuropathy is a serious and disabling complication that contributes to increased morbidity and mortality in diabetic patients. There is progressive distal to proximal axonal atrophy that ultimately leads to Wallerian
degeneration. This study was performed to identify the effect of soy bean on diabetic neuropathy using morphometry.
Male Sprague-Dawley rats were grouped into control, diabetic with red chow diet and diabetic with soy bean diet. The myelinated nerve fibers were counted and fiber size distributions were evaluated in each group at 4 and 8 weeks, respectively. Diabetic neuropathy didn’t develop in streptozotocin-induced diabetic rats at four weeks. At 8 weeks, the myelinated nerve fiber in diabetic with soy bean diet was larger in number than that in diabetic to which did not be administered insulin. The number of myelinated nerve fiber was not different between diabetic group with insulin and without insulin. Mean myelinated nerve fiber size was smaller in diabetic with soy bean diet than diabetic with red chow diet and control. Histogram of fiber size distribution was shifted to left in diabetic with red chow and soy bean diet groups compared to control. Light and electron microscopic findings showed marked degeneration of nerve fibers in diabetic with red chow diet but preservation in diabetic with soy bean diet. The level of glucose and HbA1c was lower in diabetic with soy bean diet than red chow diet. Soy bean could be effective in the protection of neuropathy induced by diabetes mellitus.
