This study investigated the relationship between regional CPR training completion rates and the provision of bystander CPR in out-of-hospital cardiac arrest (OHCA) cases in Korea. Using data from 33,402 OHCA patients recorded in the 2023 national registry, chi-square tests and multiple logistic regression were performed to analyze differences in CPR implementation by training rate, arrest location, witness status, and cause of arrest. The analysis revealed that regions with higher CPR training rates did not necessarily show higher bystander CPR rates. Compared to the lowest training group (Q1), Q2 and Q3 showed significantly lower CPR implementation (OR=0.70 and 0.57), while Q4 showed no significant difference. CPR was performed 41% less frequently in cases caused by non-cardiac factors such as trauma or poisoning, compared to cardiac-origin cases. These results suggest that CPR education should go beyond skill acquisition to include situational judgment and realistic response strategies. Future training programs and public health policy should aim to reduce regional disparities and promote effective CPR delivery in real-world emergencies.
