한국어판 음성장애지수(Voice Handicap Index, VHI)의 타당도 및 VHI-10의 임상적 유용성 = Validation of the Korean Voice Handicap Index(K-VHI) and the Clinical Usefulness of Korean VHI-10

국문 초록 (Abstract)

배경 및 목적: 본 연구에서는 한국어판 음성장애지수(Voice Handicap Index, VHI, 이하 VHI)-30 및 VHI-10의 신뢰도 및 타당도를 제시하고, VHI-10의 유용성을 확인하고자 하였다. 방법: 한국인 성인을 대상으로 후두암을 제외한 음성장애군 156명과 정상 성인 66명에게 내용타당도를 거친 VHI-30과 IPVI(Iowa Patient’s Voice Index)의 1번 문항을 설문지로 제공한 후 결과를 분석하였다. 결과: VHI-30의 검사-재검사 간 상관성이 높았고(r=.971, p<.01), Cronbach’s α 값은 .969를 보여, 신뢰도가 높은 검사임을 보였다. 환자군과 정상군 간에 뚜렷한 총점의 차이를 보였고, IPVI 1번 문항 간 상관성 또한 높은 편으로(r=.664, p <.01), 타당도가 높은 검사임을 보였다. 또한 요인분석 상 원본과 유사하게 세 가지 하위 영역으로 구분이 가능하였다. VHI-10 또한 VHI-30과 마찬가지로 신뢰도 및 타당도가 높은 검사임을 보였다. VHI-30과 VHI-10 간의 상관성이 매우 높았고(r=.968, p<.01), VHI-10 총점 대 VHI-30 총점 비율이 0.333보다 높은 0.368을 보였으므로, 임상에서 VHI-10의 유용성을 재확인하였다. 논의 및 결론: 한국어판 VHI-30 및 VHI-10 번안본에서도 신뢰도 및 타당도가 높아, 음성장애를 평가하는 도구로서 적절함을 보였다. 임상에서 시간 경제성 및 효용성을 위해서는 VHI-10을 대신 사용하는 것도 바람직하였다.

다국어 초록 (Multilingual Abstract)

Background & Objectives: The purpose of this study was to validate the Korean Voice Handicap Index(VHI) and VHI-10, and to evaluate the clinical usefulness of VHI-10. Methods: One hundred fifty-six Korean adult patients, all of whom had symptoms of dysphonia without laryngeal cancer, as well as 66 normal Korean adults participated in this study. The survey was conducted using VHI (30 items) and IPVI’s first item. Results: With regard to the reliability of VHI-30, Pearson correlations in test-retests were very high(r=.971, p<.01). Cronbach’s α for internal consistency was .969. The control group received significantly lower scores than did the patient group. With regard to concurrent validity, the Spearman correlation between the VHI-30 and IPVI’s was high (r=.664, p<.01). With regard to the construct validity, 30 VHI items were divided into three components via factor analysis. For VHI-10, statistical analyses revealed high reliability and validity values. The correlations of total scores between VHI-30 and VHI-10 was high (r=.968, p<.01). The mean ratio of total scores between VHI-10 and VHI-30 was 0.368. VHI-10 had a single component according to the results of factor analysis. Discussion & Conclusion: The validity and reliability of VHI-30 and VHI-10 questionnaires are maintained even after translation into Korean. The VHI-10 items are more sensitive than the VHI-30 items, consistent with the results of previous studies.
