목적 : 수술 중 경정맥구혈 산소포화도(jugular bulb venous oxygen saturation:SjvO₂) 의 지속적인 감시를 이용하여 마취 및 수술적 조작이 뇌산소 대사와 뇌혈류의 균형에 미치는 영향을 평가하였다.
방법 : 전신 마취 하에 개두술을 시행 받는 환자 20명을 대상으로 하여 우측 내경 정맥을 통하여 fiberoptic oximetry catheter를 우측 경정맥구에 위치시키고 SjvO₂를 지속적으로 감시하였다. SjvO₂는 마취 및 수술적 조작인 두피 절개, 두개골 절개, 경막절개, 뇌조직 견인, 경막 봉합, 두피봉합, 기관내 튜브의 분비물 흡인 및 기관내 튜브의 발관등의 의 조작 직전과 조작 후 1분, 3분, 5분 후에 측정하였다.
결과 : 각각의 수술적 조작에 따른 SjvO₂의 변화는 피부절개 후 1분, 3분, 5분에 모두 유의한 증가(P < 0.05)를 보였으며 두피봉합 후 1분과 기관내 튜브의 분비물 흡인 후 1분에 유의한 증가(P < 0.05)를 보였다
결론 : 뇌수술 시 SjvO₂의 지속적인 감시는 마취 및 수술적 조작으로 인한 뇌혈류량 변화의 조기감지에 매우 유용한 방법으로 사용할 수 있다는 결론을 얻었다.

Objectives: During intracranial brain surgery, numerous factors may alter cerebral blood flow and oxygen supply-demand balance. We evaluated the usefulness of continuous monitoring of jugular bulb venous oxygen saturation (SjvO₂) during anesthesia and brain surgery.
Methods: SjvO₂was continuously monitored before and recorded 1, 3, 5min after each beginning of procedure in skin incision, craniectomy, dura opening, dura closure, brain retraction, scalp repair, endotracheal suction and extubation in 20 patients during intracranial brain surgery.
Results: SjvO₂was increased 1, 3, 5 minutes after skin incision and 1 minute after scalp repair and endotracheal suction for removal of secretion.
Conclusions: Continuous monitoring of SjvO₂allows the early detection of cerebral blood flow and oxygen supply-demand imbalance due to anesthesia and surgical prodecure during brain surgery.
