장기요양 재원조달과 미국의 공ㆍ사 파트너십 = Public-Private Partnerships in Long-Term Care: Lessons from the United States

국문 초록 (Abstract)

본 연구는 미국이 장기간에 걸쳐 장기요양제도를 운영하는 과정에서 지속가능한 재원조달을 위해 도입한 공ㆍ사 파트너십 프로그램을 심층 분석한 후 우리에게 적합한 교훈과 정책적 시사점을 얻기 위한 목적으로 수행되었다. 공공부문이나 민간부문 그 어느 쪽에서도 단독으로 재원을 조달하기가 힘든 현실적 상황에서 양자의 적절한 협력관계에 초점을 맞춘 독특한 방식의 개혁 대안으로 주목받고 있는 것이 미국의 일부 주에서 시행되고 있는 공ㆍ사 파트너십 프로그램이다. 미국의 경우 오랜 기간에 걸친 장기요양체계의 역사에도 불구하고 만족스러운 지불체계를 갖지 못하고 있는데 장기요양 공ㆍ사 파트너십 프로그램은 이러한 문제점을 해결하기 위해 공공부문과 민간부문의 협력관계에 초점을 맞춘 장기요양 재원조달체계의 개혁 논의의 산물이라 할 수 있다. 중요한 정책적 함의와 시사점을 살펴보면, 무엇보다 제도의 원활한 연착륙을 유도하기 위해서는 장기요양 재원조달방식에 대해 관련 이해당사자의 충분한 논의 과정을 거쳐 사회적인 합의를 도출하는 절차가 선행되어야 할 것이다. 다음으로 장기요양 재원조달의 안정성과 지속가능성을 담보하기 위해서는 공공과 민간부문의 적절한 역할 분담 및 협력관계를 통한 재원의 다원화가 매우 중요하다. 이 과정에서 공ㆍ사 재원의 적정 분담과 비중을 어떻게 구성할 것인가에 대한 다각적인 검토가 필요한 데, 기본적인 원칙은 공공부문이 중심적인 역할을 담당하되 민간부문이 보완관계로 적극 활용되는 것이 바람직할 것이다. 그리고 공ㆍ사 파트너십을 통해 장기요양 재원을 확대하는 것 못지않게 비용 절감 노력과 지출의 낭비 요소를 줄이는 조치를 병행해야 하며, 특히 재원조달체계의 개혁은 장기 요양 전달체계와의 연계 하에 추진하는 것이 효율적이라 할 수 있다.

다국어 초록 (Multilingual Abstract)

With the aging of our population, the sustainable financing of long-term care for the elderly is a worldwide issue and more so for Korea. The public-private partnership program in the United States is an innovative approach for long-term care financing that rests on the combination of the public and the private sectors. The purpose of this study is to analyze the experiences and the performance of the partnership for long-term care in the United States and to draw valuable lessons for Korea. Major suggestions are as follows. First, when introducing the national long-term care system, Korea should reach a social agreement after discussing the optimal financing model for long-term care fully. Second, in designing the financing system for long-term care, the government should seriously consider the appropriate share of public and private responsibilities including the role extension of private sectors. Third, to ensure the long-run stability of long-term care financing, it should be taken various measures to control the wasteful expenditures along with the increase of total funds. Finally, to improve the performance, it is essential to reform long-term care financing in connection with service delivery.
