항생제의 발견은 의학 발전에 큰 영향을 미쳤으나 과도한 항생제 사용은 항생제 내성 증가로 이어져 전 세계적인 공공 보건 위기를 초래하고 있다. 이러한 문제를 해결하기 위한 전략으로 항생제 적정 사용을 목표로 하는 항생제 스튜어드십(Antimicrobial Stewardship, AMS)이 도입되었다. 항생제 스튜어드십은 다학제적 팀 기반의 접근이 필수적이나 국내에서는 다양한 장애 요인으로 인해 간호사의 참여가 제한적인 실정이다. 이에 시스템 엔지니어링 환자 안전 모델(Systems Engineering Initiative for Patient Safety, SEIPS)을 기반으로 종합병원 간호사의 항생제 스튜어드십 수행에 영향을 미치는 개인적 요인과 환경적 요인을 분석하여 간호사의 역할 정립과 참여 활성화를 위한 실질적인 근거자료를 제공하고자 한다. 본 연구의 대상자는 G광역시 소재 2개 종합병원과 J도 소재 2개 종합병원에서 근무하는 간호사 165명으로, 자료수집 기간은 2025년 8월 20일부터 8월 29일까지 진행하였다. 자료 분석은 SPSS Statistics 29.0 프로그램을 사용하여 빈도와 백분율, 평균과 표준편차를 산출하였으며, 집단 간 차이를 비교하기 위해 Independent t-test, one-way ANOVA를 실시하였다. 사후 검정은 Pearson’s correlation analysis 및 Hierarchical Regression analysis로 실시하였고, 항생제 스튜어드십 중요도 인식과 수행도 간의 관계를 시각화하기 위해 중요도-수행도 분석(Importance-Performance Analysis, IPA)을 시행하였다. 측정 도구의 내용 타당도는 내용 타당도 지수(CVI)를 통해 검증하였고, 신뢰도는 KR-20과 Cronbach’s α 계수를 이용하여 분석하였다. 유의수준은 α = 0.05로 설정하고 양측 검정을 통해 분석하였다. 본 연구의 결과는 다음과 같다. 1. 대상자는 총 165명으로 대부분 여성(93.3%) 이었으며, 평균 연령은 31.69±6.08세였다. 대상자의 근무부서는 일반병동 117명(70.9%), 중환자실 및 응급실 48명(29.1%) 이었으며, 평균 임상경력은 7.49±6.26년이었다. 2. 종합병원 간호사의 항생제 사용과 관련된 지식 점수는 평균 0.78±0.10점이었고, 전체 정답률은 77.7%로 나타났다. 항생제 스튜어드십 중요도 인식은 5점 만점에 평균 4.48±0.41점이었고, 세부적으로는 환자 교육 및 지지 (4.66±0.47)가 가장 높았고, 검사 시행 및 항생제 투여(4.63±0.45), 환자 관찰(4.55±0.43)순으로 나타났다. 3. 항생제 스튜어드십 관련 환경적 요인은 조직 영역이 3.90±0.69점으로 가장 높았으며 작업 영역이 2.73±0.78점으로 가장 낮았다. 4. 종합병원 간호사의 항생제 스튜어드십 수행도는 5점 만점에 평균 4.08±0.53점이었고, 검사 시행 및 항생제 투여(4.43±0.53)가 가장 높았으며, 다음으로 환자 교육 및 지지(4.42±0.60), 환자 관찰(4.21±0.54) 순으로 나타났다. 5. 개인적 및 환경적 요인과 항생제 스튜어드십 수행도 간의 단변량 분석 결과 항생제 스튜어드십 중요도 인식은 수행도와 높은 정적 상관관계를 보였으며 (r=.68, p<.001), 환경적 요인 중 기술 및 도구(r=.39, p<.001), 조직(r=.49, p<.001), 작업(r=.21, p=.006), 물리적 환경(r=.37, p<.001) 모든 하위 영역이 수행도와 유의한 정적 상관관계가 있는 것으로 나타났다. 6. 종합병원 간호사의 항생제 스튜어드십 관련 수행에 영향을 미치는 요인을 확인하기 위해 위계적 회귀분석을 시행한 결과, 최종적으로 중요도 인식(β=.56, p<.001), 환경 요인 중에서는 조직 영역(β=.18, p=.028)으로 나타났다. 최종 모형에서 변수들에 의한 설명력은 52.6%였고, 통계적으로 유의하였다(Adj R2=.526, F=37.38, p<.001). 7. 회귀분석 결과, 종합병원 간호사의 항생제 스튜어드십 수행에 영향을 미치는 개인적 요인 중 ‘중요도 인식’이 수행에 유의한 영향 요인으로 확인되어 중요도 인식과 수행도 간의 관계를 구체적으로 파악하고, 영역별 차이를 확인하기 위해 중요도-수행도 분석(Importance-Performance Analysis, IPA)을 실시 하였다. 2사분면(집중 개선 영역)에는 의사와 협의 및 제안 활동 2개 문항(3.9%)이 포함되었고, 3사분면(점진 개선 영역)에는 환자 관찰, 의사와 협의 및 제안, 검사·투약, 교육, 다학제 조정 영역의 19개 문항(37.3%)이 분포하였다. 결론적으로, 본 연구는 종합병원 간호사의 항생제 스튜어드십 수행에 영향을 미치는 요인으로는 개인적 요인 중 중요도 인식과 환경적 요인 중 조직(Organization) 영역이 확인되었다. 따라서 항생제 스튜어드십 활성화를 위해서 는 조직 차원의 리더십과 정책적 지원 강화, 명확한 보고·의사소통 체계 확립, 그리고 간호사의 역할 수행을 뒷받침할 인적·구조적 지원 환경 조성이 필요하다. 아울러 간호사가 항생제 관리 활동의 중요성을 인식하고 적극적으로 참여할 수 있도록 체계적이고 지속적인 교육 프로그램을 마련하는 것이 중요하다. 주요어: 항생제 스튜어드십 (AMS), 종합병원 간호사, 수행도, 환경적 요인, SEIPS 모델

The discovery of antibiotics has greatly contributed to advances in medicine; however, excessive antibiotic use has resulted in a public health crisis worldwide. Antimicrobial stewardship (AMS), which emphasizes the appropriate use of antibiotics, has been introduced as a key strategy to address this problem. Although a multidisciplinary team-based approach is essential for effective AMS implementation, nurses’ participation in AMS remains limited in Korea due to various barriers. Therefore, this study aimed to analyze personal and environmental factors influencing AMS performance among general hospital nurses based on the Systems Engineering Initiative for Patient Safety (SEIPS) model and to provide practical evidence to support role clarification and enhanced nurse participation. The participants of this study were 165 nurses working at two general hospitals in G Metropolitan City and two general hospitals in J Province. Data were collected from August 20 to August 29, 2025. Data analysis was conducted using SPSS Statistics version 29.0. Frequencies, percentages, means, and standard deviations were calculated. Independent t-tests and one-way analysis of variance were performed to examine differences between groups. Pearson’s correlation analysis and hierarchical regression analysis were conducted, and Importance–Performance Analysis (IPA) was used to visualize the relationship between perceived importance and AMS performance. Content validity of the measurement tools was evaluated using the Content Validity Index (CVI), and reliability was assessed using KR-20 and Cronbach’s α coefficients. Statistical significance was set at α = .05 using two-tailed tests. The results of this study are as follows: 1. A total of 165 nurses participated in the study, the majority of whom were female (93.3%). The mean age of the participants was 31.69 ± 6.08 years. Regarding work units, 117 participants (70.9%) worked in general wards, while 48 (29.1%) worked in intensive care units or emergency departments. The mean length of clinical experience was 7.49 ± 6.26 years. 2. The mean score for nurses’ knowledge related to antibiotic use was 0.78±0.10, with an overall correct answer rate of 77.7%. The mean perceived importance of antimicrobial stewardship was 4.48±0.41 on a 5-point scale. Among the subdomains, patient education and support showed the highest score (4.66±0.47), followed by implement tests and antimicrobial administration based on physician’s order (4.63±0.45) and observation of patients (4.55±0.43). 3. Among environmental factors related to antimicrobial stewardship, the Organization domain showed the highest mean score 3.90±0.69, whereas the Tasks domain showed the lowest mean score 2.73±0.78. 4. The mean antimicrobial stewardship performance score was 4.08±0.53 on a 5-point scale. Performance was highest for implement tests and antimicrobial administration based on physician’s order (4.43±0.53), followed by patient education and support (4.42±0.60) and observation of patients (4.21±0.54). 5. Univariate analysis showed that perceived importance of antimicrobial stewardship was strongly positively correlated with performance (r = .68, p < .001). In addition, all environmental subdomains, including Technology and Tools (r = .39, p < .001), Organization (r = .49, p < .001), Tasks (r = .21, p = .006), and Physical Environment (r = .37, p < .001), were significantly positively correlated with antimicrobial stewardship performance. 6. Hierarchical regression analysis was conducted to identify factors influencing antimicrobial stewardship performance among general hospital nurses. In the final model, perceived importance (β = .56, p < .001) and the Organization domain among environmental factors (β = .18, p = .028) were identified as significant influencing factors. The final model explained 52.6% of the variance in antimicrobial stewardship performance and was statistically significant (Adjusted R² = .526, F = 37.38, p < .001). 7. Based on the regression results identifying perceived importance as a significant personal factor influencing antimicrobial stewardship performance, Importance–Performance Analysis (IPA) was conducted to further examine the relationship between perceived importance and performance and to identify domain-specific differences. In Quadrant II (Concentrate Here), two items (3.9%) related to checking with/suggesting to physician based on assessments were identified. In Quadrant III (Low Priority), 19 items (37.3%) were distributed across the domains of observation of patients, checking with/suggesting to physician based on assessments, implement tests and antimicrobial administration based on physician’s order, patient education and support, and coordinating multiple disciplines and institutions. In conclusion, perceived importance as a personal factor and the Organization domain as an environmental factor were identified as key determinants of antimicrobial stewardship performance among general hospital nurses. To enhance antimicrobial stewardship, strengthening organizational leadership and policy support, establishing clear reporting and communication systems, and creating supportive structural and human resource environments for nursing practice are essential. In addition, systematic and continuous educational programs are needed to improve nurses’ awareness of antimicrobial stewardship and to promote their active participation in antimicrobial management activities.
