성인 여성의 비만, 자존감과 우울증상 간의 관계 = The Relationship between Obesity, Self-esteem and Depressive Symptoms of Adult Women in Korea

국문 초록 (Abstract)

연구배경: 비만한 여성일수록 우울증 발병률이 높고, 우울한 여성일수록 비만해지기 쉽다는 연구들이 보고되었으나, 두변인 간의 관계는 명확히 밝혀지지 않고 있다. 이에 본 연구는국내성인 여성을 대상으로 비만과 우울증상 간의 상호 관련성을 파악하고, 비만과 우울증상 간의 관계에서 자존감의 영향을확인하고자 시도되었다.
방법: 2009년 9월부터 11월까지 서울, 경기, 충청지역에 거주하는 만 20세 이상 성인여성을 임의표출하여 연구참여에 동의한 455명이 최종 분석대상이었다. 체질량지수(Body mass index, BMI)는 키와 체중을 이용해 산출하여, 25k g/m2 이상은 비만으로 분류하였고, 자존감은 10문항 4점 척도의 자가보고형 측정도구로 사용하였고, 우울증상 유병률은 한국판CES-D 도구를 이용해 절단점 21점을 기준으로 하여 분류하였다. 비만, 자존감 및 우울증상 간의 상호 관련성은 다변량 로지스틱 회귀분석을 통해 확인하였다.
결과: 비만과 우울증상 유병률은 각 23.7% (108명)와21.5% (98명)였고, 비만도에 따른 자존감 및 우울증상 유병률차이에서, 비만 여성이 정상 또는 과체중 여성에 비해 자존감은 낮고(P =.003) 우울증상 유병률은 높았다(P =.001). 인구사회학적 특성을 보정한 후 다변량 로지스틱 회귀분석결과, 비만과 낮은 자존감이 우울증상의 영향요인으로 확인되었다(P =.009, P < .001).
결론: 비만과 자존감이 우울증상 발생의 영향 요인임이 확인된 바, 향후 비만한 여성과 자존감이 낮은 여성들을 대상으로 우울증상 여부를 확인하는 것이 필요하겠다.
