보건의료서비스 전달체계와 성과의 관계 탐색 = Searching for the Relationship between Public Service Delivery System and Performance in the Field of Health Care Service

국문 초록 (Abstract)

본 논문은 보건의료서비스 전달체계의 제도적 배열과 성과 사이의 가능한 상관관계를 탐색하였다. 각국의 정부가 보건의료서비스 공급을 위해 공공부문과 민간부문 당사자들의 협력 및 계약 관계를 어떻게 설정하고 조정하는지， 즉 제도적 배열이 어떠한지에 따라서 보건의료의 성과가 다르게 나타남을 분석하였다. 보건의료서비스 전달체계의 대표적인 유형으로 꼽히는 미국 영국 독일 한국을 선정하고 비교 분석을 진행하였다. OECD 데이터를 활용하였으며 제도적 배열은 재정 서비스공급 규제의 차원으로 나눠 하위지표를 구성하였고 성과 측면은 효과성， 효율성， 형평성으로 나누어 그 관계성을 조사하였다- 분석 결과， 효과성 측면에서는 상대적으로 미국이 낮게 나타났으며 효율성의 측면에서는 한국， 형평성 측면에서는 영국과 독일이 상대적으로 높고 미국은 낮은 수준을 보이고 있어， 특정 전달체계의 포괄적 우월성을 주장하는데 한계가 있음을 확인하였다. 한편， 개별 제도의 성과적 특성을 볼 때 최근 한국에서도 도입이 논의되는 문지기 제도는 보건의료서비스의 효과성과 형평성을 제고하지만 효율성은 낮출 수 있다고 추론되었다 이러한 결과는 공공서비스 공급의 제도적 배열 양태가 다양한 성과 차원과 연결될 수 있음을 경험적으로 서술하였다는데 의의가 있다.

다국어 초록 (Multilingual Abstract)

This study investigates the relationship between institutional arrangement, -including financing, services, and regulation, and its performance in terms of the public health care system. This study researches well-known types of health care systems- NHS, SHI, NHI, and Liberal system and compares the health care system`s institutional arrangement, with a focus on those health care systems in the U.S, D.K, Germany, and Korea, since these countries are representative examples of each model In previous studies, health spending, infant mortality and life expectancy were used to measure the health care system`s performance. However these measurements are not sufficient to comprehensively evaluate health care performance because these variables measure only specific aspects of output. So we measured the health care system performance by not only just considering the quantitative variables but also focusing on the qualitative aspects using the OECD data. From the results there is no one-size-fits-all institutional arrangement for performance and institutional arrangement needs to be studied based on the context. Also the results suggest the possible relationship between specific institutional arrangement and performance in the field of health care service.
