인공진피(Terudermis.)와 부분층 피부이식을 이용한 전완피판 공여부 수복 = Application of artificial dermis(terudermis.) and split thickness skin graft on the donor site of radial forearm flap

국문 초록 (Abstract)

대한악안면성형재건외과학회지: Vol. 29, No. 3, 2007228의 재건에 널리 사용되고 있다. 반면에 단점으로는 공여부의 피부결손부위에 피부이식이필요한데 이식피부의 생착여부, 인대의 노출, 이식피부의수축, 변색 등에 의한 비심미성 등이 있다. 공여부 창상의치유지연은 이식 피부의 생착 실패가 주된 원인으로 하부굴곡인대의 외전운동이 제한되며 이를 극복하기 위하여 굴곡인대 paratenon의 조심스러운 보호가 중요한 것으로 알려져 있다. 만약 피부이식이 실패하면 flexor pollicislongus와 flexor digitorum superficialis가 피부 밖으로노출되어 굴곡과 잡는 힘의 감소 등 기능장애를 유발할 수있다3).이러한 단점을 극복하기 위하여 paratenon의 보호, 주위근육을 이용한 인대의 보호, 전층피부이식4), V-Y 봉합술의이용5,6), 조직확장기 사용7,8), 동종진피와 피부이식의 사용9)등이 보고되고 있으나 아직도 많은 임상적 방법들의 개발이필요하다.
